33 月月 1133 日日（（水水））ドドゥゥリリケケルル病病院院訪訪問問

カトマンズ市内から約 30 キロ東に位置するドゥリケル病院を訪問。ドゥリケル病院はカトマンズ大

学（Kathmandu University）の附属病院で、2017 年 2 月よりアジア遠隔医療センターが関わる遠隔

医療会議にも参加している。エンジニアの Saran Shrestha 氏の案内で同施設の部署を見学。皮膚科で

は病院が独自開発した化粧品の販売もされていた。眼科では眼鏡の販売もしており、へき地から訪れ

てくる患者のため市場価格より安く、その場で眼鏡を作成できる体制が整っていた。またスマートフ

ォンを利用した遠隔診断も進めているという話も聞き、そのために開発された機材も拝見した。遠隔

医療会議のためのインターネット環境は整っているとは言い難かったが、ネパール研究教育ネットワ

ーク（NREN）が提供するオンライン・ラーニングシステムなども活用されており、今後の IT 環境の

整備が期待できる。午後にはカトマンズ大学（Kathmandu University）を訪問。大学副総長の Ram

K Makaju Shrestha 教授と面会し、カトマンズ大学やドゥリケル病院との遠隔医療を活用した共同研

究の可能性についてディスカッションを行った。カトマンズ大学でもオリジナル商品の提供があり、

また歴史資料館も見学した。このような形の大学のアピールは、九州大学でも参考にできる点ではな

いかと考えた。

カトマンズ市のイエローパゴダホテルにて第2回ネパール遠隔医療ワークショップが開催された。

ネパール各地より 100 名以上が参加し、遠隔医療の活用、技術的問題点や今後の計画などについて、

非常に活発な発表・討議が行われた。山岳地帯や医療サービスの乏しいへき地が大部分を占めるネパ

ールでは遠隔医療の活用は必須であり、貧しい人々にも安価に、より広い地域に医療サービスを提供

するための試行錯誤が重ねられている。インターネットの普及率はまだ高くはなく安定しているとも

言えないが、学術研究ネットワークを管轄する NREN の協力のもと、ネパールの遠隔医療を牽引する

ネパール地域医療遠隔医療センターはじめ軸となる病院を中心に、へき地に点在する医院と連携し、

アジア遠隔医療開発センターも協力し世界的にもモデルとなり得る遠隔医療の体制を作って行ければ

と思う。

33 月月 1155 日日テティィルルガガンンガガ眼眼科科研研究究所所訪訪問問

ティルガンガ眼科研究所はネパールのみならずアジアでもトップクラスの眼科専門の病院で、アジア

各地やオーストラリア等からも研修医が学びに来ている。特に義眼の技術は素晴らしく、本物と区別

することが困難であった。2017年より遠隔医療活動に関わっており、今後さらなる活動が見込まれる。

ネネパパーールル医医科科大大学学訪訪問問

ネパール医科大学もカトマンズ地域では長い歴史を持つ大学である

が、アジア遠隔医療開発センターでは接続実績がなく、今後のネパー

ルにおける遠隔医療活動の拡充のためにも是非とも協力を仰ぎたい

施設の一つである。オーストラリア・ニュージーランドの消化器病学

トレーニングセンターが内視鏡トレーニングを開催するなどオセア

ニア地域との関りも強く、遠隔医療活動の活用も見込まれる。

ネネパパーールル国国立立医医学学院院ビビーールル病病院院訪訪問問

ビール病院は 2018 年より新たに遠隔医療活動に加わった病院である。

施設内には遠隔医療会議専用の部屋も開設されており、H.323 の設置

も確認できた。専属のエンジニアは 1 名のみで、一人で病院だけでな

くアカデミーの遠隔医療活動に技術支援を行っている。今後スタッフ

を充実させる見込みはあるとのことだったので、活動環境がさらに整

い、これからも活発に遠隔医療活動に参入してもらいたいと考える。

2016 年にハノイ、2018 年にホーチミンで開催されたベトナム遠隔医療ワークショップに続き今回は

3 回目の開催であった。世話人はハノイ医科大学の Prof. Hai An が務め、TEMDEC は参加者の招聘

やプログラムの作成等のサポートを行った。

ハノイ、ホーチミン、フエ、ニンビンから医師、歯科医師、エンジニアが参加した。今回のワークショ

ックの目的は、①これまでのベトナムにおける遠隔医療の活動報告、②遠隔医療を行うための技術や

手順の紹介、③今後に向けた議論と計画の 3 つであった。

午前は遠隔医療に関わっている医師・エンジニアよりこれまでの活動報告があった。リハビリテーシ

ョン、眼科、内視鏡、口腔外科といった様々な分野の医師・歯科医師より現状の活動報告や遠隔医療を

行う上での問題提起、今後の展望について発表があった。

午後は 2 グループに分かれて遠隔医療を行う上での問題点、その解決策について討論が行われた。各

グループは医師・エンジニア数名ずつから構成され、医師からはニーズや今後の展望、エンジニアから

は技術的な問題についてベトナム語で討論され、チームの代表者が英語で発表を行った。

最後にスマートフォンを用いて、今回のワークショップについての感想を選択形式でアンケートを行

い、回答結果をモニター上にグラフ化し共有した。どの参加者も遠隔医療の必要性を感じているとい

う結果であった。ベトナムは南北に長く、ハノイやホーチミンのような大都市とその他の地域とでは

医療の質や病院の数の差がある。ベトナム国内で遠隔会議を使って医師や歯科医師同士での経験を共

有することの必要性を感じているようであった。また、海外との遠隔会議については言語の問題が挙

げられた。しかし他国医師とも遠隔会議を行いベトナムの国全体で医療の質を向上できるよう希望が

あった。来年度の第 4 回ベトナム遠隔医療ワークショップは、初めて中部フエで行われる予定である。

今後これまで以上に参加施設が増え、ベトナム国内での活発な遠隔医療活動が行われることを期待し

たい。

ビビシシュュケケクク市市内内のの医医療療施施設設・・大大学学訪訪問問

今後キルギスにおける遠隔医療活動の必要性、また、その実現に向けての将来的活動の議論を行う

ため 2 日間で医療施設・大学及び日本大使館・JICA 事務所を訪問した。医療施設を訪問し、内視鏡設

備が不足していることが判明した。そのため、今後どのような活動をすればキルギス及び日本政府に

支援を得られるかを議論した。10 月末に再度訪問し、内視鏡及び遠隔医療会議の技術的トレーニング

を実施することを仮決定し、その実績を踏まえて両政府に相談することになった。

第第 11 回回キキルルギギスス遠遠隔隔医医療療ワワーーククシショョッッププ参参加加

6 月 26 日に第 1 回キルギス遠隔医療ワークショップが開催された。TEMDEC は遠隔医療カンファ

レンスデモンストレーションセッションの技術的サポートを提供した。デモンストレーションセッシ

ョンではキルギス会場と日本の九州大学病院を接続し、九州大学病院から内視鏡処置のライブ配信を

行った。また清水先生より TEMDEC の遠隔医療活動の紹介、私より遠隔医療カンファレンスを開催

する際の技術的要素の発表を行った。その他のセッションでは、キルギスの医療施設で既にどのよう

な遠隔医療が実施されているのか、また、その課題について発表が行われた。キルギスで定期的な遠

隔医療カンファレンスを実施するための予算、技術者のスキルアップの必要性に関する多くの議論が、

会場の医師・技術者間で行われた。

第第 11 回回ブブーータタンン遠遠隔隔医医療療ワワーーククシショョッッププをを開開催催

10 月 15 日にブータンで初めてとなる遠隔医療ワークショップを、同国保健省の Mr. Tashi と、同

国の研究教育ネットワークを管理する DrukREN に在籍する Mr. Karma の協力のもと、ホテルドゥ

ルクティンプーで開催した。ホテルのあるティンプー以外の都市からも参加者が集まり、全体で医師

とエンジニアを併せて 60 人近くが集まる大変盛況な会となった。当科の清水先生から挨拶と遠隔医療

の概論についての講演が行われたのち、このワークショップに参加した 37 施設における遠隔医療の現

状やニーズ、インターネットの状況等を報告し、今後の展開について全員でディスカッションを行っ

た。ワークショップの間は九州大学病院からもエンジニアが遠隔で参加してくれていたが、時間不足

のためディスカッションに加わることができなかったことが残念であった。

自分自身も胃癌による死亡率をどのように減らしていくかについて講演を行ったが、ブータンには

内視鏡を施行できる施設が 5 病院しかなく、絶対的な医療機器不足を解消しないと、ソフトパワーだ

けでの医療協力には限界があることを思い知った。

施施設設訪訪問問

16 日はまず保健省を訪問した。そこで大臣と面会し、同国の首相が泌尿器科でまだ医療現場に立ち

続けていること、そして彼が胃癌ゼロを目指して国家予算の一部を対策費に使用することを知った。

胃癌ゼロのために国民全員についてピロリの除菌治療を行うか、内視鏡技術を浸透させて早期発見率

の向上を目指すか、活発な意見交換を行った。次にブータンケサルギャルポ医科大学とその教育病院

であるジグミ・ドルジ・ワンチュク国立病院を訪問したところ、JICA の方も我々の訪問を待ってくだ

さっていた。現地の医療事情を説明してもらい、胃癌のみならず今後我々が遠隔医療を通じてできる

こと、JICA 草の根事業として行われている「HIGAN」プロジェクトとのコラボレーションの可能性

について、大変有用なディスカッションを行うことができた。

第第 44 回回イインンドドネネシシアア遠遠隔隔医医療療ワワーーククシショョッッププをを開開催催

11 月 14 日に、インドネシアのマランにあるブラウィジャヤ大学において、インドネシアでは第 4

回になる遠隔医療ワークショップを開催した。インドネシアは我々のパートナーの中でも遠隔医療が

活発な国で、医師とエンジニアが強力なタッグを組んで消化器、神経内科の領域を中心に定期的に遠

隔教育プログラムを開催している。初めてジャカルタ以外での開催となった本会であったが、早朝か

ら 50 名以上の医師とエンジニアが参加し、彼らの遠隔医療に対する本気度が伺えた。また、会の中で

はインドの施設から内視鏡のライブデモも披露され、同国のエンジニアのスキルの高さも再認識する

ことができた。今回のワークショップの目的はこれまでの活動報告に加え、新たな分野を広げていく

ための礎となることであった。九州大学からは心臓外科、救急医療、自然資源を用いた医薬品の創出と

いった、これまでの我々のプログラムにはなかった新しい分野の遠隔医療に関する提案がなされ、多

くのインドネシア人が興味を示していた。実際、心臓外科は本会の直後から遠隔教育に関する具体的

なプログラム作成が始まったこともあり、目的を果たすことができたと考えられる。個人的には、イン

ドネシアでの消化器診療に関する遠隔医療のセッションで司会を行い、同国の内視鏡の診療レベルが

よく理解できたこと、その上でどのように貢献できるかを考える一助となったことが大変有意義であ

った。

を締結した。また、附属病院として建設中の Brawijaya Hosp を訪問した。同院は看護師が院長を務め

ており、日本との違いに驚いた。また、同大学の教育病院であるサイフル・アンワル・マラン地域総合

病院も訪問し、今回のワークショップを主催した Dr. Syifa のオフィスや内視鏡室、遠隔会議室の状況

を確認した。

１日目は MMA に参加し、Telemedicine のセッションで、ミャンマーと日本の、医師と技術者それ

ぞれから発表を行った。私は遠隔医療技術について、基礎、トレンド、マナーやトラブルを防ぐコツに

ついて発表した。パネルディスカッションでは、多くの医師から技術的な質問を受けたが驚いたこと

に非常に実務的で、技術的なことも医師が自ら実施しているのだということを改めて実感した。

２−３日目は施設にて技術調査やネットワーク接続試験を行なった。ヤンゴンコンピュータ大学では、

mmREN のテレビ会議接続テストを Prof. Zin や学長らの立ち合いのもと実施した。Vidyo, H.323,

Zoom が全て十分な品質で接続できることが確認できたが、試験中、3 回も電源が落ちるという問題も

あった。ネットワーク機器も全てダウンしたため、復旧には 10 分以上かかる。周辺一帯の電源が落ち

ており、大学の問題ということではないが、この問題は mmREN 全体に影響を及ぼすであろう。改善

が必要である。ヤンゴン専門病院では、私たちも長年付き合いのあるソウル大学ブンダン病院からの

ライブセッションを実施したということがわかった。帯域が足りず、通信品質が低いことで、4Mbps

の回線を一時的に 100Mbps にまで上げることで接続を樹立したということで、帯域不足、通信費用の

捻出が問題となりがちなミャンマーでの活動においては、本病院の手法は参考になると考えられる。

ヤンゴン第一医科大学では翌日からのワークショップの設営を、10 月に研修した Dr. Khaw, Ms. Nillar,

Mr. Phyo と一緒に実施した。大きな会場でのテレカンファレンスの設営で、映像回りは問題なかった

が、音の品質が低かったため、テレビ会議システムへの直接の繋ぎ込みを指導しながら設営した。彼ら

自身が問題に気付き、その解決に至るまでには今後の継続的な指導が必要であろう。

最終日には、ミャンマー遠隔医療ワークショップが実施された。ミャンマーからの参加者のほとん

どは医師であり、ミャンマー国内と海外との深刻な医療格差を問題視していたため、遠隔医療に寄せ

られる期待は非常に大きいことがわかった。病院の経費を管理する保健スポーツ省からの参加者があ

ったことで、多くの医師が遠隔医療に必要な機材の支給を強く求め、これらについて支給を前向きに

認めてもらったことは良いことだと思う。

今回初めてミャンマーという国に赴き、病院における ICT や国外との通信も含めた整備の必要性を

強く感じた。今後の活動につなげていきたい。
