The Broward Regional Health Planning Council (BRHPC) is proud to present the Broward County HIV/AIDS Service
Directory. The intent of this guide is to provide information about available resources to consumers, their families and
friends, community members, and other service providers in Broward County. The directory distinguishes Ryan White
Part A services, but also includes other services that are available to individuals affected and/or infected with HIV/AIDS.
Please note that resource information in our community may change. While staff members have made every effort to
provide accurate content, there may have been corrections that were not available at the time this book was created.

The Ryan White Part A Program is considered “payer of last resort” and may be used only after all other public and
private insurance sources have been exhausted. Documentation required to access Part A services in Broward County,
must verify the following:

HIV Diagnosis
One Document from below
Positive ELISA with confirming Western Blot test results
RNA-PCR or Branched DNA (BDNA) test showing detectable viral load of HIV virus
Broward County Residency
One Document from below. MUST be current/not expired with applicant’s name & current address. Florida driver’s license OR Florida photo ID
Utility bill
Homestead exemption
Lease and/or rental agreement
Rent receipts noting applicant’s name, address and landlord’s name
Letter and contact information (i.e. phone number) from resident providing housing stating that client resides at address
Voter’s registration
Letter of award from Social Security (SSI/SSDI), Food Stamps, TANF, or VA
Statement from a homeless shelter/homeless services provider attesting to homeless status
Applicants with undocumented immigration status, may utilize a statement signed by case manager or eligibility worker and
applicant, stating he/she does not have a valid state ID due to undocumented immigration status and does not possess
documents that could otherwise be used to verify residency
Proof of Income
Must Bring ALL that applies to applicant and other household members (i.e. spouse, partners, applicant’s children
and significant others)
Most recent income tax return IRS 1040 or W-2 forms
Bank statements (savings or checking) from the last 3 months with the bank’s name/logo
If employed, pay stubs from the last 3 months
o If not available, Signed employer statement with dates, position , phone number and salary
Unemployment income
Rental income for the last 3 months
Retirement statements for the last 3 months
A recent TPQY/SEQY printout from Social Security
Letter of award from Social Security (SSI/SSDI), Food Stamps, TANF, or VA
If no income/unemployed, signed support letter from person providing assistance stating how applicant receives food,
clothing, shelter, monthly allowance amount and how long assistance is expected to last
In isolated cases for clients who have an undocumented immigration status, a statement signed by case manager or eligibility
worker and client, stating client does not hold a valid INS work permit (authorization), and staff verified that client is not
receiving any federal, state or county entitlements.

Centralized Intake and Eligibility Determination (CIED) Centralized Intake and Eligibility for Persons Living with HIV/AIDS in Broward County provides a single entry point of
service for new clients as well as assistance with applications and referrals to third party benefits and community services
to all eligible Broward County residents. Services are provided in the CIED main office as well as several provider sites.

Food Bank/Vouchers
The food bank offers quality food to any Broward resident living with HIV/AIDS while respecting clients’ rights to
privacy and dignity. A variety of foods are provided with the necessary nutrients essential to control body weight while
preserving the integrity of lean body mass and satisfying hunger. Once enrolled, the Poverello Center uses client numbers
rather than personal names to maintain confidentiality. A nutritionist is available to clients by appointment.

Legal Assistance

Local AIDS Pharmaceutical Assistance

This service provides a full-range of prescription medications by expert pharmacists who specialize in HIV medications.
Additional services include direct billing to most insurance plans, free home delivery or easy pick-up at pharmacy and
free tools to improve adherence.

Medical Case Management
Eligibility: Proof of income, 400% FPL
Medical Case Management (MCM) provides client centered services that link clients and family members with health
care, psychological and other services to ensure timely, coordinated access to medically appropriate levels of health and
support services. MCM includes the provision of treatment adherence counseling to ensure readiness for, and adherence
to, complex HIV/AIDS treatments.

nurse specialist, or nurse practitioner in an outpatient setting. Services include diagnostic testing, early intervention and

What is AICP? AICP is a statewide program for persons who are diagnosed with AIDS or are HIV positive with
symptoms and who, because of their illness, are unable to maintain their private health insurance coverage. The
program makes direct payments (up to $750/month) to each client's employer or insurance company for the
continuation of medical, dental, mental health and optical coverage.

What is ADAP? The AIDS Drug Assistance Program (ADAP) provides HIV/AIDS related prescription drugs to
uninsured and underinsured individuals living with HIV/AIDS.
