PFO
Storia
sani

PFO
Quando ?
• Ictus giovanile (<45 anni)• Emicrania
• Apnee ostruttive del sonno• Subacquei
• Chirurgia ortopedica (anca, ginocchio, colonna)
• Neurochirurgia in posizione seduta
• Ictus ( criptogenico )• Emicrania• Scuba divers• Chirurgia ortopedica (anca,
ginocchio, colonna)• Neurochirurgia in posizione
seduta• Ipertensione polmonare• Apnee ostruttive del sonno
Epidemiologia PFO
Thompson ,1930;Hagen, 1984
• Ecocardiografia Transtoracica (cTTE)• Ecocardiografia Transesofagea (cTEE)• Doppler Transcranico (cTCD)
Sensibilità e specificità del cTCD
Droste et al., ‘99
sensibilità: 95%
specificità:75%
• Paz.posizione supina
• Iniezione in bolo e rapidamente a paziente a riposo
Consensus Meeting,Venezia ESNCH ’99
E.Zanette,C.Jauss
Quale M.D.C. ?
Fan, 1999
• Echovist 300 superiore alla soluzione salina agitata
Droste 2002
TCD finestra acustica
MV
Dr Peter Wilmshurst (Royal Shrewsbury Hospital, UK), refused to sign off on the MIST paper, in part because he was never given full access to the trial data. He also protested the fact that the Circulation paper made no mention of a second echo review that the trial sponsor had, in fact, asked him to conduct.
Wilmshurst's echo review, never published, pointed to a higher residual shunt rate than the 6% reported in the MIST manuscript.
Late last year, the British High Court ordered NMT to pay £200 000 in security to the court to cover the legal costs for Wilmshurst should NMT lose its libel suit against him, an amount ultimately raised to £211 000.

Is Migraine Associated with Right-to-Left Shunt a Separate Disease? Results of the SAM Study
• Anzola GP, e coll.
• 460 casi • Shunt Associated Migraine ( SAM ) Shunt Unassociated
Migraine ( SUM )
Cephalalgia, April 2008; 28, 4:360-366.,

• PFO alone does not portend an increased risk of subsequent stroke or death in patients who have had a cryptogenic stroke and are treated medically
• Coexisting ASA is a substantial and probably the most important potentiator of stroke risk in patients with PFO thus
• Patients with PFO and ASA could be defined « high-risk» patients for recurrent events

PFO predictor
… failed to identify PFO as an independent factor for a first-in-a-lifetime stroke
Meissner, 2006; Di Tullio,2007
Both SPARC & NOMAS studies suggest that the risk of stroke from PFO in the general population is low…
Mattle, 2010
.. Thus PFO could be regarded as an independent risk factor for stroke similar to hypertension, diabetes, hypercholesterolemia although with less weight than the traditional risk factors

Randomized Evaluation of Recurrent Stroke Comparing PFO Closure to Established Current Standard of Care Treatment (RESPECT) and PC Trial both missed their primary end point. The two trials had assessed whether a patent foramen ovale (PFO) closure device could reduce the risk of recurrent stroke in patients who had experienced cryptogenic stroke.

Closure I, Respect, PC-Trial
RESPECT : Analisi post-hoc ( Carroll J. Oral com TCT 2012)
