Who We AreWhen I was young, my grandfather was diagnosed with Parkinson’sDisease. Despite this challenge, he led an active life and used to takeshort walks around the block near his home in Green Bay, WI.

One day, Grandpa Charlie went out for a walk and never came home.He was a missing person for a year until his remains were discovered.

Every time I think of how homecare can help one of my customers, Ithink about my grandfather and how proper in-home care from agood provider could have saved him from this tragedy.

How services are provided

Although bundled visits may be provided, services are generally provided in hourly

increments, with a XX hour minimum, up to 24 x 7 coverage

• During the initial phone intake process

– In-home Visit/Assessment is scheduled

How services are delivered

• In-Home Visit

– FirstLight HomeCare meets with the client, family caregiver and other involved parties

– Completes a basic assessment of the client, environment, family support structure, etc.

– Orients family on what to expect when receiving care

– Verification of date for start of care

How services are delivered

• Office staff chooses and schedules the appropriate caregiver

• Staff goes through a mini orientation based on client’s particular needs and preferences.

• Caregiver is introduced to the Client.

• Regular Billing Cycle

• Supervision by FirstLight HomeCare staff occurs on regular basis

– Gives clients a chance to reassess their needs

– Supervision of staff delivering the care

• Phone screened• Application process includes:

Why use FirstLight HomeCare over a private hire?

• Private Hire(PH) often not covered by Workers Comp

• PH screening process often incomplete (i.e. background check, drug tests, reference checks)

• No formal supervision of PH employee

• No oversight of care delivered
