PENDAHULUAN
Sistem sirkulasi mengendalikan gerak kontinyu seluruh cairan tubuh, dengan fungsi
utama mengangkut oksigen dan nutrien ke jaringan dan karbondioksida serta produk limbah
metabolisme lainnya dari jaringan menuju ginjal dan paru-paru. Sistem sirkulasi juga terlibat
dalam pengaturan suhu dan distribusi molekul seperti hormon, dan sel seperti yang dari
sistem imun. Sistem sirkulasi memiliki dua komponen fungsional : sistem vaskular darah dan
sistem vaskular limfe.
Sistem vaskular darah terdiri atas lingkaran pembuluh yang aliran darahnya
dipertahankan oleh jantung yang memompa terus menerus. Sistem arteri membentuk jalinan
yang menuju kapiler yang merupakan tempat utama pertukaran gas dan metabolit antara
jaringan dan darah. Sistem vena mengembalikan darah dari kapiler ke jantung. Sebaliknya,
sistem vaskular limfe semata-mata adalah sistem drainase pasif untuk mengembalikan cairan
ekstravaskular yang berlebihan, yaitu limfe, ke dalam sistem vaskular darah. Sistem vaskular
limfe tidak mempunyai mekanisme pompa intrinsik.

SISTEM PEMBULUH DARAH
Sistem pembuluh darah mempunyai selapis sel endotel yang selalu melapisi
lumennya.Pada pembuluh kapiler, lapisan sel endotel ini merupakan bagian utama
dindingnya. Dengan meningkatnya kaliber pembuluh darah terlihat penambahan lapisan
pada dindingnya.
Keseluruhan sistem sirkulasi memiliki struktur dasar umum :
Satu lapis dalam terdiri atas selapis sel epitel sangat gepeng yang disebut endotel,
ditunjang oleh membran basal dan jaringan elastin halus; mereka membentuk tunica
intima;
Satu lapis muskular intermediat, terdiri atas lapisan sel otot polos membentuk tunica
media;
Satu lapis jaringan penyokong luar disebut tunica adventisia.
Pembuluh Kapiler
Pembuluh kapiler merupakan tabung endotel sederhana yang menghubungkan arteri
dan vena dari sistem peredaran darah. Garis tengahnya rata-rata 7-9 mikron atau mikrometer
(kira-kira sesuai dengan garis tengah eritrosit). Tingkat metabolisme suatu organ menentukan
kepadatan jalinan kapiler. Jalinan kapiler yang padat terdapat pada paru, hati, ginjal,
membran mukosa, kelenjar, otot rangka dan subsansia grisea otak. Sedangkan jalinan kapiler
yang longgar terdapat pada tendo, saraf, jaringan otot polos dan membran serosa.
Dinding kapiler terdiri atas selapis sel endotel gepeng, yang dipisahkan dari jaringan
di sekitarnya oleh lamina atau membran basal. Setiap sel endotel berupa lempeng tipis

melengkung dengan inti lonjong. Umumnya sel tertarik searah dengan poros pembuluh dan
meruncing pada kedua ujungnya.
Kapiler dikelilingi selubung tipis terdiri atas serat kolagen dan elastin tipis dan disertai
sel perivaskuler atau perisit. Setiap perisit diliputi membran basal. Perisit, kemungkinan
merupakan sel yang belum berkembang dan dapat berkembang menjadi sel jenis lain
termasuk otot polos.
PENGGOLONGAN KAPILER

Sifat-sifat dari kapiler sinusoid :
Lumennya berkelok-kelok
Mempunyai diameter yang lebih besar , 30-40 mikrometer
Sirkulasi darah lambat
Terbuka antara ruang antar sel
Tidak dibatasi endotel secara kontinyu, di tepi terdapat sel dengan dinding bulat yang
berfungsi untuk fagositosis (sel kupfer ).
terdapat dalam hati dan organ hemopoietik seperti sumsum tulang dan limpa.

Fungsi Kapiler
Kapiler sekurang-kurangnya melakukan tiga fungsi penting. Mereka berfungsi
sebagai sawar permeabilitas selektif, sebagai sistem sintetis dan metabolik serta
antitrombogenik
1. Permeabilitas
Kapiler dan venul pasca kapiler seringkali disebut sebagai pembuluh pertukaran
karena pada tempat inilah oksigen, karbon dioksida, dan metabolit dipindahkan dari darah ke
jaringan dan dari jaringan ke darah. Molekul hidrofobik dan hidrofilik kecil misalnya oksigen,
karbon dioksida,glukosa dapat berdifusi atau ditranspor secara aktif melintasi plasmalema sel
endotel kapiler. Substansi ini kemudian ditranspor melalui difusi ke permukaan sel
berlawanan, tempat mereka dibebaskan ke celah ekstraseluler.Air dan molekul hidrofilik lain,
bergaris tengah kurang dari 1,5 nm dan berberat molekul kurang dari 10.000 dapat melintasi
dinding kapiler dengan berdifusi melalui taut interselular. Dalam keadaan abnormal seperti
peradangan yang diinduksi oleh bakteri, bahan kimia, dan racun mengubah permeabilitas
tautan antar sel-sel endotel. Permeabilitas kapiler dan venul pasca kapiler sangat meningkat,
dengan cara menerobos tautan sel endotel lekosit ke luar dari aliran darah dengan
menerobos di antara sel endotel dan memasuki celah-celah jaringan melalui proses yang
disebut diapedesis.
2. Fungsi Metabolik
Sel endotel kapiler dapat memetabolisme berbagai macam substrat.
a.Aktivasi-Konversi angiotensin I menjadi angiotensin II
b.Inaktivasi-Konversi bradikinin, serotonin, prostaglandin, norepinefrin , trombin dan lain-lain
menjadi senyawa yang secara biologis tak aktif
c.Lipolisis-Perombakan lipoprotein menjadi trigleserida dan kolesterol (substrat untuk sintesis
hormon steroid dan struktur membran)
d.Produksi faktor vasoaktif-Sel-sel endotel menghasilkan berbagai macam substansi yang
memiliki efek pada tonus vaskuler, misalnya endotelin yang meruplakan obat
vasoaktif,dan nitrogen oksida, faktor relaksasi.
3. Fungsi Antitrombogenik
Bila sel endotel mengelupas, jaringan ikat subendotel yang tersingkap menginduksi
agregasi trombosit. Koagulasi fibrin yang terjadi kemudian membentuk massa padat yang
disebut trombus, yang dapat membesar dan menutup aliran vaskuler yaitu suatu keadaan
yang potensial fatal. Sel-sel endotel mencegah kontak trombosit dengan jaringan ikat
subendotel dengan demikian menghasilkan efek antitrombogenik.
ARTERI
Arteri diklasifikasikan menurut ukuran diameter lumennya :
Tipe Besar / elastis : Aorta dan cabangnya

Tipe Sedang / muskuler : Arteri biasa
Tipe Kecil : Arteriole
Dinding arteri pada umumnya terdiri atas tiga lapis atau tunika :
1.Tunika intima , yang paling dalam terdiri atas selapis sel endotel di sebelah dalam dan
diluarnya diliputi oleh lapisan subendotel yang merupakan jaringan ikat
fibroelastis halus; dan yang paling luar disebut membrana / lamina elastika
interna .
2.Tunika media, lapis tengah terutama terdiri atas sel otot polos yang tersusun melingkar.
Serat-serat elastin dan kolagen dalam jumlah yang beragam terselip di antara
sel-sel otot polos.
3.Tunika adventisia, terutama terdiri atas jaringan ikat yang kebanyakan unsurnya tersusun
sejajar sumbu panjang pembuluh . Berbatasan dengan tunika media mungkin
terdapat tunika elastika eksterna yang jelas. Ketebalan relatif dari setiap lapisan
tergantung pada jenis dan ukuran pembuluh.

ARTERIOL
Pembuluh ini, bergaris tengah 100 mikrometer atau kurang mempunyai tunika intima
terdiri atas endotel dan membrana elastika interna saja. Tidak ada jaringan sub endotel.
Membran elastika interna berupa jalinan serat yang terlihat dengan mikroskop cahaya
sebagai garis tipis berkilau tepat di bawah endotel.

Tunika media terdiri atas satu sampai lima lapis utuh sel otot polos dengan serat-serat elastin
tersebar diantaranya.
Tunika adventisia, lebih tipis dari tunika medianya berupa selapis jaringan ikat yang
mengandung serat kolagen menyatu dengan jaringan ikat di sekitarnya.
Arteriol mempnyai dinding yang relatif tebal dengan lumen yang relatif sempit. Pembuluh ini
mampu mengontrol distribusi darah ke dalam berbagai jaringan kapiler yang berbeda dengan
vasodilatasi dan vasokonstriksi setempat.

ARTERI KECIL DAN SEDANG.
Meliputi semua arteri yang bernama dan semua arteri kecil yang tidak bernama /
arteri innominatus / anonymous. Kelompok arteri ini disebut arteri distribusi atau arteri
pembagi.
Tunika intima mempunyai tiga lapisan yang jelas. Di bawah endotel yang berada di
atas membran basal yang tipis terdapat lapisan subendotel yang mengandung serat kolagen
dan elastin halus, beberapa fibroblas. Membran elastika interna sangat jelas dan membentuk
sabuk tebal yang terdiri atas jalinan padat serat elastin. Pada sediaan histologi lapisan ini
tampak bergelombang akibat kontraksi pasca-mati unsur otot tunika media.

Tunika media hampir semuanya dibentuk oleh serat otot polos yang tersusun
melingkar. Di antara lapisan otot ini, yang jumlahnya mencapai 40 terdapat sejumlah kecil
jaringan ikat yang mengandung serat elastin, kolagen dan retikulin dan sedikit fibroblas.
Tunika adventisia pada umumnya setebal tunika media. Lapisan ini terdiri atas
jaringan ikat longgar yang mengandung serata kolagen dan elastin yang hampir seluruhnya
tersusun memanjang atau melingkar ulir. Serat elastin terutama terdapat di bagain dalam
tempat mereka biasanya membentuk membran elastika eksterna. Bagian luar tunika
adventisia berbaur dengan jaringan ikat di sekitarnya tanpa batas yang jelas di antara
keduanya .
ARTERI BESAR
Arteri besar digolongkan dalam arteri tipe elastis. Sel endotel tunika intima
berbentuk poligonal tidak memanjang seperti pada arteri yang lebih kecil. Lapisan subendotel
terdiri atas serat elastin dan kolagen serta fibroblas.
Di bagian dalam tunika intima terdapat berkask-berkas kecil serat otot polos.
Sejumlah besar serat elastin terutama tersusun memanjang. Berjalan di bagian dalam lapis
subendotel .
Tunika media dicirikan oleh banyaknya membran elastin, 40-60 jumlahnya.Sel-sel
oto polos mempunyai banyak tonjolan pendek yang terpancang pada jaring-jaring elastin.
Tunika adventisianya berupa selubung tipis, tidak demikian tersusun secara khusus
sehingga sulit di bedakan dari jaringan di sekitarnya. Tidak jelas adanya tunika atau membran
elastika eksterna.

Jenis arteri elastis : aorta; arteri innominatus / anonymous; arteri pulmonaris; arteri
subclavia; arteri carotis communis
VENADarah di dalam sistem vena bertekanan sepersepuluh dari tekanan darah arteri dan
karena itu harus menampung volume darah lebih besar dari pada sistem areri . Kaliber vena
umumnya lebih besar daripada arteri, tetapi dindingnya jauh lebih tipis yang terutama
disebabkan oleh berkurangnya unsur otot dan elastinnya.
Dindingnya sebagian besar terdiri atas jaringan ikat kolagen. Serabut- serabut otot
dan serabut-serabut elastisnya jauh kurang menyolok dibanding dengan dinding arteri.
Dibandingkan dengan dinding arteri, vena tak mempertahankan bentuknya setelah kematian
karena sedikit mengandung serabut-serabut elastis sehingga dindingnya kolap dan bentuk
lumennya tak teratur.
Penggolongan Vena
1. Venula
2. Vena kecil dan sedang
3. Vena besar
VENULA
Perubahan dari kapiler ke venula berlangsung bertahap, sedikit demi sedikit bermula
meliputi penambahan jaringan ikat kemudian otot polos. Venula yang paling kecil mempunyai
intima terdiri atas endotel saja dengan selubung serat kolagen di luarnya.Venula ini terlibat
dalam saling tukar metabolit antara darah dan jaringan .
Venula menerima darah dari kapiler, pada tempat-tempat tertentu venula menerima
darah dari jembatan arteriovenosa.
Venula mempunyai diameter lebih besar dari kapiler.
Dindingnya terdiri atas :
Sel endotel dengang sitoplasma mengandung lisosom dan vesicle pinositotik
Membrana basalis , membran ini semakin tua semakin tebal .
Perisit
Serabut- serabut kolagen yang terdapat di luar perisit
Venula melanjutkan diri ke vena-vena kecil. Di sini perisit berubah menjadi sel-sel
otot polos.
Perisit pada kapiler dam venula cenderung bersifat makrofage.
Pada pembentukan vena baru dari venula, sel-sel perisit berubah menjadi sel-sel otot polos
dan dapat pula membentuk membran basalis dan serabut kolagen.

VENA KECIL DAN SEDANG
Struktur vena kecil dan sedang hampir sama, hanya berbeda dalam diameternya.
Garis tengahnya berkisar antara 1-9 mm.
Tunika intimanya tipis, sel endotelnya pendek dan berbentuk poligonal. Jaringan ikat
subendotel tidak jelas.
Tunika media tipis dibandingkan arteri yang setara. Lapisan ini terdiri atas berkas
kecil serat otot polos yang tersusun melingkar di pisahkan oleh serat-serat kolagen dan
jalinan halus serat elastin. Tunika media lebih berkembang pada vena anggot gerak bawah.
Tunika adventisia sangat berkembang dan membentuk sebagian besar dindingnya.
Lapisan ini terdiri atas jaringan ikat longgar dengan berkas serat kolagen kasar tersusun
memanjang dan sering terdapat otot polos.
VENA BESAR
Golongan vena ini termasuk vena kava inferior dan superior, vena porta, dan cabang-
cabang utamanya.
Tunika intimanya berstruktur sama dengan vena yang lebih kecil, tetapi sedikit lebih
tebal.
Tunika media kurang berkembang dan otot polos pembentuknya sangat berkurang
atau tidak ada.
Tunika adventisianya paling tebal dari ketiga lapisannya, terdiri atas tiga lapis.Tepat
di luar tunika media berupa suatu lapis mengandung jaringan ikat padat fibroelastis dengan
serat kolagen kasar. Daerah tengah mengandung banyak serabut otot memanjang, dan yang
paling luar hanya terdiri atas jalinan serat kolagen dan serat elastin

Beberapa vena tidak mempunyai otot polos sehingga tidak mempunyai tunika media.
Termasuk di dalamnya vena-vena serebral dan meningeal, sinus duramater dan vena-vena
serebral dan meningeal, sinus duramater dan vena-vena pada retina, tulang, jaringan erektil
penis dan bagian maternal plasenta.
KATUB VENA (VALVE)
Banyak vena-vena sedang terutama pada ekstremitas inferior mempunyai katub-
katub untuk mencegah aliran darah kembali ke jantung akibat gaya berat badan. Umunya
terdiri atas tonjolan tunika intima ke dalam lumen dan saling berhadapan. Katub disusun oleh
jaringan ikat elastis yang permukaannya di lapisi oleh endotel. Antara katub dengan dinding
vena sebelah distal katub membentuk suatu pelebaran akibat dinding di sini tipis.
Pelebaran ini disebut sinus. Pada vena-vena superficialis jaringan penyangganya
kurang sekali sehingga sinus makin melebar membentuk varises. Varises akan bertambah
lebar bila terjadi sumbatan di bagian distas.
ANASTOMOSIS ARTERIOVENOSA
Pada beberapa alat tubuh arteri dihubungkan dengan vena tidak hanya oleh kapiler-
kapiler tapi ada yang langsung dihubungkan melaui arteriovenosis. Pada anastomosis ini,
endotel langsung terletak di atas tunika media khusus yang berfungsi sebagi sfingter.
Pembuluh ini banyak terdapat di dalam kulit tubuh yang tidak tertutup misannya telapak
tangan dan kaki, bibir dan hidung serta bagian tubuh yang kegiatan metabolismenya berbeda
seperti kelenjar tiroid dan sistem pencernaan. Bila pintas ini tertutup, darah mengalir ke
dalam kapiler seperti biasa. Pada berbagai tempat,anastomosis ini bergelung jalannya dan
dikitari oleh selubung jaringan ikat terpisah membentuk glomus. Dinding anastomosis
memplunyai lapisan otot yang tebal dan banyak ditemukan akhiran-akhiran saraf vasomotor
yang bersifat simpatik. Bila otot di sini kontraksi, maka aliran darah areriol mengalir ke kapiler
melalui vena. Bila otot relaksasi, darah selain ke kapiler ada yang langsung ke vena melalui
anastomosis ini.
PEMBULUH DARAH BULUH DARAH (VASA VASORUM)
Arteri dan vena dengan garis tengah lebih dari 1 mm disuplai oleh pembuluh nutrisi
kecil yang disebut pembuluh darah buluh darah atau vasa vasorum. Pembuluh ini masuk ke
dalam tunika adventisia dan berakhir sebagai jalinan kapiler padat yang merasuk jauh ke
dalam lapisan terdalam tunika media. Jalinan buluh limfe ditemukan di dalam tunika
adventisia sebagian arteri dan vena yang lebih besar.
INERVASI SARAF

Dinding pembuluh darah, terutama arteri mempunyai banyak persarafan. Akson tak
bermielin yang merupakan vasomotor berasal dari ganglion simpatis masuk ke dalam tunika
adventisia dan berakhir membentuk hubungan dengan sel otot polos tunika media .
Serat saraf bermielin sebagai reseptor atau berfungsi sensoris berakhir sebagai ujung bebas
sensorik terdapat terutama di dalam adventisia.
JANTUNG
Jantung berfungsi memompa darah ke seluruh bagian tubuh. Organ ini mempunyai
empat ruangan utama yaitu atrium kiri dan kanan dan ventrikel kiri dan kanan. Vena kava
superior dan inferior mencurahkan darah vena yang berasal dari seluruh bagian tubuh ke
dalam atrium kanan. Darah didorong dari ventrikel kanan melaui arteri pulmonalis menuju
kedua paru tempat pertukaran gas terjadi dan kemudian kembali ke atrium kiri melaui vena
pulmonalis. Darah mengalir dari atrium kiri menuju ventrikel kiri dan kemudian diedarkan ke
seluruh tubuh oleh aorta beserta seluruh percabangannya.
Dinding jantung terdiri atas tiga lapisan :
1. Lapis dalam, endokardium
2. Lapis tengah, miokardium yang membentuk massa utama jantung
3. Lapis luar, epikardium
Endokardium
Endokardium merupakan homolog tunika intima pembuluh darah dan menutupi
seluruh permukaan dalam jantung.Permukaannya diliputi oleh endotel yang bersinambungan
dengan endotel pembuluh darah yang masuk ke jantung. Di bawahnya terdapat lapisan
subendotel yang mengandung serat kolagen halus.Sedangkan lapisan yang menyatu dengan
miokardium disebut lapisan subendokardial yang terdiri atas jaringan ikat longgaaar.Lapisan
ini mengandung banyak pembuluh darah, saraf dan cabang-cabang sistem hantar rangsang
jantung.
Miokardium
Miokardium atau lapis tengah, yang bersesuaian dengan tunika media terdiri ats otot
jantung. Ketebalannya beragam pada tempat yang berbeda, yang lpaling tipis terdapat pada
kedua atrium dan yang plaling tebal terdpat pada ventrikel kiri.
Lembar otot atrium dan ventrikel melekat berikut dengan jaringan interstisialnya kepada
bangungan penyangga utama jantung yang di sebut kerangka jantung. Kerangka jantung
berupa jaringan ikat padat fibrosa. Komponen yang utama adalah septum membranaseum,
trigonum fibrosum, anulus fibrosus.
Epikardium
Sesuai dengan tunika adventisia pembuluh darah. Merupakan bagian viseral dari
kantung pericardium yang membungkus jantung. Dilapisi oleh sel-sel mesotel yang berbentuk

pipih selapis. Terikat pada myocardium oleh suatu lapisan jaringan ikat longgar vaskuler,
yaitu lapisan subepicardium.
Katub / Valvula
Merupakan lipatan dari endocardium yang di tengahnya diperkuat oleh suatu
lempengan jaringan ikat padat fibroelastis.Kegunaan katub jantung adalah untuk mencegah
kembalinya aliran darah.
Katub atrioventrikuler ( trikuspidal dan mitral ) merupakan lipatan endokardium
bertulangkan jaringan ikat fibrosa yang menyatu dengan anulus fibrosus. Endokardiumnya
lebih tebal pada permukaan yang menghadap atrium daripada yang menghadap ventrikel dan
lebih banyak mengandung serat elastin.Semua katub dihungungkan dengan muskulus
papilaris ventrikel oleh benang fibrosa disebut korda tendinea, yang mengendalikan katup
dan mencegah katup terbalik saat ventrikel berkontraksi.
Katub semiluner aorta dan arteri pulmonalis strukturnya sama dengan katup
atrioventrikuler.Setiap katub terdiri atas tiga daun, bagian tengahnya berupa lempeng fibrosa
setiap katup membentuk penebalan ( nodulus arantius ) pada pinggir bebasnya.

SISTEM LIMFOID
(SEL & JARINGAN DARI SISTEM IMUN)
PENDAHULUAN
Jaringan Limfoid merupakan komponen utama dari Sistem Imun, dan di dalamnya
tersusun dari kumpulan sel-sel yang berperan dalam sistem imun. Fungsi dari Sel dan
Jaringan tersebut adalah sebagai komponen yang sangat berperan dalam sistem pertahanan
tubuh atau sistem imun. Fase Pengenalan dan Fase Aktivasi dari Respon Imun Spesifik
yang diperankan oleh limfosit, makrofag dan sel asesori, terkonsentrasi pada jaringan atau
organ limfoid.

berbagai organ limfoid kaitannya dengan sistem imun.

SISTEM IMUN
Yang dimaksud dengan Sistem imun merupakan semua mekanisme yang digunakan
tubuh untuk mempertahankan keutuhannya, sebagai perlindungan terhadap bahaya yang
dapat ditimbulkan oleh berbagai bahan dalam lingkungan hidup (benda asing).

Pembagian Sistem Imun :
Non Spesifik / Alamiah (Natural Immunity) :
Merupakan sistem pertahanan terdepan yang dapat melawan secara langsung serta
sudah ada/siap sejak lahir.
Komponen yang terlibat :
1. Fisik/Mekanik : Kulit, mukosa, silia, batuk, bersin.
2. Bahan Larut :
a. Biokimia : Asam lambung, Lisosim, Laktoferin dll
b. Humoral : Komplemen, Interferon.
3. Seluler : Fagosit (Makrofag, PMN, Sel NK)
Spesifik / Didapat (Acquired Immunity) :
Merupakan sistem pertahanan yang didapatkan setelah melalui proses pengenalan
dengan benda asing (Antigen), sehingga bersifat spesifik, heterogen dan memiliki
memori.
Komponen yang terlibat :
1. Seluler : Sel T (Thelper = Th1,Th2; Tsupressor =Ts; Tcitotoxic =Tc; Tmemory ).
- Tempat pematangannya terjadi di Timus
- Sel T (Limfosit T) merupakan 60-80% limfosit dalam darah.
- Dapat mengenali antigen yang disajikan oleh MHC (Major Histocompatibility
Complex) pada permukaan sel penyaji / makrofag / sel sasaran.
- Berperan sebagai regulator (Th & Ts) dan efektor (Tc).
2. Humoral : Sel B (Limfosit B).
Sel B merupakan Limfosit yang mengalami pematangan pada Bursa Fabricius
(unggas) atau Bursa Equivalent (mamalia).
Setelah mengalami aktivasi sel B akan berproliferasi dan berdeferensiasi menjadi sel
B matang yang siap memproduksi Antibodi yaitu berupa sel Plasma. Sebagian lagi
ada yang menjadi sel B memori.
PEMBULUH LIMFE
Aliran limfe / getah bening hanya menuju ke satu jurusan, yaitu dari jaringan tubuh ke
sistem vena (bermuara pada vena cava).
Pembuluh getah bening (pembuluh limfe) yang terkecil disebut: kapiler limfe, yang
dindingnya hanya terdiri dari lapisan jaringan ikat tipis sekali dan sel-sel endotel.
Pembuluh limfe yang lebih besar mempunyai dinding yang terdiri dari 3 lapisan :
1. Tunica intima : terdiri atas serat elastis
2. Tunica media : terdiri dari serabut-serabut otot polos sirkuler dan obliq
yang disokong serat elastis.
3. Tunica adventitia : terdiri dari serat otot polos longitudinal dan obliq, juga terdapat
pembuluh darah kecil.

Pembuluh limfe bermuara pada vena cava melalui 2 pembuluh limfe besar, yaitu :
1. DUCTUS THORACICUS, yang mengangkut cairan limfe dari bagian kiri dada dan kepala,
seluruh badan di bawah dada.
2. DUCTUS LYMPHATICUS DEXTRA, mengangkut cairan limfe dari dada & kepala sebelah
kanan.
ORGAN LIMFOID
Organ Limfoid meliputi :
1. Organ Limfoid Primer/Sentral : merupakan tempat pembentukan & pematangan limfosit :
1.1.Timus.
1.2.Sumsum Tulang.
2. Organ Limfoid Sekunder/Perifer : merupakan tempat limfoist memberikan respons
terhadap antigen :
2.1.Limfonodus.
2.2.Limpa/Lien.
2.3.Tonsil.
2.4.Limfonodulus/Nodulus Limfatikus/Nodulus Malpighi.
TIMUS
Merupakan organ limfoid primer.
Bentuk : pipih, terdiri dari 2 lobus
Letak : dalam mediastinum, + setinggi pembuluh darah besar jantung.
Ukuran : tergantung umur.
Dalam masa kandungan sampai dengan 2 tahun post natal, mengalami pertumbuhan sesuai
dengan cepatnya pertunmbuhan badan, tetapi setelah 2 tahun hingga dewasa tak sesuai lagi
dengan cepatnya pertumbuhan badan. Setelah pubertas akan mengecil (involusi).
Berat timus : waktu lahir 12-15 g, pubertas 30-40 g, usia lanjut 10-15 g.Berbeda dengan limfonodus, timus tidak mempunyai pembuluh limfe afferent sehingga tidak
ada cairan limfe yang masuk, juga tidak mempunyai limfonoduli.
Mikroskopis:
Tiap lobus dibungkus oleh kapsula fibrosa yang tipis.
Kapsula dari masing-masing lobus membentuk septa (sekat) yang membagi lobus menjadi
lobulus-lobulus yang ukurannya : 0,5 – 2 mm. Sekat-sekat tadi walaupun masuk sampai
bagian dalam tetapi sering tidak saling berhubungan satu sama lain, sehingga bagian tengah
lobulus satu dengan lobulus lainnya bisa berhubungan.
Tiap-tiap lobulus mempunyai daerah padat limfosit, yaitu daerah dekat kapsula/sekat
yang tampak lebih gelap disebut: Kortex. Sedangkan bagian tengah dimana limfosit lebih
sedikit, sehingga tampak lebih pucat disebut: Medula.
Kortex timus :

Tampak lebih gelap karena padat dengan populasi limfosit T yang sedang
berkembang, sehingga terdapat limfosit T yang muda, sedang dan matang.
Limfosit T pada kortex timus terlindung dari kontak antigen, karena :
tidak ada pembuluh limfe aferen,
antara antigen pada darah dengan limfosit dibatasi oleh : endotel, membran basal,
jaringan perivaskuler, membran basal sel retikuler & sel retikuler,
serta adanya makrofag perivaskuler
Kortex timus merupakan sumber limfosit T. Limfosit T yang dihasilkan akan keluar
dari timus melalui venulae yang ada di antara kortex-medula, tanpa memasuki medula.
Limfosit T yang sudah beredar tidak akan kembali ke kortex, tetapi dapat masuk ke medula
terutama setelah teraktivasi.
Jadi di dalam korteks selain terdapat limfosit T juga terdapat makrofag serta sel retikuler
(dendritik retikuler). Sel plasma sangat jarang ditemukan di kortex.
Medula timus :
Sel-sel retikuler terdapat lebih banyak, sering mengumpul konsentris dan di
tengahnya terdiri dario bahan-bahan yang berdegenerasi mati membentuk keratin. Bagian ini
disebut: Badan Hassal (Corpusculum Hassale). Bagian luar korpuskulum ini dikelilingi lapisan
sel-sel epitel gepeng, makin ke dalam terdapat bagian yang mati. Bangunan ini dianggap
tanda khas dari Timus.
Limfosit di medula lebih sedikit. Limfosit yang berasal dari aliran darah bisa masuk ke
timus pada bagian medula ini, terutama yang sudah teraktivasi. Limfosit B yang memasuki
medula timus bisa teraktivasi menjadi sel plasma.
Hormon timus :
Sel retikuler epitelial menghasilkan hormon timus yang disebut timopoetin dan timosin .
Hormon ini berperan dalam pematangan limfosit T dengan memacu ekspresi petanda
permukaan dari limfosit T.
Hubungan dengan kelenjar endokrin :
Hormon pertumbuhan dari pars anterior Hipofise & hormon Tiroid akan memacu
pertumbuhan kelenjar timus, sedangkan hormon steroid (termasuk hormon seks) akan
mempercepat involusi Timus.
Pembuluh darah :
Arteriole menembus kapsula & septa untuk memberikan cabang kapiler menuju kortex
sampai medula, dan darah arteri akan dialirkan ke venulae terus ke vena.
Di medula lebih banyak pembuluh darah dari pada di kortex.

Fungsi :
Timus berfungsi memproduksi limfosit T, dan sedikit sel plasma. Tidak berfungsi sebagai
filtrasi antigen.
LIMFONODUS
Cairan lime yang dibawa dari jaringan untuk menuju ke ductus thoracicus & ductus
limphaticus dextra terlebih dahulu akan melewati kelenjar-kelenjar limfe yang disebut
Limfonodus atau Limfoglandula atau Nodus Limfatikus.
Limfonodi bergerombol & terdapat pada : axilla, inguinal, poplitea, sepanjang pembuluh-
pembuluh besar leher, sejumlah besar di dalam thorax dan abdomen/mesenterium
Ukuran : + sebesar kacang (panjang 2 cm).
Bentuk : seperti ginjal, mempunyai permukaan cembung dan permukaan cekung.
Pada beberapa tempat di bagian cembung masuk saluran limfe yaitu pembuluh limfe
afferens, sedangkan di bagian cekung (hilus) dimana arteri masuk dan vena keluar ada
saluran limfe keluar dari limfonodus yang disebut pembuluh limfe efferens. Kedua pembuluh
limfe ini mempunyai katub/klep/valvula .
Limfonodus dibungkus oleh kapsula fibrosa, dan di sebelah luarnya juga terdapat jaringan
lemak. Kapsula fibrosa biasanya di hilus lebih tebal, sedangkan di beberapa tempat di
bagian cembung kapsula masuk ke dalam sebagai trabekula untuk menyokong & jalan
pembuluh darah.
Di dalam limfonodus terdapat ruang-ruang :
- Sinus Marginalis / Sinus Subkapsularis : ruang di bawah kapsula fibrosa,
- Sinus Trabekularis : ruang di sekitartrabekula,
- Sinus Medularis : ruang-ruang di medula.
Ruangan-ruangan tsb merupakan tempat penampungan cairan limfe. Dari pembuluh limfe
afferens sinus marginalis sinus trabekularis sinus medularis keluar melalui
pembuluh limfe efferens di hilus. Sebagian kecil cairan limfe menerobos masuk ke parenkim
limfonodus.
Parenkim limfonodus disusun oleh serabut retikuler di antara trabekula beserta sel-sel
retikuler dendritik, makrofag, limfosit dan sel plasma.
Limfonodus, secara mikroskopis dibedakan menjadi bagian-bagian :
Kortex
Merupakan bagian luar (tepi), tersusun dari limfonodulus /nodulus limfatikus/ folikulus
limfatikus.
Nodulus limfatikus/limfonodulus dibedakan menjadi 2 macam yaitu : limfonodulus primer yang
tanpa sentrum germinativum dan limfonodulus sekunder yang di tengahnya terdapat sentrum
germinativum.
Sentrum germinativum tampak lebih pucat karena berisi sel-sel limfosit B yang masih muda
yang mengandung relatif sedikit kromatin, sehingga mengambil lebih sedikit cat dan
warnanya lebih pucat. Sentrum germinativum ini berkembang karena adanya rangsangan

antigen, sehingga limfosit B teraktivasi untuk selanjutnya berdeferensiasi dan berproliferasi di
tempat tsb (daerah mitosis). Sehingga bila ada radang/infeksi maka terjadi pembesaran
sentrum germinativum ini yang menyebabkan limfonodus tampak membesar (pembesaran
limfonodi regional). Limfosit B yang sudah matang / sel plasma akan bermigrasi menuju ke
medula untuk memproduksi Imunoglobulin (Ig, terutama IgG) yang akan dikeluarkan lewat
pembuluh limfe efferens.
Limfonodulus primer berisi limfosit B yang tidak aktif, biasanya ditemukan pada janin/baru
lahir dan pada hewan yang dipelihara secara aseptik.
Medula
Medula limfonodus terdiri dari ruangan-ruangan (sinus medularis) dan jaringan padat limfoid
berbentuk panjang disebut korda medularis (medullary cord) yang banyak berisi sel plasma.
Imunoglobulin yang dihasilkan dari sel plasma di sini akan dikeluarkan melalui pembuluh
limfe efferens.
Daerah parakortex / Daerah parafolikuler
Merupakan daerah kortex dalam atau daerah yang terletak di antara kortex dan medula.
Venulae (lanjutan dari kapiler) di daerah ini mempunyai kekhususan yaitu berupa dinding
endotelnya yang tersusun dari sel kuboid (catatan: endotel biasanya sel gepeng/skuamus),
sehingga dikatakan sebagai venula dengan endotel tinggi (High endothelial venules : HEV)
(Gambar 8-6). Melalui endotel ini sel B & sel T yang berasal dari aliran darah akan menempel
pada endotel dan menerobos masuk ke area sel T atau daerah “thymus dependent” dari
limfonodulus. Sehingga HEV merupakan tempat sel B/sel T menyeberang dari darah ke limfe
dalam jalur resirkulasi di dalam tubuh (Gambar 8-7).
Limfosit T yang bertemu antigen akan menjadi aktif dan berproliferasi di daerah internoduler
untuk kemudian meninggalkan nodus melalui pembuluh limfe efferens.
Fungsi Limfonodus :
1. membersihkan cairan limfe dari benda-benda asing (filter imunologik dari benda asing
yang ada di cairan limfe), terutama untuk respons imun lokal.
2. Menghasilkan limfosit & sel plasma pembentuk antibodi.
LIMPA / LIEN
Lien merupakan organ limfoid trerbesar dalam sistem sirkulasi.
Berbeda dengan limfonodulus, lien tidak dirangkaikan dengan sistem saluran limfe, tetapi
dirangkaikan dengan sistem peredaran darah dan mempunyai banyak sel-sel fagositik.
Lien merupakan pertahanan yang penting terhadap mikroorganisme yang menembus aliran
darah dan juga merupakan tempat destruksi sel-sel darah merah, sehingga merupakan filter
imunologik terhadap benda asing yang ada dalam sistem sirkulasi darah.
Fungsi Limfa

1. menghasilkan limfosit dan sel plasma dalam respons imun terhadap antigen yang masuk
dalam aliran darah, (terutama resposn imun humoral yang akan menghasilkan Ig
terutama IgG).
2. Mengenyahkan eritrosit & trombosit yang rusak/tua (difagosit oleh makrofag). Fe yang
dibebaskan dar Hb akan dipakai untuk membentuk eritrosit baru di sumsum tulang.
3. Memadatkan & menyimpan trombosit.
4. Pada foetus : berperan sebagai organ hemopoesis mieloid.
Struktur Umum
Berwarna merah-ungu karena banyak menampung darah.
Ukuran & bentuknya sesuai kepalan tangan.
Lunak, mudah ruptur/pecah dengan gencetan.
Lien dilapisi kapsula fibrosa yang terdiri dari serat-serat kolagen & eladtis dengan sel-sel
fibroblasnya. Pada manusia terdapat sedikit otot polos, pada binatang lebih banyak .
Seluruh kapsula diliputi peritoneum (lapisan serosa) yang licin, tersusun dari selapis sel
gepeng (epitel skuamus simplex).
Daerah tempat dimana pembuluh darah masuk (a. lienalis) & keluar (v. lienalis) disebut :
Hilus (daerah cekungan), dan di hilus ini kapsula fibrosanya banyak mengadakan
percabangan masuk ke dalam membentuk Trabekula (sebagai kerangka/penyokong lien) &
pembawa pembuluh darah.
Parenkim lien terletak di bawah kapsula fibrosa di antara trabekula-trabekula.
Parenkim lien dibedakan menjadi 2 macam, berdasarkan perangai makroskopisnya :
1. Pulpa Putih (pulpa alba).
2. Pulpa Merah ( pulpa rubra).
Pulpa Putih
Pada irisan lien secara makroskopis, tampak daerah-daerah berbentuk bulat-bulat warna
putih di atas dasar warna merah, yaitu pulpa putih yang tampak seperti pulau-pulau kecil
dengan penampang + 1 mm.
Pulpa putih terdiri dari jaringan retikuler dengan sel-sel limfosit B & sel retikuler dendritik.
Daerah bentuk bulat tadi disebut juga sebagai limfonoduli (folikulus limfatikus = folikel limfoid)
yang mengandung : limfosit terutama sel B, dengan sentrum germinativum yang di tengahnya
dilalui a. sentralis.
Seluruh bentuk bulat (folikulus limfatikus) dengan arteri di dalam dan di sekelilingnya disebut :
korpuskulum Malpighi, yang merupakan ciri unik dari lien.
Daerah yang mengelilingi percabangan arteri, dinamakan periarterioler limphoid sheaths
(PALS) yang berlanjut pada folikel limfoid. Tempat ini dipadati limfosit T terutama T helper.
Sel B aktif yang berproliferasi & berdeferensiasi akan didesak masuk ke zona marginal &
pulpa merah untuk menjadi matang/sel plasma & menghasilkan Ab (terutama IgG).

Pulpa Merah
Berwarna merah (makroskopis), terdiri dari sinusoid yang berisi makrofag & darah, serta
penyokong berupa serat-serat retikuler & kolagen dari trabekula /kapsula.
Ditemukan sel-sel : retikuler dendritik, makrofag, limfosit, lekosit & banyak eritrosit merah.
Sinusoid di dalam pulpa merah merupakan ruangan-ruangan yang menampung darah dari
cabang-cabang arteri untuk disalurkan ke vena-vena.
Sinusoid : lumen lebar, ireguler, di antara sel-sel endotel terdapat celah sempit untuk lewat
eritrosit & makrofag.
Jaringan padat di antara sinusoid tersusun menyerupai pita yang disebut Splenic cord, yang
banyak berisi sel plasma.
Lanjutan dari a. penisili, di medula bercabang menjadi a. penisili & a. selubung.
Daerah yang mengelilingi pulpa putih disebut dan PALS dinamakan Zona Marginalis, yang
banyak terisi makrofag limfosit T helper dan limfosit B. Pada daerah Zona Marginalis dan
PALS terjadi proses penyajian & pengenalan Ag dilanjutkan dengan aktifasi sel B. Sel B yang
teraktifasi akan migrasi ke sentrum germinativum untuk berproliferasi/deferensiasi.
Sirkulasi darah di Lien
Darah masuk dari hilus melalui a. lienalis masuk bercabang-cabang di trabekula : a.
trabekularis keluar dari trabekula & masuk ke dalam folikulus limfatikus di pulpa putih : a.
folikularis (a. sentralis), keluar dari pulpa putih & bercabang di pulpa merah sebagaio : a.
penisili (masih mempunyai otot polos), bercabang lagi menjadi arteriole yang diselubungi
jaringan retikuler tebal, disebut : a. selubung (sheated artery = Hulsen artery).
Pada manusia a. selubung kurang berkembang, tidak terdapat tunika media & selubungnya
langsung melekat endotel.
Dari a. selubung arteri-arteri kecil / kapiler membawa darah ke sinusoid di pulpa merah.
Disini terdapat 3 teori mengenai cara masuknya darah ke sinusoid:
1. Sirkulasi darah terbuka : darah dari kapiler dicurahkan di sekitar sinusoid masuk ke
sinusoid melalui celah di antara sel endotel sinus mengalir ke sistem vena melalui v.
trabekularis v. lienalis.
2. Sirkulasi darah tertutup : darah dari kapiler berhubungan langsung ke sinusoid, tanpa
melalui celah endotel sinusoid ke sistem vena.
3. Campuran antara 1 & 2, dimana ke 2 sistem tersebut bisa terjadi di lien.
Di antara teori ini masih belum ada kesesuaian pendapat.
TONSIL
Organ limfoid ini mengitari isthmus faucium, membentuk bangunan seperti cincin, yang
disebut : Cincin Waldeyer, terdiri dari : 2 tonsila palatina, 1 tonsila faringea, tonsila lingualis &
2 tonsila tubaria.

Tonsila Palatina :
Ada 2 buah, terletak pada pars oralis faring (orofaring), di kiri kanan istmus faucium.
Permukaannya diliputi epitel skuamus kompleks tanpa keratin & mengadakan invaginasi ke
dalam membentuk Kripte (celah). Kripte tadi ada yang bercabang membentuk kripte
sekunder. Jaringan limfoid yang berada di bawah epitel tersusun dari limfonoduli dengan atau
tanpa sentrum germinativum.
Di dalam kripte terdapat benda-benda agak merah disebut : Benda Liur (korpuskulum
Spichel), yang terdiri dari : sel epitel yang rusak, sel limfoisit, sisa makanan &
mikroorganisme.
Di bagian yang menempel pada orofaring, dilapisi kapsula fibrosa yang pada beberapa
tempat membentuk septum.
Kadang-kadang epitel tidak nyata karena limfosit mengadakan infiltrasi ke dalam epitel.
Tonsila membentuk limfosit B & sel plasma yang terutama menghaslkan IgA. Juga dapat
menangkap kuman yang seringkali menyebabkan peradangan tonsil (Tonsilitis). Karena
peradangan ini tonsil membesar, akan mengganggu pernafasan & merupakan fokal infeksi,
sehingga harus diangkat secara operasi : Tonsilektomi.
Tonsila Faringea
Merupakan tonsil tunggal yang terletak pada bagian superoposterior faring.
Pada anak tonsil ini bisa mengalami peradangan & membesar, membentuk adenoid yang
bisa menyumbat jalan nafas. Sedangkan pada dewasa mengalami atrofi.
Susunan mikroskopis hampir sama dengan tonsila palatina, hanya bedanya :
1. Tonsila palatina lebih difus.
2. Permukaannya tidak membentuk kripte tetapi mempunyai lipatan-lipatan.
3. Epitel yang melapisinya : kolumner psudokomplex bersilia dengan sel goblet.
Tonsila Lingualis
Terletak di bawah pangkal lidah.
Mempunyai epitel skuamus komplex tanpa keratin dan ditemukan adanya kripte yang
kadang-kadang bercabang. Di antara kripte banyak limfonoduli yang mempunyai sentrum
germinativum. Saluran-saluran kelenjar mukous dari lidah akan bermuara ke dalam kripte.
LIMFONODULUS / FOLIKEL LIMFOID
Merupakan jaringan limfoid yang terdapat di jaringan ikat longgar dan tak berkapsul.
Terdapat di jaringan di bawah epitel mukosa :
- Saluran nafas bagian atas,
- Saluran cerna,
- Saluran kencing (traktus urinarius).
Tersusun sebagai bulatan dengan penampang + 1 mm, bersifat sementara atau permanen.

Terdiri atas limfosit yang tercat kuat dengan HE, di daerah sentrum germinativum tampak
pucat & lebih padat (tempat aktif terutama bila ada radang).
Sentrum germinativum merupakan tempat proliferasi limfosit B yang teraktifasi.
Sedangkan limfosit T terdapat pada daerah tepinya.
Contohnya banyak ditemukan di :
- kolon,
- appendiks dan
- ileum (Plaques Peyeri / Peyer’s patches).
Imunoglobulin yang dihasilkan terutama IgA yang akan dilepas sebagai sIgA, yang
mempunyai aktivitas :
- antivirus/antibakteri
- antitoksin
- menurunkan daya lekat Ag pada mukosa.
Organ ini merupakan bagian dari MALT (Mucosal Associated Lymphoid Tissues) yang
merupakan pertahanan pertama terhadap benda asing (Ag) yang masuk melalui barier
mukosa.
SISTEM IMUN PADA MUKOSA
Epitel mukosa merupakan barier tubuh dengan lingkungan luar, sehingga merupakan garis
pertahanan pertama terhadap benda asing / antigen yang masuk melalui mukosa.
Termasuk dalam MALT adalah :
- NALT : Nasal associated lymphoid tissues.
- BALT : Bronchial associated lymphoid tissues.
- GALT : Gut associated lymphoid tissues.
- Lymphoid tissues in urogenital tract.
Jaringan limfoid disini tidak berkapsul, letaknya berhubungan langsung dengan epitel
mukosa, serta tidak memiliki pembuluh limfe afferent.
Komponen selulernya terdiri dari :
- Limfosit intraepitel (mayoritas sel T)
- Di lamina propria terdapat Limfosit (sel T, sel B & sel plasma), serta makrofag, eosinofil &
mastosit.
- Folikel limfoid : di usus halus ( Peyer’s patches), appendix, kolon, saluran nafas.
- M (membranous/microfold) cells : diantara sel epitel, tidak mempunyai mikrofili, aktif
mengadakan pinositosis untuk membawa molekul (Ag) dari lumen ke bawah epitel.
Imunoglobulin yang dihasilkan terutama IgA.

SISTEM IMUN PADA KULIT
Kulit merupakan barier fisik antara tubuh dengan dunia luar. Banyak benda asing (antigen)
masuk ke dalam tubuh melalui kulit, sehingga banyak respons imun yang terjadi di kulit.
Komponen selulernya antara lain :
- Keratinosit : dapat memprodksi beberapa sitokin serta kemokin (yang menyebabkan
kemotaksis & aktivasi lekosit).
- Sel Langerhans : derivat dari sel dendritik yang terletak di atas stratum basale dari
epidermis, berperan sebagai sel penyaji (APC).
- Limfosit epidermal : sel T
- Pada Dermis terdapat Limfosit (sel T teraktivasi & T memory) serta makrofag.
PENUTUP

AGING AND THE IMMUNE RESPONSE
INTRODUCTION
The world is not always kind to the human body. Accidental bumps, cuts, and scrapes, chemical and thermal burns, extreme cold, and ultraviolet radiation are just a few of the hazards in our physical environment. Making matters worse, the world around us contains an assortment of viruses, bacteria, fungi, and parasites capable of not only surviving but thriving inside our bodies—and potentially causing us great harm. These organisms, called pathogens, are responsible for many human diseases. Each pathogen has a different lifestyle and attacks the body in a specific way. For example, viruses spend most of their time hiding within cells, whereas many bacteria multiply in interstitial fluids, and the largest parasites burrow through internal organs. A Closer Look: The Nature of Pathogens Many organs and systems work together in an effort to keep us alive and healthy. In this ongoing struggle, the lymphatic system plays a central role. The lymphatic system consists of (1) lymph, a fluid; (2) a network of lymphatic vessels; (3) specialized cells called lymphocytes; and (4) an array of lymphoid tissues and lymphoid organs scattered throughout the body.
AN OVERVIEW OF THE LYMPHATIC SYSTEM
We introduced lymphocytes, the primary cells of the lymphatic system, in Chapters 4 and 19. These cells are vital to our ability to resist or overcome infection and disease. Lymphocytes respond to the presence of (1) invading pathogens, such as bacteria or viruses, 2) abnormal body cells, such as virus-infected cells or cancer cells, and (3) foreign proteins, such as the toxins released by some bacteria. They attempt to eliminate these threats or render them harmless by a combination of physical and chemical attacks.

The body has several physical and chemical barriers that either prevent or slow the entry of infectious organisms. These barriers are known as nonspecific defenses, because they do not distinguish one potential threat from another. In contrast, lymphocytes respond to specific threats. If bacteria invade peripheral tissues, lymphocytes organize a defense against that particular type of bacterium. For this reason, lymphocytes are said to provide a specific defense, known as the immune response. Immunity is the ability to resist infection and disease through the activation of specific defenses.
All the cells and tissues involved with the production of immunity are sometimes considered to be part of an immune system, a physiological system that includes not only the lymphatic system but also components of the integumentary, cardiovascular, respiratory, digestive, and other systems. For example, interactions between lymphocytes and Langerhans cells of the skin are important in mobilizing specific defenses against skin infections. We begin this chapter by examining the organization of the lymphatic system. We shall then consider how the lymphatic system interacts with cells and tissues of other systems to defend the body against infection and disease.
ORGANIZATION OF THE LYMPHATIC SYSTEM
The lymphatic system consists of the following: 1. A network of lymphatic vessels, often called lymphatics, which begin in
peripheral tissues and end at connections to veins. 2. Lymph , a fluid that resembles plasma but contains a much lower
concentration of suspended proteins. 3. Lymphoid organs, which are connected to lymphatic vessels and contain
large numbers of lymphocytes.
FUNCTIONS OF THE LYMPHATIC SYSTEM
The three primary functions of the lymphatic system are:
1. The production, maintenance, and distribution of lymphocytes. Lymphocytes are produced and stored within (1) lymphoid tissues and organs, such as the spleen and thymus, and (2) areas of red bone marrow.
2. The return of fluid and solutes from peripheral tissues to the blood. Capillaries normally deliver more fluid to the tissues than they carry away. The return of tissue fluids through lymphatic vessels maintains normal blood volume and eliminates local variations in the composition of the interstitial fluid.
The distribution of hormones, nutrients, and waste products from their tissues of origin to the general circulation. Substances that originate in the tissues but are for some reason unable to enter the bloodstream directly may do so by way of the lymphatic vessels. For example, lipids absorbed by the digestive tract commonly fail to enter the bloodstream through capillaries. They reach the bloodstream only after they have traveled along lymphatic vessels
LYMPHATIC VESSELS
Lymphatic vessels carry lymph from peripheral tissues to the venous system. The smallest lymphatic vessels are called lymphatic capillaries.
Lymphatic Capillaries

The lymphatic network begins with the lymphatic capillaries, or terminal lymphatics, which branch through peripheral tissues. They differ from blood capillaries in that lymphatic capillaries (1) originate as blind pockets, (2) are larger in diameter, (3) have thinner walls, and (4) in sectional view typically have a flattened or irregular outline. Although lymphatic capillaries are lined by endothelial cells, the basement membrane is incomplete or absent. The endothelial cells of a lymphatic capillary are not tightly bound together, but they do overlap. The region of overlap acts as a one-way valve. It permits the entry of fluids and solutes, even those as large as proteins, as well as viruses, bacteria, and cell debris, but it prevents their return to the intercellular spaces.
Lymphatic capillaries are present in almost every tissue and organ in the body. Prominent lymphatic capillaries in the small intestine are called lacteals; these are important in the transport of lipids absorbed by the digestive tract. Lymphatic capillaries are absent in areas that lack a blood supply, such as the cornea of the eye. The bone marrow and the central nervous system also lack lymphatic vessels.
Small Lymphatic Vessels
From the lymphatic capillaries, lymph flows into larger lymphatic vessels that lead toward the trunk. The walls of these lymphatic vessels contain layers comparable to those of veins, and, like veins, the larger lymphatic vessels contain valves. The valves are quite close together, and at each valve the lymphatic vessel bulges noticeably. As a result, large lymphatic vessels have a beaded appearance. The valves prevent the backflow of lymph within lymph vessels, especially those of the limbs. Pressures within the lymphatic system are minimal, and the valves are essential to maintaining normal lymph flow toward the thoracic cavity.
Lymphatic vessels commonly occur in association with blood vessels. Notice the differences in relative size, general appearance, and branching pattern that distinguish the lymphatic vessels from arteries and veins. Characteristic color differences are also apparent on examining living tissues. Most arteries are bright red; veins are dark red; and lymphatic vessels are a pale golden color. In general, a tissue will contain many more lymphatic vessels than veins, but the lymphatic vessels are much smaller.
Major Lymph-Collecting Vessels
Two sets of lymphatic vessels collect lymph from the lymphatic capillaries: superficial lymphatics and deep lymphatics. Superficial lymphatics are located in the subcutaneous layer deep to the skin; in the areolar tissues of the mucous membranes lining the digestive, respiratory, urinary, and reproductive tracts; and in the areolar tissues of the serous membranes lining the pleural, pericardial, and peritoneal cavities. Deep lymphatics are larger lymphatic vessels. They accompany deep arteries and veins supplying skeletal muscles and other organs of the neck, limbs, and trunk and the walls of visceral organs.
Superficial and deep lymphatics converge to form even larger vessels called lymphatic trunks, which in turn empty into two large collecting vessels: the thoracic duct and the right lymphatic duct. The thoracic duct collects lymph from the body inferior to the diaphragm and from the left side of the body superior to the diaphragm. The smaller right lymphatic duct collects lymph from the right side of the body superior to the diaphragm.

The thoracic duct begins inferior to the diaphragm at the level of vertebra L2. The base of the thoracic duct is an expanded, saclike chamber called the cisterna chyli. The cisterna chyli receives lymph from the inferior part of the abdomen, the pelvis, and the lower limbs by way of the right and left lumbar trunks and the intestinal trunk.
The inferior segment of the thoracic duct lies anterior to the vertebral column. From the second lumbar vertebra, it penetrates the diaphragm alongside the aorta and ascends along the left side of the vertebral column to the level of the left clavicle. After collecting lymph from the left bronchomediastinal trunk, the left subclavian trunk, and the left jugular trunk, it empties into the left subclavian vein near the left internal jugular vein. Lymph collected from the left side of the head, neck, and thorax, as well as lymph from the entire body inferior to the diaphragm, reenters the venous circulation in this way.
Lymphedema
Blockage of the lymphatic drainage from a limb produces lymphedema. In this condition, interstitial fluids accumulate and the limb gradually becomes swollen and grossly distended. If the condition persists, the connective tissues lose their elasticity, and the swelling becomes permanent. Lymphedema by itself does not pose a major threat to life. The danger comes from the constant risk that an uncontrolled infection will develop in the affected area. Because the interstitial fluids are essentially stagnant, toxins and pathogens can accumulate and overwhelm the local defenses without fully activating the immune system.
Temporary lymphedema can result from tight clothing. Chronic lymphedema can result from scar tissue formation owing to repeated bacterial infections or from surgery that cuts or removes lymphatic vessels. Breast cancer surgery or radiation treatment can lead to lymphedema of the upper limb on the affected side. Lymphedema can also result from parasitic infections; in filariasis, larvae of a parasitic roundworm, generally Wuchereria bancrofti, are transmitted by mosquitoes or black flies. The adult worms form massive colonies within lymphatic vessels and lymph nodes. Repeated scarring of the passageways eventually blocks lymphatic drainage and produces extreme lymphedema with permanent distension of tissues. The limbs or external genitalia typically become grossly distended, a condition known as elephantiasis.
Therapy for chronic lymphedema consists of treating infections by the administration of antibiotics and (when possible) reducing the swelling. One possible treatment involves the application of elastic wrappings that squeeze the tissue. This external compression elevates the hydrostatic pressure of the interstitial fluids and opposes the entry of additional fluid from the capillaries.
LYMPHOCYTES
Lymphocytes account for 20–30 percent of the circulating white blood cell population. However, circulating lymphocytes are only a small fraction of the total lymphocyte population. The body contains some 1012 lymphocytes, with a combined weight of over a kilogram.
TYPES OF LYMPHOCYTES
Three classes of lymphocytes are in blood: (1) T ( t hymus-dependent) cells , (2) B ( b one marrow–derived) cells , and (3) NK (natural killer) cells. Each type has distinctive biochemical and functional characteristics.

T Cells Approximately 80 percent of circulating lymphocytes are classified as T cells. Many types of T cells exist, including the following:
Cytotoxic T cells, which attack foreign cells or body cells infected by viruses. Their attack commonly involves direct contact. These lymphocytes are the primary cells involved in the production of cell-mediated immunity, or cellular immunity.
Helper T cells, which stimulate the activation and function of both T cells and B cells.
Suppressor T cells, which inhibit the activation and function of both T cells and B cells.
The interplay between suppressor and helper T cells helps establish and control the sensitivity of the immune response. For this reason, these cells are also known as regulatory T cells.
These are the T cells that we will examine in the course of this chapter. It is not a complete list, however; other types of T cells participate in the immune response. For example, inflammatory T cells stimulate regional inflammation and local defenses in an injured tissue, and suppressor/inducer T cells suppress B cell activity but stimulate other T cells.
B Cells B cells account for 10–15 percent of circulating lymphocytes. When stimulated, B cells can differentiate into plasma cells. Plasma cells are responsible for the production and secretion of antibodies, soluble proteins that are also known as immunoglobulins. These proteins bind to specific chemical targets called antigens. Most antigens are pathogens, parts or products of pathogens, or other foreign compounds. Most antigens are proteins, but some lipids, polysaccharides, and nucleic acids can also stimulate antibody production. When an antibody binds to its target antigen, a chain of events begins that leads to the destruction of the target compound or organism. B cells are responsible for antibody-mediated immunity, which is also known as humoral ("liquid") immunity because antibodies occur in body fluids.
NK Cells The remaining 5–10 percent of circulating lymphocytes are NK cells, also known as large granular lymphocytes. These lymphocytes attack foreign cells, normal cells infected with viruses, and cancer cells that appear in normal tissues. Their continuous policing of peripheral tissues has been called immunological surveillance.
Life Span and Circulation of Lymphocytes
Lymphocytes are not evenly distributed in the blood, bone marrow, spleen, thymus, and peripheral lymphoid tissues. The ratio of B cells to T cells varies with the tissue or organ considered. For example, B cells are seldom found in the thymus, but in blood, T cells outnumber B cells by a ratio of 8:1. This ratio changes to 1:1 in the spleen and 1:3 in bone marrow.
The lymphocytes in these organs are visitors, not residents. All types of lymphocytes move throughout the body. They wander through a tissue and then enter a blood vessel or a lymphatic vessel for transport.
T cells move relatively quickly. For example, a wandering T cell may spend about 30 minutes in the blood, 5–6 hours in the spleen, and 15–20 hours in a lymph node. B cells, which are responsible for antibody production, move more slowly. A typical B cell spends about 30 hours in a lymph node before moving on.

Lymphocytes have relatively long life spans. Roughly 80 percent survive for 4 years, and some last 20 years or more. Throughout your life, you maintain normal lymphocyte populations by producing new lymphocytes in your bone marrow and lymphoid tissues.
Lymphocyte Production
In Chapter 19, we discussed hemopoiesis, the formation of the cellular elements of blood. Erythropoiesis (red blood cell formation) in adults is normally confined to bone marrow, but lymphocyte production, or lymphopoiesis, involves the bone marrow, thymus, and peripheral lymphoid tissues. Bone marrow plays the primary role in the maintenance of normal lymphocyte populations. Hemocytoblast divisions in the bone marrow of adults generate the lymphoid stem cells that produce all types of lymphocytes. Two distinct populations of lymphoid stem cells are produced in the bone marrow.
One group of lymphoid stem cells remains in the bone marrow. Divisions of these cells produce immature B cells and NK cells. B cell development involves intimate contact with large stromal cells (stroma, a bed) in the bone marrow. The cytoplasmic extensions of stromal cells contact or even wrap around the developing B cells. Stromal cells produce an immune system hormone, or cytokine, called interleukin-7, which promotes the differentiation of B cells. (We will consider cytokines and their varied effects in a later section.)
As they mature, B cells and NK cells enter the bloodstream and migrate to peripheral tissues. Most of the B cells move into lymph nodes, the spleen, or other lymphoid tissues. The NK cells migrate throughout the body, moving through peripheral tissues in search of abnormal cells.
The second group of lymphoid stem cells migrates to the thymus While in the thymus, these cells and their descendants develop further in an environment that is isolated from the general circulation by the blood–thymus barrier. Under the influence of thymic hormones, the lymphoid stem cells divide repeatedly, producing the various kinds of T cells. At least seven thymic hormones have been identified. Their precise functions and interactions have yet to be determined.
When their development is nearing completion, T cells reenter the bloodstream and return to the bone marrow. They also travel to lymphoid tissues and organs, such as the spleen.
The T cells and B cells that migrate from their sites of origin retain the ability to divide. Their divisions produce daughter cells of the same type; for example, a dividing B cell produces other B cells, not T cells or NK cells. As we shall see, the ability to increase the number of lymphocytes of a specific type is important to the success of the immune response.
LYMPHOID TISSUES
Lymphoid tissues are connective tissues dominated by lymphocytes. In a lymphoid nodule, or lymphatic nodule, the lymphocytes are densely packed in an area of areolar tissue. Lymphoid nodules occur in the connective tissue deep to the epithelia lining the respiratory, digestive, and urinary tracts. Typical nodules average about a millimeter in diameter, but the boundaries are not distinct, because no fibrous capsule surrounds them. They commonly have a central zone called a germinal center, which contains dividing lymphocytes.

MALT
The collection of lymphoid tissues linked with the digestive system is called the mucosa-associated lymphoid tissue (MALT). Clusters of lymphoid nodules deep to the epithelial lining of the intestine are known as aggregate lymphoid nodules, or Peyer’s patches .
In addition, the walls of the vermiform appendix, a blind pouch that originates near the junction between the small and large intestines, contain a mass of fused lymphoid nodules.
Tonsils
Large nodules in the walls of the pharynx are called tonsils (Figure 22-6b ). Most people have five tonsils:
1. A single pharyngeal tonsil, often called the adenoids, located in the posterior superior wall of the nasopharynx.
2, 3. A pair of palatine tonsils, located at the posterior margin of the oral cavity, along the boundary with the pharynx.
4, 5. A pair of lingual tonsils, which are usually not visible because they are located under the attached base of the tongue.
Infected Lymphoid Nodules The lymphocytes in a lymphoid nodule are not always able to destroy bacterial or viral invaders that have crossed the adjacent epithelium. If pathogens become established in a lymphoid nodule, an infection develops. Two examples are probably familiar to you: tonsillitis, an infection of one of the tonsils (generally the pharyngeal or palatine), and appendicitis, an infection of the vermiform appendix that begins in the lymphoid nodules. Treatment commonly consists of antibiotic therapy, sometimes combined with the surgical removal of the infected tissues. Infected Lymphoid Nodules
LYMPHOID ORGANS
A fibrous connective tissue capsule separates lymphoid organs from surrounding tissues. These organs include the lymph nodes, the thymus, and the spleen.
Lymph Nodes
Lymph nodes are small, oval lymphoid organs ranging in diameter from 1 to 25 mm (to about 1 in.). The general pattern of lymph node distribution in the body. Each lymph node is covered by a capsule of dense connective tissue. Bundles of collagen fibers extend from the capsule into the interior of the node. These fibrous partitions are called trabeculae (trabecula, a wall).
The shape of a typical lymph node resembles that of a kidney bean. Blood vessels and nerves attach to the lymph node at the hilus, a shallow indentation. Two sets of lymphatic vessels are connected to each lymph node: afferent lymphatics and efferent lymphatics.

1. Afferent lymphatics carry lymph to the lymph node from peripheral tissues. The afferent lymphatics penetrate the capsule of the lymph node on the side opposite the hilus.
2. Efferent lymphatics are attached to the lymph node at the hilus. These vessels carry lymph away from the lymph node and toward the venous circulation.
Lymph Flow Lymph delivered by the afferent lymphatics flows through the lymph node within a network of sinuses, open passageways with incomplete walls. Lymph first enters a subcapsular sinus, which contains a meshwork of branching reticular fibers, macrophages, and dendritic cells. Dendritic cells are involved in the initiation of the immune response; we shall consider their role in a later section. After passing through the subcapsular sinus, lymph flows through the outer cortex of the node. The outer cortex contains B cells within germinal centers that resemble those of lymphoid nodules.
Lymph then continues through lymph sinuses in the deep cortex (paracortical area). Lymphocytes leave the bloodstream and enter the lymph node by crossing the walls of blood vessels within the deep cortex. The deep cortical area is dominated by T cells.
After flowing through the sinuses of the deep cortex, lymph continues into the core, or medulla, of the lymph node. The medulla contains B cells and plasma cells organized into elongate masses known as medullary cords. After passing through a network of sinuses in the medulla, lymph enters the efferent lymphatics at the hilus.
Lymph Node Function A lymph node functions like a kitchen water filter: It filters and purifies lymph before that fluid reaches the venous circulation. As lymph flows through a lymph node, at least 99 percent of the antigens in the lymph are removed. Fixed macrophages in the walls of the lymphatic sinuses engulf debris or pathogens in lymph as it flows past. Antigens removed in this way are then processed by the macrophages and "presented" to nearby lymphocytes. Other antigens bind to receptors on the surfaces of dendritic cells, where they can stimulate lymphocyte activity. This process, antigen presentation, is generally the first step in the activation of the immune response.
In addition to filtering, lymph nodes provide an early-warning system. Any infection or other abnormality in a peripheral tissue will introduce abnormal antigens into the interstitial fluid and thus into the lymph leaving the area. These antigens will then stimulate macrophages and lymphocytes in nearby lymph nodes.
To protect a house against intrusion, you might guard all entrances and exits or place traps by the windows and doors. The distribution of lymphoid tissues and lymph nodes follows such a pattern. The largest lymph nodes are located where peripheral lymphatics connect with the trunk, such as in the groin, the axillae, and the base of the neck. These nodes are often called lymph glands. Because lymph is monitored in the cervical, inguinal, or axillary lymph nodes, potential problems can be detected and dealt with before they affect the vital organs of the trunk. Aggregations of lymph nodes also exist in the mesenteries of the gut, near the trachea and passageways leading to the lungs, and in association with the thoracic duct. These lymph nodes protect against pathogens and other antigens within the digestive and respiratory systems.

A minor injury commonly produces a slight enlargement of the nodes along the lymphatic vessels draining the region. This symptom, often called "swollen glands," typically indicates inflammation or infection of peripheral structures. The enlargement generally results from an increase in the number of lymphocytes and phagocytes in the node in response to a minor, localized infection. Chronic or excessive enlargement of lymph nodes constitutes lymphadenopathy. This condition may occur in response to bacterial or viral infections, endocrine disorders, or cancer.
The Thymus
The thymus is located in the mediastinum, generally just posterior to the sternum. It is pink and has a grainy consistency. In newborn infants and young children, the thymus is relatively large, commonly extending from the base of the neck to the superior border of the heart. The thymus reaches its greatest size (relative to body size) in the first year or two after birth. Although the thymus continues to increase in size throughout childhood, the body as a whole grows even faster, so the size of the thymus relative to that of the other organs in the mediastinum gradually decreases. The thymus reaches its maximum absolute size, at a weight of about 40 g (1.4 oz), just before puberty. After puberty, it gradually diminishes in size and becomes increasingly fibrous; this process is called involution. By the time an individual reaches age 50, the thymus may weigh less than 12 g (0.3 oz). It has been suggested that the gradual decrease in the size and secretory abilities of the thymus may make the elderly more susceptible to disease.
The capsule that covers the thymus divides it into two thymic lobes. Septae, fibrous partitions that originate at the capsule, divide the lobes into lobules, which average 2 mm in width. Each lobule consists of a densely packed outer cortex and a paler, central medulla. Lymphocytes in the cortex are dividing; as the T cells mature, they migrate into the medulla. After roughly 3 weeks, these T cells leave the thymus by entering one of the medullary blood vessels.
Lymphocytes in the cortex are arranged in clusters that are completely surrounded by reticular epithelial cells. These cells, which developed from epithelial cells of the embryo, also encircle the blood vessels of the cortex. The reticular epithelial cells (1) maintain the blood–thymus barrier and (2) secrete the thymic hormones that stimulate stem cell divisions and T cell differentiation.
As they mature, T cells leave the cortex and enter the medulla of the thymus. The medulla has no blood–thymus barrier. The reticular epithelial cells in the medulla cluster together in concentric layers, forming distinctive structures known as Hassall’s corpuscles. Despite their imposing appearance, the function of Hassall’s corpuscles remains unknown. T cells in the medulla can enter or leave the bloodstream across the walls of blood vessels in this region or within one of the efferent lymphatics that collect lymph from the thymus.
Hormones of the Thymus The thymus produces several hormones that are important to the development and maintenance of normal immunological defenses. Thymosin is the name originally given to an extract from the thymus that promotes the development and maturation of lymphocytes. The thymic extract actually contains a blend of several complementary hormones: thymosin-a, thymosin-b, thymosin V, thymopoietin, thymulin, and others. The term thymosins now is sometimes used to refer to all thymic hormones.
The Spleen

The adult spleen contains the largest collection of lymphoid tissue in the body. It is about 12 cm (5 in.) long and weighs on average nearly 160 g (5.6 oz). The spleen lies along the curving lateral border of the stomach, extending between the ninth and eleventh ribs on the left side. It is attached to the lateral border of the stomach by the gastrosplenic ligament, a broad band of mesentery.
Functions of the Spleen In gross dissection, the spleen is deep red owing to the blood it contains. In essence, the spleen performs the same functions for blood that lymph nodes perform for lymph. Functions of the spleen can be summarized as (1) the removal of abnormal blood cells and other blood components by phagocytosis, (2) the storage of iron from recycled red blood cells, and (3) the initiation of immune responses by B cells and T cells in response to antigens in circulating blood.
Surfaces of the Spleen The spleen has a soft consistency, so its shape primarily reflects its association with the structures around it. It is in contact with the stomach, the left kidney, and the muscular diaphragm. The diaphragmatic surface is smooth and convex, conforming to the shape of the diaphragm and body wall. The visceral surface contains indentations of the shape of the stomach (the gastric area) and of the kidney (the renal area).
Splenic blood vessels and lymphatic vessels communicate with the spleen on the visceral surface at the hilus, a groove marking the border between the gastric and renal areas. The splenic artery, the splenic vein, and the lymphatic vessels draining the spleen are attached at the hilus.
Histology of the Spleen The spleen is surrounded by a capsule containing collagen and elastic fibers. The cellular components within constitute the pulp of the spleen
Red pulp contains large quantities of red blood cells, whereas white pulp resembles lymphoid nodules.
The splenic artery enters at the hilus and branches to produce a number of arteries that radiate outward toward the capsule. These trabecular arteries branch extensively, and their finer branches are surrounded by areas of white pulp. Capillaries then discharge the blood into the red pulp.
The cell population of the red pulp includes all the normal components of circulating blood, plus fixed and free macrophages. The structural framework of the red pulp consists of a network of reticular fibers. The blood passes through this meshwork and enters large sinusoids, also lined by fixed macrophages. The sinusoids empty into small veins, and these ultimately collect into trabecular veins that continue toward the hilus.
This circulatory arrangement gives the phagocytes of the spleen an opportunity to identify and engulf any damaged or infected cells in circulating blood. Lymphocytes are scattered throughout the red pulp, and the area surrounding the white pulp has a high concentration of macrophages and dendritic cells. Thus any microorganism or other antigen in the blood will quickly come to the attention of the splenic lymphocytes.
Cancer and the Lymphatic System Lymphatic vessels are located in almost all portions of the body except the central nervous system, and lymphatic capillaries offer little resistance to the passage of cancer cells. As a result, metastasizing cancer cells commonly spread along lymphatic vessels. Under these circumstances, the lymph nodes serve as way

stations for migrating cancer cells. Thus, an analysis of lymph nodes can provide information on the spread of the cancer cells, and such information helps determine the appropriate therapies. We shall discuss one example: identifying the stages of breast cancer by the degree of nodal involvement. Lymphomas, one group of cancers originating in the lymphatic system, are discussed in the Applications Manual. Lymphomas
Injury to the Spleen
An impact to the left side of the abdomen can distort or damage the spleen. Such injuries are known risks of contact sports, such as football and hockey, and of more-solitary athletic activities, such as skiing and sledding. However, the spleen tears so easily that a seemingly minor blow to the side can rupture the capsule. The result is serious internal bleeding and eventual circulatory shock. The spleen can also be damaged by infection, inflammation, or invasion by cancer cells.
Because the spleen is relatively fragile, it is very difficult to repair surgically. (Sutures typically tear out before they have been tensed enough to stop the bleeding.) A severely ruptured spleen is removed, a process called a splenectomy. A person without a spleen survives without difficulty but has a greater risk for bacterial infections than do individuals with a functional spleen.
THE LYMPHATIC SYSTEM AND BODY DEFENSES
The human body has multiple defense mechanisms, but these can be sorted into two general categories:
1. Nonspecific defenses do not distinguish one type of threat from another. Their response is the same, regardless of the type of invading agent. These defenses, which are present at birth, include physical barriers, phagocytic cells, immunological surveillance, interferons, complement, inflammation, and fever. They provide a defensive capability known as nonspecific resistance.
2. Specific defenses protect against particular threats. For example, a specific defense may protect against infection by one type of bacterium but ignore other bacteria and viruses. Many specific defenses develop after birth as a result of accidental or deliberate exposure to environmental hazards. Specific defenses depend on the activities of lymphocytes. The body’s specific defenses produce a state of protection known as immunity, or specific resistance.
Nonspecific and specific resistances are complementary. Both must function normally to provide adequate resistance to infection and disease.
