Most patients who suffer a hemorrhage while on longtermanticoagulant therapy continue to be at risk of thrombosis.

Indicazioni alla terapia con warfarin: 50.5% FA, 24.4 % VTE, 15.6 % protesi valvolari (MHV).
La ripresa del trattamento con Warfarin si associava ad una incidenza di TE e a una mortalità significativamente più bassa.
Inoltre c’era una maggiore incidenza di emorragie GI nel gruppo che riprendeva l’anticoagulante
Limite: retrospettiva, monocentrica, in entrambi i gruppi non veniva cosiderata la terapia antiaggregante.

Coorte prospettica di 3409 pts con nonvalvular AF complicata da GIB con follow-up mediano di 2 anni.

Lo studio ha confermato che la ripresa di un singolo anticoagulante correla con una ridotta incidenza di TE e mortalità se confrontati con l’interruzione o la sospensione della terapia anticoagulante.
Le recidive di GIB erano significativamente più alte nel gruppo di ripresa della terapia.

Le linee guida della American College of Gastroenterology
suggeriscono la ripresa della terapia antiaggregante al più presto,
comunque entro 7 giorni (strong recommendation, low-quality
evidence).
Questa raccomandazione riguarda la terapia antiaggregante e per i
pazienti con sanguinamento dal basso intestino (lower risk of GIB
recurrence).
Non vi sono raccomandazioni per la tempistica in cui ripredere la
terapia anticoagulante dopo sanguinamento maggiore.
