ภาวะชกตอเนอง (Status Epilepticus)
• A condition resulting either from the failure of the mechanisms responsible for seizure termination or from the initiation of mechanisms, which lead to abnormally prolonged seizures (after time point t1).
• It is a condition , which can have long‐ term consequences (after time point t2) including neuronal death, neuronal injury, alteration of neuronal networks and functional deficits, depending on the type and duration of seizure.

อาการชกเกดขนโดยมปจจยกระตนหรอไม
1. Correctable provoked factors: hypoNa, hypoCa, hypoMg, hypoglycemia
specific Rx, no AED
2. Acute symptomatic seizure: CNS infection, inflammation, trauma, stroke, toxic
specific Rx + short‐term AED
3. Unprovoked seizure
3.1 First unprovoked seizure: 50% recurrence risk
3.2 Recurrent unprovoked seizure: Epilepsy

1. At least two unprovoked seizures occurring more than 24 hours apart.

การสงตรวจเพมเตมในผปวยทมอาการชก
• พจารณาสงตามภาวะทสงสยจากประวต และการตรวจรางกาย
• CBC• BS, Electrolytes, Ca, Mg, BUN/Cr, liver function test
• Toxic screening• Metabolic screening: ammonia, lactate
• CSF analysis• Brain imaging: CT brain, MRI
• EEG• Genetic testing (precision medicine)
คลนไฟฟาสมอง (EEG)
• ในผ ปวยโรคลมชก พบ EEG ผดปกต 50‐70%• ในคนปกตทไมมอาการชก พบ EEG ผดปกต 2‐5%ดงนน ในผปวยทมอาการเขาไดกบโรคลมชก อาจพบคลนไฟฟาสมองปกตไดประโยชนของ EEG ในการวนจฉยโรคลมชกชวยสนบสนนการวนจฉยโรคลมชกชวยแยกชนดของอาการชก และกลมอาการชก (epileptic syndrome)

Treatment• Specific treatment:
– Consult neurosurgeon for craniotomy and drainage• Cefotaxime 225mg/kg/day q 8 hr
• Supportive treatment:– Incerase intracranial pressure
• Head elevation, • on ventilator keep Pco2 30‐35• Keep I/O balance • Keep normal sodium• 20%mannitol 1g/kg/dose iv then 0.5 gm/kg/dose IV
– Status epilepticus• Phenytoin 5mg/kg/day IV q 12 hours
CSF culture: numberousHaemophilus influenza
Practical Points
• Febrile convulsion:• Seizure occurs within first 48 hours
• CNS infection should be considered in patient, who presents with seizure after 48 hours of fever.
• Status epilepticus is defined as• Seizure that lasts more than 5 minutes • Seizure that is repeated frequently enough that the
individual does not regain consciousness between seizures lasts more than 5 minutes
Increase intracranial pressure should be aware in patient with status epilepticus
