1/L’ effet de l’aspirine est indiscutable chez des patientes à haut risque pour ce qui concerne la pré– éclampsie,la mortalité périnatale ,la prématurité (en particulier induite et vraisemblablement le rciu
2/Un traitement précoce à une dose efficace(Généralement 100 mg) est nécessaire
3/Certaines indications sont établiesLes résultats sont discordants pour d’autres
4/Une prescription précoce basée sur le doppler utérin à 12 sa semble être une voie prometteuse.

ASPIRINE : Modalités de prescription
La dose : en moyenne 100 mg selon le poids, peut être majorée à 150
mg, selon le temps de saignement peut être minorée à 50 ou 60
mg/ jour.
Début : le plus tôt possible, en général (antécédent) vers 12 à 15
semaines. Dans certains cas (échec antérieur d’Aspirine ou échec
précoce de grossesse) plus tôt vers 8 semaines parfois même en pré-
conceptionnel.
Produits associés : dans un contexte « immunitaire » parfois utile
d’associer aux corticoïdes en particulier si syndrome des
antiphospholipides.
En cas de lupus ou de syndrome des antiphospholipides souvent
combiné à l’Héparine.
Des prescriptions plus tardives sont possibles jusqu’à 22- 23 semaines, mais avec une effi cacité bien moindre
Aspirine : Modalités de prescription (suite)
Précautions :
- Eliminer les contre- indications à l’Aspirine et les hypersensibilités.
- Temps de saignement avant la prescription et après la prescription
(au moins 10 jours
- Le temps de saignement sera eff ectué par la méthode de I VY
- Dans la plupart des cas, arrêt vers 34- 35 semaines permettant
une péridurale.
- Le temps de saignement doit se situer entre 4 et 8 minutes,
idéalement il doit légèrement augmenter (tout en restant dans ces
normes) après utilisation d’Aspirine.
