ผลการตรวจเสมหะโดยการยอมสทสมพนธกบการดอยา MDR ในผปวยใหมของสถาบนโรคทรวง ป พ.ศ. 2550-2552

ผปวยใหม ทเสมหะยอมพบเชอวณโรค จานวน 283 ราย หลงการรกษา สนเดอนท 1 เสมหะยอมพบเชอ 76 ราย (26.9%) เปน MDR 11 (14.5%) ราย หลงการรกษา สนเดอนท 2 เสมหะยอมพบเชอ 39 ราย (13.7%) เปน MDR 11 (28.2%) ราย

Performance of Two Commercial Line Probe Assays Kit Available in Thailand for Detection of Isoniazid Resistant M.Abscessus • Study was done in Central Chest Institute of Thailand and Bureau of

Tuberculosis to compare MTBDRplus and REBA MTB-MDR. • 55 and 82 strains of MDR from CCIT and BTB were enrolled. And 48

and 18 strains of INH mono-resistant were enrolled. • Use phenotypic DST and DNA sequencing as standard .

MTBDRplus REBA MTB-MDR Sensitivity INH resistant 99.0 % 99.5 % Sensitivity RMP resistant 99.5 % 97.5 % • MTBDRplus and REBA MTB-MDR have the same performance for

detection of drug resistant tuberculosis.

• ใชกบการดอยา INH+SM ดวย •การรกษาวณโรค แนะนา 6-9 RZE

•RMP mono-resistant จะทาให smear negative ชาลง เพราะ RMP จะเปนยาท ฆา เชอวณโรคในเซลลไดด (sterilizing activity)

•RMP mono-resistant จะทาให relapse สงขน แต cure rate เทาเดม

•WHO แนะนาใหใชสตร MDR รวมกบ INH

•EMB mono-resistant จะไมมผลตอการรกษาดวยสตรยา 2HRZE/4HR

Extended Spectrum Regimen • มความนยมใช regimen : 2HRZE+Levofloxacin ในผปวย

ใหม โดยคดวา จะฆาเชอไดเรวกวา แลวหายไดเรวกวา

• มการเตม Ofloxacin หรอ Levofloxacin ในสตรยา ระหวางการรกษาในผปวยทสงสยจะดอยา ซงไมควรปฏบต เพราะ เปน monotherapy ควรจะตรวจเสมหะและสง rapid test

• ไมควรให fluoroquinolone รวมกบ rifampicin - ใน clinical trial ให OFX จะทาให culture negative นอยกวา สตรยามาตราฐาน - การเพม FQs ไมเพมอตราการ หรอ ความเรวในการฆาเชอ

- Rifampicin กระตน efflux pump ของเชอวณโรคใหดอ OFX

Non-tuberculous Mycobacteria (NTM) Infection • Prevalence of NTM infection is increasing because culture

was done more frequent in clinical case management. • Most common organ that was infected with NTM was lung. • Diagnosis of NTM infection is based on clinical

signs/symptoms, X-ray abnormalities and culture positive at least 2 cultures in 12 months with moderate growth.

• NTM naturally resisted to most of anti-TB drugs but susceptible to antibiotics and no standard drug susceptibility test.

• Treatment is based on species of NTM so identification is very important. Molecular identification is standard test.

• NTM is divided into “slower” and “rapid” grower.

A nevel gene, erm(41) confers macrolide inducible resistant to clinical isolated of M.abscessus but is absent from M.chelonae

Summary • Molecular diagnostic test will substitute smear and staining because

of high sensitivity. • Line probe assay will be a standard rapid drug susceptibility test in

patient who have risk to carry drug resistance. • In patient who did not have risk of drug resistant over 15%, screening

rapid drug susceptibility test should be confirmed by second test. • Treatment of Non-tuberculous mycobacteria is based on species, no

empiric treatment of NTM infection without species identification. • Line probe assay is a standard test for species identification of

mycobacteria. • For rapid grower NTM, differentiate M.massilense from M.abscessus

was necessary because it is related to prognosis and treatment response.
