NATURAL HISTORY OF 407 UNTREATED HCC(Multicentric Italian Study Group on HCC)
ESTIMATED SURVIVAL RATE OF SINGLE HCC <5 cm (n=131)
Livraghi, Bolondi et al, J Hepatol, 1995
Justifications du Dépistage
• Arguments épidémiologiques • Amélioration significative de l’histoire
naturelle• Méthode simple peu onéreuse :
Echographie +++Alpha Foeto-Protéïne

VARIABLE GROWTH SPEED OF SMALL HCC
313 patients follow-up de 56 31 mois
74 nodules (23.6 %)
13 cas non CHC 61 CHC (19.5 %)
Bolondi et al. Gut 2001
Moyens• Gold standard : Radiologie
Recommandations Barcelone : 2000EASL 2001AASLD 2005
• Alpha foeto-protéïne • Futur : diagnostic histo-moléculaire du CHC

Clinical impact of screening/surveillance programs for HCC in cirrhosis
Effect on survival
Bolondi, Gut 2001
Group 1: surveilled patients
Group 2: unscreened patients
Dépistage du CHC sur cirrhose
ELIGIBLE A LA TH
CHC diagnostiqué au coursd’un programme de
surveillance
surveillance
p < 0.0126 % 13 % (Bolondi, Gut 2001)
p < 0.00168.5 % 32.3 %
(Trevisani, Am J Gastro 2002)
Dépistage du CHC sur cirrhose
ELIGIBLE A U TRT CURATEUR
CHC diagnostiqué au coursd’un programme de
surveillance
surveillance

SCREENING FOR HCC IN CIRRHOSISANALYSIS OF SURVIVAL BENEFIT
Significant longer survivals for screened vs non screened

No Significant difference (Sarasin, Am J Med 1996)*
