CC ชาที่แขน-ขา ลิ้นแข็งPI 6 ชม.กอน นําปลามาตมกิน 2-3 ตัว โดยกินกับลูกชาย
แตลูกกินแตเนือ 2-3 คํา ที่เหลือผูปวยกินหมดทั้งตัว 2 ชม. กอนมารพ. ผูปวยรูสึกมีอาการชารอบปาก ชาปลายมือ
ปลายเทา เวียนศรีษะ คลื่นไส อาเจียน ลิ้นแข็ง 1 ชม.กอน ผูปวยพูดไมชัด กลืนลําบาก เกร็งกระตุก ตองถูก
intubate และ ใสเครื่องชวยหายใจลูกชายไมมีอากการใดๆผิดปกติ
ผูปวย 5 คน จังหวัดนาน
ผูปวยทั้งหมด 5 คน มารพ.นานดวยอาการปวดทอง ทองเสีย เวียนศีรษะ ตาพรา บางรายเห็นภาพซอน เจ็บคอ ลิ้นแข็ง พูดไมชัดและกลืนลําบาก
2 ใน 5 คนมีกลามเนื้อแขนขาออนแรง ตอมาเกิด cardiac arrest ไดรับการ CPR แตไมขึ้น และเสียชีวติ
สวนอีก 3 คน ตองใชเครื่องชวยหายใจ

Tetrodotoxin-resistant voltage-gated sodium channel(TTX-r Na+ channel) - TTX binds with low micromolar affinity - Located in cardiac conduction system

Tetrodotoxin is a heat-stable (except inalkaline environments) and water-soluble nonprotein
TTX is thought to be synthesized by a bacterial or dinoflagellate species

Tetrodotoxin (TTX)
Tetrodotoxin is a heat-stable (except inalkaline environments) and water-solublenonprotein.
TTX is thought to be synthesized by a bacterial or dinoflagellate species.
The toxin is concentrated in the liver, gonads,and skin puffer fish.
The level of toxicity is variable depending on season.

TTX poisoning
Onset: 15 min – 3 hr (may up to 20 hr)
Initial symptoms lip and tongue paresthesias followed by facial and extremity
paresthesias and numbness

A rapid ascending paralysis occurs over4-24 hr Extremity paralysis Bulbar paralysis Respiratory muscle paralysis Deep tendon reflexes are preserved early
in the course of paralysis.
TTX poisoning
Severe cases cardiac dysfunction with hypotension and
dysrhythmias (bradycardia) central nervous system (CNS) dysfunction (eg,
coma), and seizures develop
Death can occur within 4-6 hr Typically, death occurs from respiratory
muscle paralysis and respiratory failure

Unfavorable condition: spores
– Tolerates boiling temp up to 100ºC > 1 hours

– Low nitrite level

A 900 KDalton single polypeptide chain– Neurotoxic component (150 KDa)

Heat labile– Destroyed by heat
• 80ºC for 30 min or • 100ºC for 10 min• High attitude needs higher temp.
Botulinum toxin: mechanism of toxicity
Blocking Acetylcholine (ACh) release– Neuromuscular junction
– Presynaptic ganglia
– Postsynaptic ganglia of sympathetic and parasympathetic fibers
The inhibition process is irreversible.

Botulinum antitoxin: rationale
Botulinum antitoxin generally neutralizes the toxin in the circulation.
It can prevent progression of the neurological deficit, but cannot reverse the existing symptoms.
Botulinum antitoxin: rationale
Patients who have progressive diseases will get the most benefit from the therapy.
It can shorten the clinical course and decrease the mortality rate.
Efficacy of the antitoxin decreases with time.

Factors contributing to the outcome
Quality of respiratory care– Recent respiratory care is far advanced
and better than the past.
Antitoxin therapy– Antitoxin, even late administration,
might also have some beneficial effects
