In un recente studio è stata esaminata la relazione tra funzione tiroidea, funzione cardiaca e mortalità in 458 pazienti (follow-up medio 17 mesi) con scompenso cardiaco da cardiomiopatia dilatativa idiopatica: · il 12% dei pazienti era affetto da ipotiroidismo (conclamato nel 3% e subclinico
nel 9%), · il 7% da ipertiroidismo subclinico e · il 4% da sindrome da bassa T3 (TSH ed FT3 inferiori ai valori normali). Al ridursi di FT3 e all’aumentare di TSH la funzione cardiaca è risultata peggiore (classi NYHA III e IV) nell’analisi trasversale e la mortalità è aumentata nell’analisi di sopravvivenza. Considerando la funzione tiroidea globale piuttosto che i singoli parametri ormonali, l’ipotiroidismo conclamato è risultato il più forte predittore di mortalità (RR = 4.2), seguito dalla sindrome da bassa T3 (RR = 3.1) e dall’ipotiroidismo subclinico (RR = 2.9), mentre l’ipertiroidismo subclinico non ha mostrato impatto sulla mortalità. Risultati del tutto analoghi sono stati riscontrati limitando l’analisi alla mortalità cardiovascolare.
