• ควรใชเฉพาะมขอบงชเทานน
• กรณผปวยมอาการรนแรง โรคประจ าตวตบ ไต ผปวย SE ควรตองตรวจวด
ระดบยา
• ระวงเมอใหรวมกบยา meropenam เพราะระดบยาจะลดลงมากๆๆ
• เมอมการ loading แลว ควรตองใหยา maintenance ตอดวย
• โดยการใหนนควรใหระยะหางจากครงแรกตามคาครงชวตของยานนๆ
• ควรตองใหยา maintenance หลงจากให loading dose ไปแลว ประมาณเทากบ half life ของยา
• ยกเวนกรณใหยา sodium valproate สามารถให maintenance ตอไดเลย 20-25 mg/kg/day
• ยกเวนกรณใหยา Pb เนองจาก half life ยาวมากควรเรมให maintenance หลง loading ไปประมาณ 12 ชม.
• การ loading phenytoin iv ตอดวย maintenance สามารถใหไดทง one
daily/q 8 hr ไมมความแตกตางกน
• Pb for maintenance : 300 mg/day…180 mg/day
• การ maintenance นนใหเลอกใชยาทไดผลดในการควบคมชก
• ถาผปวยยง critical ควรให iv form กอน
• ถาผปวย feed อาหาร ควรใหยาแบบน า feed ใหหางจากมออาหาร 2 ชม.
• มการ loading ยาบอยมาก โดยไมมขอบงช
• การ loading oral form ม 2 แบบ คอ single dose(8 cap stat)
และ split dose 3-3-2/4-4
• ผลการตรวจวดระดบยาได therapeutic range เหมอนกน
• กรณ SE ควรตรวจวดระดบยากรณ 1. uncontrol SE 2. liver/ renal failure 3. hypoalbumin 4. drug interaction;meropenam,anti TB, anti fungus 5. coma ตลอด เพอดวาม drug overdose ? • ตรวจเมอไหร ?
• เมอผปวยอาการดขนแลว ไมมความจ าเปนตองไดยา IV กควรเปลยนเปนยาทาน
• การใชยาทานตองระมดระวงเรอง drug interaction • ยากนชกทม protein binding สงเชน phenytoin ควรใหหางจากมอ
อาหาร 2 ชม. • กรณ SE เมอผปวยหยดชกแลว 24 ชม. กตองคอยๆลดขนาดยาลง

• ไมทราบวาโรคลมชกตองรกษานาน 2-3 ป • ไมทราบวาตองรบประทานยาทกวน สม าเสมอ • เขาใจวาทานยาเฉพาะชวงทมอาการชกเทานน • ไมทราบวาตองดแลสขภาพอยางอนดวย • ไมทราบวาการหยดยาทนทมขอเสยทอาจรายแรง • ไมทราบวธการทานยาเมอลมทานยาไปบางครง
• แพทยยงมความเขาใจผดเกยวกบการใชยา เชน ขนาดยา ใชยารนใหมกอนยามาตรฐาน การใชยาหลายชนดพรอมกน
• แพทยขาดการประเมนผลการรกษา เพราะขาดการตอเนอง
• แพทยยงมความเขาใจเกยวกบการรกษาทคลาดเคลอน เชน การรบปรบยาโดยทไมไดมการประเมนสาเหตของการควบคมอาการไมได
• การแปรผลการตรวจวดระดบยา “therapeutic range”
• ความสมพนธระหวางแพทย กบผปวยและญาตไมด
• ผปวยชาย 25 ป โรคลมชกมา 5 ป
• ทานยา phenytoin สม าเสมอ แตยงมอาการชก1-2 ครง/ป
• มปญหาเหงอกบวมโตมากขน
• แพทยจงหยดยาเดม และเรมยา valproate
• หลงจากนนผปวยมอาการชก 3 ครง/วน
