CASE PRESENTATION

آقای گلوله 60بیمار اصابت دلیل به سالهبه 7ساعت تهران اورژانس توسط صبح

. شود می آورده تیر هفت بیمارستان

PRESENT ILLNESS آقای تفنگ 60بیمار توسط کار محل در صبح که ساله

ناحیه به متری نیم فاصله از گلوله اصابت مورد شکاری. میشود صورت

دهان سقف به و کرده عبور جلو های دندان بین از گلولهقفسه ناحیه به شتم و ضرب مورد میکندوبیمار برخورد

. گیرد می قرار نیز سینه

PRIMARY SURVEY

A:airway. . نشد دیده آپنه و سیانوز باز هوایی راه شد فیکس گردنی کالر . نداشت وجود اینتوباسیون به نیاز خودی به خود تکلم

B:breathing خودی به خود تنفس : ندارد صدا کاهش ریه سمع

C:circulation with hemorrhage control گاز خونریزی کنترل جهت که دهان داخل از خونریزی

شد گذاشته دهان داخل نداشت وجود شوک BP:140/90 PR:90عالیم

PRIMARY SURVEY

D:disabilityGCS:15 نور به نرمال واکنش با قرینه و نرمال ها مردمک: نرمال ها اندام حرکت

E:exposure/enviroment نشد مشاهده بدن نقاط سایر در زخم از اثری

و باال نیش و پیش های دندان در شکستگی و سوختگی آثارپایین

سخت 2 و نرم کام ناحیه در گلوله ورود از ناشی کوچک زخمگردنی چهارم تا اول های مهره ناحیه در میدالین تندرنس

پیدا نورورلوژیک عالیم و نشد تنفسی دیسترس دچار بیمارنکرد.

SHOTGUN WOUND

 low to intermediate velocity

The shotgun was designed to strike a small,fast-moving target at close range.

The tissue damage is proportional to the specific gravity and inversely proportional to the elastic properties of the affected organ.(liver is more vulnerable than the lungs).

CLASSIFICATION

Based on distance from the weapon to the victim type I : a long range (>7 yards)

penetration of subcutaneous tissue and deep fascia only spread greater than 25 cm in diameter

Type II :3 to 7 yards may create a large number of perforated structures.10 to 25 cm in diameter

Type III :(<3 yards) a massive destruction of tissue.less than 10 cm in diameter.

MANAGEMENT:

EMERGENCY TREATMENT Clear Airway and provide patent airway

Cleaning of blood, vomit and theet from inside of mouth with fingers

Aspiration of blood, saliva, and gastric contents Early Intubation or Tracheostomy

Control Hemorrhage Direct pressure on the wound Tying of bleeding vessels(a. Facialis, a. Temporalis

superfic., a. Angularis, a. Carotis externa) Angiographic demonstration and embolization of the

bleeding point Anterior-posterior nasal packing

Treat Shock Evaluate Associated Injuries ( cervical vertebrea,

skull base, intracranial, thoracal, intraabdominal) Diagnosis and treatment of facial injuries
