ThefirstprojectofmypresidentialyearstartedinHunan,Chinawithwhitesnow.Duringourvisitundertheextremeweatherandlivingconditions,Isawhowchildrentreasuredtheirstudyopportunity.

BLACKChangeMakerForumwashostedsolelybyKJCthisyearandthethemecolorwasallblack.Withthiscoolcolortoneandanimpressivelogodesign,wesuccessfullyinvitedfoureminentspeakersandattractedaudiencesofover400peopletoattendthisforum.Thiswasavaluableexperienceforourmembersandhelpedthemrealizethatwecanhandleeventsofthissize.Afterthisevent,ourOCteamgainedmoreconfidenceinhandlingsimilarprojectsandwereencouragedtomakemorecommitmentstoourChapter.

REDThisyearwasthe20thSisterhoodAnniversarybetweenPanMacJCandKJC.Thethemecolorofthecelebrationbanquetwasred.InChinesetradition,redisthecolorthatrepresentshappiness.Wewerehonoredtohaveover10pastpresidentsattendandmembersofKJCjoinedtogethertocelebratethismemorableevent.WehadawonderfulreunionandgatheringwithPanMacmembersandgottoknoweachothermoreaftertheevent.

GREENGreenisfullofvitalityanditisthebestcolortodescribebabies.Inour“MostPopularKids’ProductSelection”project,wewitnessedhowenergeticand livelybabieswere.Wesuccessfully collected information fromparentson theirfavoritekids’productbrandsinordertohelpnewparentsselectreliableproductsfortheirnewbornbabies.

ORANGEInJuly,wehostedthefirstever“Sleepout”project inHongKong.Thisprojectspanned2daysand1night.Fromorangesunsettoorangesunrise,wewitnessedcardboardboxescoveringeverycorneroftheconcreteground.Thisovernighteventallowedtheparticipantstorealizehowtolivewithashortageofresources.

BLUEBluewasthethemecolorofourfirst issueof the“Dialogue”publication.Startingfromthefirst issue,therewasabreakthrough inouroveralldesignandcontent.We invitedmanycelebrities toshare theirsuccessfulstoriesandexperiences.Throughthepreparationofthe“Dialogue”,theteamhadanopportunitytointerviewprominentfiguresface-to-face.

Alltoooften,whenpeoplehearthatIamachiropractor,I see a confused look on their faces because theysimplyhavenoideawhatchiropracticis.Then,whenItellthemthatItreatpeople’sboneandmusclepains&problemswithouttheuseofanymedication,theyfrownevenmorebecausetheycan’tputtheirmindaroundhowonecanfixpainsandacheswithoutanymedication.

Chiropractic and the human body

The word “chiropractic” comes from the Greek wordmeaning “treatment by hand” which is exactly what achiropractor does. Our expertise is in locating all thebones that are out of place, which may be impingingon the nerves causing the symptoms of pains andnumbness.Totreattheproblem,weuse“adjustments”alongwithotherappropriateexercisesandphysiotherapymachinestogentlybringthebonesbacktotheiroptimalpositionstorestoremobility.

Thespineisimportantnotonlybecauseitistheframeworkofourbody,butalsobecauseithousesourspinalcord,whichis"theliterallyirreplaceablecablebetween(the)brainandtherestof(the)body."1Thespinalcordgivesrise to its branches called spinal nerveswhich controlall our muscles and give us sensations. Irritation tothesenerveswillcausesymptoms.

Spinal nerves supply our body with sensation and movement. If any of these nerves are disrupted, our body may not function to its full capacity.

The preventative and conservative approach

Chiropractorsbelieveoneofthemostimportantwaystomaintainyourspinalhealthisthroughpreventative

care. The lateststudypublished in the journalSpineconfirmed that regularmaintenancecareiseffective inmaintaininglong-termbenefitsforpatientswithchronic lowbackpain.2Regularexercisesandproperposturearealsocrucialtomaintainingyourspinalhealth.

ChiropracticisaregulatedhealthprofessioninHongKongandmanydevelopedcountries.According to the American Chiropractic Association, more than 30 million people todayincorporatechiropracticcareeveryyeartohelpthemlivehealthyandactivelives.

Chiropractic is a medication-free approach to treat your skeletal, muscular and nervous system conditions. Learning to listen to your body is the first step toward a healthier life.

Listen – what is your body trying to tell you?

AsDr.LeahMillheiserofStanfordUniversitysays,our body is always trying to talk to us, to keep usinformedofhow it isdoing. Whenyouexperiencerecurring symptoms (like the frequent headaches,neckandbackpain,ornumbness),itisyourbody’swayoftellingyousomethingiswrong.Learntopayattention and look at your symptomsof discomfortas an alarm. Be aware about your physicalcondition, habits and posture, and understand thebasicreasonswhycertainsymptomsoccur.Asthesayinggoes, the first step is admitting youhaveaproblem.

Q1. Hospital Authority (HA) hospitals and staffs work conscientiously and efficiently to deliver healthcare services to our patients. At the same time, the expectation from the Hong Kong Public has been growing. In your opinion, what are the top three challenges of HA at this point of time?

Dr.PYLeung:Firstofall, thegrowingandagingHongKongpopulation isoneofourchallengesasthemedicalneedsandexpenditurestotreattheelderlyincreasesexponentiallyrelativetotheirages.Themedicalexpenditureofelderlypatientsmaybe10timesmorethanpatientsintheir40’s.Onaverage,around75%ofmedicalcostsarespentduringthelast2yearsofthelifejourney.

Secondly, the advancement in medical technology creates expectations from thepublic. Although technology helps patients achieve better treatment results, theresources required (e.g. manpower, preparation time and consumables) increaseaccordingly.Itcreatesapressuretoourlimitedresources.

Thirdly,HA is under constant pressure ofmeeting increasing services demandwith a limitedsupply of healthcare professionals, including nursing and allied health staff. The previousgovernment’s short-term budgetary difficulty resulted in a fluctuation in healthcare studentintake.Thenumberofmedicalgraduateshasfluctuatedfrom340in2001to240in2006to320in2013.

Furthermore,thedemandfrommainlandpatientsandtheirprivatemarketiscausingincreasedcompetitionforourhealthcareprofessionalstoservetheHKpublic.

Q2. In view of our aging population and rising medical costs, given the limited resources, how will you prioritize different needs from Hong Kong public?

Dr. PY Leung:Generally HA receives aroundHK$40 billion every year to cover our servicesandcapitalexpenditures.However,thefundingavailabilityissubjecttoHongKong’seconomicsituation. To determine our services priority, we rely on information collected from differentsources, including but not limited to different specialty steering committees, hospital visits,statisticsandanalysisgeneratedfromourhospital’sinformationsystems,consultancyreports,and also the Internet. By considering factors like the disease pattern, patient numbers anddiseasecharacteristics,we try toallocateour resources to the targetedareasofneed.At thesame time, cost effectiveness is one of our main concerns.A fine balance should be struckbetweenbeinganinnovativepioneerorasmartfollower.

Q3. The latest Patient Satisfactory Survey (PSS) about a patient’s journey in HA has been released recently. Could you share the key findings and insights from it?

Dr.PYLeung:Thepurposeofthepatientsatisfactorysurveyistoassesspatients’perceptionsandtheirexperienceforourHAservices.Ithelpsusto improveif thereareinsufficientareas.In general, 80%of patients are satisfiedwith our services andward environment.Over 87%patientshadconfidence inourdoctors&nurses.At thesametime, thevastmajority(88%)ofrespondentsreportedthattheyweretreatedwithrespectanddignityinHAhospitals.Inviewofourlowpublicmedicalexpenditure(~2.5%ofHKannualGDP)comparedwithotherdevelopedcountries,weareprovidingahighstandardinmedicalserviceswithrelativelylowcost.Throughthissurvey,weknowthatlong-waitingemergencyadmission,taste&choiceofhospitalfoodandcommunicationwithpatientsandtheirfamiliesareareasweshouldfurtherimprove.

Q4. Following the Healthcare Reform initiated from Food and Health Bureau(FHB), a number of public and private partnership programs, such as the Cataract Surgeries Program and Haemodialysis Public Private Partnership(HD PPP) , have been successfully launched in the community. Will these PPP programs continue in the future?

Dr.PYLeung:ThePPPprogramshaveprovidedopportunities tobetterutilize theproficiencyand capacity of the private sector. We aim to shorten the waiting time of our HApatientsforhigh-demandservices.ThefuturedevelopmentofPPPprogramsdepends

onwhether theprivatemarkethasspare capacity, the affordability ofHongKongcitizens,andwillingnessof private service providers.However, since HA bears the roleoftraininghealthcareprofessionalstoserveourcommunity,wewillnotfully outsource our services. Theexperiences gained through theexistingPPPprojectsprovidegoodreferences for potential areas toexpand primary care partnershipsand service model enhancementstudies.

Q5. What are the benefits and concerns of Health Protection Schemes (HPS) under the recent healthcare financing reform proposal?

Dr.PYLeung: Ingeneral,HAsupports theproposed introductionof theHPSsince itwants toencourage more individuals to use private healthcare services by subscribing to the privatehealthinsurance,therebyeasingthepressureonthepublichealthcaresysteminthelongrun.However,toimplementtheHPSsuccessfully,weneedtoensurethattheassociatedexpansionoftheprivatehealthcarecapacityisonpacewiththeimplementationoftheHPS,forexample,the number of private beds and supply of healthcare professionals. The packaged chargingmechanism,qualityassuranceandservicescoverageareareasthatshouldbehighlightedforfurtherinvestigation.

Q6. More and more liberalization measures for medical services have been included under CEPA and lands have been reserved for encouraging the building of private hospitals in the Government policy address. How do you look at the private medical services development and medical tourism of Hong Kong in the long run?

Dr. PY Leung: Provided that the problems like healthcare manpower shortage and servicecapacityhavebeenaddressed,HongKonghas thepotential tostrengthen its foundationandservepatientsallovertheworld.However,theHongKonghealthcareindustryshouldfirstfocusonservingourgeneralpublicasthisisourprimaryresponsibility.Inthefuture,wemaydevelopa regulated non-government body to run the healthcare services with flexibility. The incomegeneratedfromservingtheexternalpatientsmaysubsidizeourlocalneedsandresearch.Thiswouldbeawin-winsituationtoallofus.

Q7. As the leader of HA, different medical incidents and issues have been reported in the past. Could you share your experience on risk management?

Dr.PYLeung:Inmyopinion,themostimportantthinginhandlingriskisfact-findingfortheevent.Sometimes,theinformationonhandmaynotreflectthetruth.Iwould

try to seekadvice from theexpertsandspecialistsandassess the impactsbefore I giveanyfeedback.Agood leader shouldnotdrawconclusions tooearly.Asonceaworker inFHB forfood hygiene, I understand that sometimes effective controls rest on the general public. Forexample,bypublicizing thosesourcesof illness, thepublicwill avoid thehigh riskareasandcarryoutpreventivemeasures.Iteffectivelyhelpsstopthespreadofinfectiousdiseaseorfoodpoisoning.Thus,transparencyisequally important inriskmanagementandthewisdomofthegeneralpublicshouldnotbeunderestimated.

Q8. As one of the largest employers in Hong Kong, HA currently has nearly 60,000 employees. What are the key elements in managing expectations from such large population of staff?

Dr. PY Leung: In managing thehugeorganizationofHA,Ibelieveattitude towards the employeesis the most important factor. Welisten to their needs and collecttheir opinions through differentchannels likeonline forums,CE’sblog,hotlinesandleaflets.Wetryto work together as a team andcreatearelaxingatmospherethatencourages people to speak up.Thestaffcanfeelyourattitudebyobserving how you react to theirrequests.

Q9. What is the difference between being a front line doctor and the HA management? What are your philosophies in management?

Dr.PYLeung: I love todealwithhumanbeingsand I thinkmanagingpeople isan interestingchallenging.Iunderstandgoodmanagementisalwaysthekeytosuccess.Ihavewidenedmyexposure throughbeing inmanagementand Ikeep learningeveryday. Inmyopinion,agoodmanager does not need expertise in every area, but he has to find the right people to solvetheproblemsat theright time forhim.Foraclinician,mostofourwisdom is insideourbrain.But formanagement, theirwisdom isoutside theirheadandrestsonwhetherhecan find therightperson.Ialwayshaveacontact list thathelpsmetosolvethoseimpossiblemissions.Inaddition,oneshoulddevelophisown judgmentandsuggestionbeforehecommunicateswithhisboss.Ithelpstomovethematterforwardandsolveproblems.

Believingthat thenewideawillbring benefit to the organization, thememberwhoproduces it andotherswho support it arediscouraged.Unfortunately,theyhaveignoredthepoliticalrealitythatresistancetonewideasfrequently stems froma real or fancied threat to thedecisionmaker’splace in thehierarchy.It isalsoacruel reality that theorganization’s interestmaynotbe totallycompatiblewith thedecisionmaker’sinterest.Anewideamustthereforebepackagedsothatitdoesnotposeanykindofchallengeinanyformtothepositionofthosewhomustdecideonit.Alternatively,thenewidea,whenpresentedbyathirdparty,maystandgreaterchanceofbeingacceptedbythedecisionmakers.Itistheinfluenceofthethirdpartythatmatters,andnottheideaitself.

In office politics, the perception that ideasandchangeought tobe judgedon theirownmerits is but a naive idealism resulting fromunawareness of the pervasive and powerfulinfluences that dominance signaling andhierarchical factorshave indecisionmaking.If a new idea, i.e. a proposed change tothe existing system, structure or mode ofoperation,ispackagedsothatitmatchestheneeds or interests of the decision maker, itstandsagood chanceof acceptance. If, ontheotherhand,an idea isexpected tostandon its own intrinsicmerit, it often is doomedtofail.

Dating back to 19th Century in Hong Kong, Chinesepeople relied heavily on Chinesemedical treatment. Theresistance toWesternmedicinecouldbebetterexplainedthrough understanding the cultural difference betweenEastandWest,whenWesternmedical treatmentsuchassyringeinjectionandsurgicaloperationslargelyconflictedwithChinesetraditionalvalues.

AsHongKong population surged in the late 19thCentury, the government saw the pressingneedtocopewithpublichealthproblem,whichwasaggravatedwhenChineseresidentsdefiedWestern medicine treatment. It was against this backdrop of tension that the governmenthadsupportedagroupofChineseelites to found theTungWahHospitalas the firstChinesemedicine hospital in 1870 in the territory.The bubonic plague in 1894 had ledEuropeans todistrust Chinesemedicine and sown the seed of the colonial government’s hope to promoteWesternmedicinethroughaChinese-managedhospital.

Thisarticleseeks toexplore,usingcentury-oldKwongWahHospitalasanexample,how thependulumhadswungtoWesternmedicineinthelongcolonialhistory.

The early days of Kwong Wah HospitalKwongWahHospital was set up by the Board of Directors of TungWahHospital in 1911 tomeettherapidriseofpopulationinKowloonandtheNewTerritories.TheHospitalwasbuiltonapieceofbarren land inYauMaTeigrantedby thegovernment.Butunlike itspredecessorsin TungWah Hospital, KwongWah Hospital had been practising both Chinese andWesternmedicine fromdayone.With thegovernmentsubsidy, theHospitalwasable tobuildWesternmedical facilities such as operation theatres, pharmacy and consultation rooms. In-patientsadmitted to Kwong Wah Hospital were able to choose either Chinese or Western medicinetreatment(includingmedicine),offeredtothemfreeofcharge;whereasout-patientsweregivenfreeWestern treatment (includingmedicine),whichwas covered by the government subsidy,aswellasfreeChinesemedicineconsultation.However,Chinesemedicinewasnotforfreeas

herbswereexpensiveandtheywerenotsubsidizedbythegovernment.

Interestinglystill,despitethefactthatout-patientshadtopayforChinesemedicine,thereweremoreout-patientschoosingChinesemedicinetreatmentbetween1911and1914.Thetrendwashoweverdownwardfrom1915to1922whichsawadeclineinthenumberofpatientschoosingtopayforChineseherb.

In the 1920s, there were signs showing the colonial officials’ measures to actively promoteWestern medicine treatment and westernized hospital management through Kwong WahHospital,likeimplementingmeasurestorestrictnumberofpatientsreceivingChinesemedicinetreatmentandtoprohibitChinesemedicalpractitionersfromtreatinginfectiousdiseases.ThesemeasuresmetwithoppositionsfromtheKwongWahHospitalBoardofDirectors,asrecordedinitsBoardMeetingminutes.SomelikenedtheabolishmentofChinesemedicineservicestotheabolishmentoftheHospital.

Thewesternizationprocesssomehowhadasetbackin1922,whenananonymouswomanmadeseveraldonationstotalingahandsomeamountof$50,580tourgeKwongWahHospitaltoofferfreeChinesemedicinetothepublic.TheBoardofDirectorscontinuedherkindworkbyraisingan additional $70,000 to set up a fund in order to generate interest for coving free ChinesemedicineinKwongWahHospital.ThemovehadledtoanenormousincreaseofattendanceinChinesemedicineout-patientclinicfrom18,080in1922to162,779in1935.

The setting up of Medical Committee As theSino-JapaneseWar broke out in 1937, a large number ofMainland refugees came toHongKong.At this time,KwongWahHospitalandTungWahHospitalhadmergedwithTungWahEasternHospital(setupin1929)tobecomethe“TungWahGroupofHospitals”underthegovernance of oneBoard of Directors. Suffering from the bad economy,TungWahGroup ofHospitalshadrunintodeficitasaresultofspendingonmedicalservicesandrelieffundtohelptheMainlandimmigrants.TheGroupcouldnotbutseekmoregovernmentsubsidytosavethethreehospitalsfromclosing.

Seeing this as an opportunity to exert more control over the TungWah Group of Hospitals,thegovernment promised to provide full subsidybut demanded the settingupof theMedicalCommitteewiththeaimtosupervisethemedicaladministrationofTungWah.Thegovernmentalso introduced seven pre-conditions for the full subsidy, one of these was the abolition ofChinesemedicalserviceinthethreehospitalsofTungWahGroupofHospitals.

AlthoughChinesemedicalserviceswereretainedat last, thenewMedicalCommitteehad leftlong-lasting influence through measures like allocating more resources to Western medicalservices,hiringmoreWesternmedicalstaffwithbetterpay,introducingmoreadvancedmedicalfacilitiessuchasx-rayexaminationroomandlaboratorytoKwongWahHospital.

Changes after World War IIUnder the influence of Western culture and effective cure of Western medical treatment,ChineseattitudetowardsWesternmedicinegraduallybecamemoreopenafter theSecondWorldWar. By this time,Chinesemedicine service, only available in out-

patientclinic,wasdwarfedby thewell-establishedWesternizedhospitalmanagementandthewidelypromotedWesternmedicineknowledgethroughtheHospital’sworkoftranslatingforeignmedical magazines and papers into Chinese articles, organizing Western medical treatmentseminars,implementingvaccinationforthepublicandthelike.

AsKwongWahHospital completed its large-scale redevelopment by1960s, theHospital hasessentially transformed into one of themostmodernized andWesternized hospitals in HongKong.Theredevelopmenthadreceivedamassivegovernmentsubsidyandagrantofadditionalsite area to leave more room for expansion of specialist medical departments, with moreresourcesandadvancedmedicalequipmentsavailableforWesternmedicinetreatment.

The revival of Chinese medicine servicesThe return of sovereignty of Hong Kong to Mainland China has marked a watershed in thedevelopmentofChinesemedicine in the territory.ThestatusofRegisteredChineseMedicinePractitioner was established in 2000 following the ChineseMedicineOrdinance promulgatedin1999.Under thenew regulatory framework, therewasa revivalof interestandacceptanceforChinesemedicineinthecommunity,accompaniedbygovernment’sinitiativetorunChinesemedicinepilotschemesinsomepublichospitals,andtheblueprinttosetup18Chinesemedicinepublicclinicsintheterritory.

AdvancesweremadebyTungWahGroupofHospitals.TheKwongWahHospital–TheChineseUniversityofHongKongChineseMedicineClinicalResearchandServicesCentre,foundedinKwongWahHospitalin2001,wasoneofthetrend-settersinthatitprovidedaone-stopshopfortreatmentservices,researchandteaching inChinesemedicine. In theyearsthat follow,TungWahGroupofHospitals had further introducedan integratedChineseandWesternmedicinetreatmentcentreforinpatientsinKwongWahHospitalin2006,andsetupanintegratedhealthmanagement centre in 2009 pioneering integrated medicine health management service inKwongWahHospital.

As Kwong Wah Hospital is turning to 100 years by 9 October 2011, its westernization history from 1911 to post-war decades has shed light on the profound Western influence on Chinese medicine development as well as the culture of the local society.
