Fungsi jantung adalah mensirkulasikan darah ke seluruh tubuh dengan cara:
•Memompa darah ke paru dengan membuang CO2 dan mengambil darah yang mengandung O2
•Darah yang teroksigenasi di pompa ke seluruh tubuh untuk menyediakan oksigen dan nutrisi dan membuang sampah hasil metabolisme
•Arteri koroner merupakan pembuluh yang menyuplai darah dan O2 ke otot jantung
2 arteri koroner dari aorta berada diatas katup aorta. “arteri koroner tidak lebih besar dari sedotan minum, di bagi 2 yang melingkari permukaan jantung .Arteri koroner mengirim darah ke jantung sekitar 130 galon dalam sehari ( 1 galon ± 10 liter)

Coronary Artery Disease (CAD) /Penyakit Jantung Koroner (PJK)
• Coronary artery disease adalah salah satu yang paling umum dan serius yang merupakan efek dari penuaan (aging). Lemak menempel di dinding pembuluh darah (lumen) yang mana pembuluh darah merupakan tempat untuk sirkulasi darah. Hal ini yang menyebabkan kondisi yang disebut atherosklerosis yang selalu menghentikan aliran darah ke arteri koroner (blocked coronary artery) dan menimbulkan serangan jantung(heart attack).
“Penyakit Cardiovascular 39.4 persen menyebabkan kematian dari semua kematian yang terjadi di USA atau 1 dari 2.5 persen meninggal di tahun 2000. CVD kira –kira 60 persen total kematian di USA, artinya dari kematian lebih dari 2,400,000 semua kasus, CVD merupakan penyakit pertama yang berkontribusi sebagai penyebab kematian sekitar 1,415,000 (American Heart Disease)
•Sejak tahun 1900, CVD merupakan penyebab kematian No. 1 di United States setiap tahun
•Sekitar 2,600 di Americans orang meninggal karena CVD setiap hari, rata-rata 1 meninggal setiap 33 detik ( setengah menit)
•CVD menempati urutan pertama penyebab kematian setelah cancer, chronic lower respiratory diseases, accidents, diabetes mellitus, influenza and pneumonia.
•Lebih dari 150,000 orang American meninggal karena CVD setiap tahun di bawah umur 65 th.
•Di Indonesia, meskipun belum diketahui data resmi, namun posisi penyakit ini terus meningkat dalam menyumbang kematian.
•Pada tahun 1972 penyakit kardiovaskuler berada di posisi ke 11, tahun 1986 diurutan ke 3 dan tahun 1992 menempati urutan pertama penyebab kematian
•Dari jumlah tersebut, sekitar 476.126 kematian disebabkan oleh penyakit jantung iskhemik (PJI) yang menjadi penyebab kematian nomor satu.

ischemia. Darah ke jantung terbatas karena CAD, tetapi tidak merasakan beberapa efek.
• Chest pain: If your coronary arteries can’t supply enough blood to meet the oxygen demands of your heart, the result may be chest pain called angina.
• Shortness of breath: Some people may not be aware they have CAD until they develop symptoms of congestive heart failure- extreme fatigue with exertion, shortness of breath and swelling in their feet and ankles.
• Heart attack: Results when an artery to your heart muscle becomes completely blocked and the party of your heart muscles fed by that artery dies.

Tetapi atherosklerosis di jantung akan menimbulkan efek yang sangat bahaya.Tubuh tergantung dari pompa jantung untuk mensirkulasikan darah ke seluruh tubuh dan ke otot jantung itu sendiri. Jika arteri koroner tersumbat, otot jantung mengalami kegagalan dan sirkulasi menurun, termasuk sirkulasi ke otot jantung sendiri.

1) Stenting
• a stent is introduced into a blood vessel on a balloon catheter and advanced into the blocked area of the artery
• the balloon is then inflated and causes the stent to expand until it fits the inner wall of the vessel, conforming to contours as needed
• the balloon is then deflated and drawn back
•The stent stays in place permanently, holding the vessel open and improving the flow of blood.

2) Angioplasty• a balloon catheter is passed through the guiding catheter to the area near the narrowing. A guide wire inside the balloon catheter is then advanced through the artery until the tip is beyond the narrowing.
• the angioplasty catheter is moved over the guide wire until the balloon is within the narrowed segment.
• balloon is inflated, compressing the plaque against the artery wall
• once plaque has been compressed and the artery has been sufficiently opened, the balloon catheter will be deflated and removed.

3) Bypass surgery
• healthy blood vessel is removed from leg, arm or chest
• blood vessel is used to create new blood flow path in your heart
• the “bypass graft” enables blood to reach your heart by flowing
around (bypassing) the blocked portion of the diseased artery. The increased blood flow reduces angina and the risk of heart attack.
