Etiology ：

链球菌 65% subacute— 草绿色

葡萄球菌 25% acute— 金黄 少见微生物为肺炎球菌，淋球菌，流感

杆菌，肠球菌，表皮葡萄球菌，真菌及立克次体、衣原体。

·subacute 未经治疗：第 1 日间隔 1 小时采血 3 次培养 次日无细菌生长再采血 3 次即开 始治疗 已用抗生素：停药 2—7 日采血治疗；·acute ：入院即采血 3 次，之后马上治疗。

经食道 95%

Cardiac complication ：瓣膜关闭不全、穿孔、腱索断裂、瓣周脓肿、心包积液

Duke 诊断标准 主要诊断标准① 两次血培养阳性，而且病原菌完全

一致，为典型的感染性心内膜炎致病菌。

② 超声心动图发现赘生物，或新的瓣膜关闭不全

鉴别诊断 differential diagnosis

急 性 acute— 金黄色葡萄球 Staphylococcus aureus 、淋球 gonococcus 、肺炎球菌 pneumococcus 和革兰氏阴性杆菌败血症

亚急性 subacute — 急性风湿热 acute rheumatic fever 、系统性红斑狼疮 SLE 、左房粘液瘤 left atrial myoxoma 、淋巴瘤腹腔内感染 intra-abdominal infection of lymphoma 、结核病 tuberculosis等

※ 治疗 treatment一、 principle of antimicrobial therapy ： ① 早期应用 early

② 充分用药 sufficient ：杀菌性、大剂量、长疗程③ 静脉用药为主 intravenous

④ 病原微生物不明时 unknown pathogen ：急性者针对金葡菌、链球菌、革兰氏阴性杆菌均有效广谱抗生素

⑤ 病原微生物已分离时 unknown pathogen ，据药敏结果

※ 治疗 treatment 经验治疗： 急性 萘呋西林 2g q 4h iv/ivgtt

加氨苄西林 2g q 4h iv 或 庆大霉素 160～ 240mg qd iv

亚急性 青霉素 320～ 400万 q 4～ 6h

或加庆大霉素

※ 治疗 treatment 已知致病微生物青霉素 敏感 首选青霉素 耐药 青霉素加庆大甲氧西林 敏感 萘呋西林或苯唑西林 耐药 万古霉素真菌感染 两性霉素 B

※ 治疗 treatment二、人工瓣膜置换术适应症 indication of prosthetic repla

③ 血培养持续阳性或反复复发④ Recurrent arterial embolism ，赘生物≥ 10mm⑤ A-V block

预后 prognosis 自然病程 natural course of disease ： acute

4 周内死亡 subacute ≥6月

细菌学治愈 bacterial cure: 五年存活率 60%~

70%

复发 relapse ： 10%在治疗后数月或数年再发
