The 15th annual AzCHOW conference "Community Health Workers: Uniting
Our Voice to Advance Health Equity" is the signature event for the
Community Health Worker (CHW) workforce in Arizona. Community Health
Workers are a vital workforce in today's healthcare system: we connect
patients to care, improve health service delivery, and advocate for
vulnerable populations in our state. The annual AzCHOW conference
strengthens the CHW workforce by providing training, building professional
capacity and sharing knowledge about the pressing health issues facing
Arizona's communities.
The annual conference is a time for CHWs to come together, learn, celebrate
success and move forward as a workforce. The AzCHOW conference will
feature speakers from throughout Arizona discussing innovative programs
and approaches to health and well-being. Our focus this year will be on the
social determinants of health and how CHWs advocate for themselves and
their community.

THEME: COMMUNITY HEALTH WORKERS: UNITING OUR VOICE
TO ADVANCE HEALTH EQUITY
Community health workers promote health equity in our most vulnerable
communities. This conference will focus on ways that CHWs promote health
equity through a focus on the social determinants of health and provide
training and support for CHWs who seek to advance health equity in their
communities.

The Arizona Community Health Workers Association (AzCHOW) would like to recognize and honor the service of a Community Health Worker (CHW) in Arizona.
The American Public Health Association definition of a CHW is:
“A Community Health Worker (CHW) is a frontline public health trusted member of and/or has an in depth understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service deliver. A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.” (American Public Health Association, 2009)
CHWs are also known as Community Health Representatives, Cultural Health Navigators, Promotores, Community Health Advisors, etc…
Recognition of Awardee will occur during luncheon award in recognition of Dr. Joel S. Meister (pioneer and co-founder of the Promotora/CHW movement in Arizona) on June 22.

Esta relación de confianza permite al CHW servir como enlace e intermediario para facilitar el acceso a los servicios sociales, de salud y la comunidad; y así mejorar la calidad y la competencia cultural de los servicios ofrecidos. Un Trabajador Comunitario también fomenta la capacidad individual al igual que comunitaria, incrementando su conocimiento de salud y auto suficiencia a través de una serie de actividades como alcance y educación comunitaria, consejería informal, apoyo social y abogacía” (Asociación Americana de Salud Pública, 2009)
