�日本での単純ヘルペス脳炎の発生頻度は，年間 100 万人あたり 3.5〜3.9 人と推計されている．日本での新生児・小児のみを対象とした報告はない．

■ 背景・目的単純ヘルペス脳炎を含めた急性脳炎の発生率について多くの報告があるが，地域（先進国での

検討か発展途上国での検討か）や年齢（全世代を対象とした検討か，小児あるいは成人のみを対象とした検討か），調査された集団の単位（一地域での検討か国単位での検討か），罹患者の定義

（意識障害を伴う発熱患者すべてを対象とした報告か，神経放射線学的所見や脳波，髄液を含めた神経学的検査所見やウイルス学的検査所見（PCR法などによるウイルスの同定あるいは抗体測定の結果）を加味したうえで対象を定義した報告か）などにより罹患者数が大きく変動してしまうという問題がある．以下，急性脳炎の発生頻度と単純ヘルペス脳炎の発生頻度について文献的に検討する．

■ 解説・エビデンスJmorらは 1950 年から 2007 年までに報告された急性脳炎に関する論文から発生頻度について

詳細に検討されている 25 報を抽出し，小児での急性脳炎の発生頻度は少なくとも 10.5/10 万人/年，成人では 2.2/10 万人/年と推計している 1）．また，Granerodらは 1950 年から 2002 年までの急性脳炎の発生頻度を詳細に検討した 45 報から急性脳炎の発生頻度を 0.07〜12.6 人/10 万人/年と推計している 2）．

単純ヘルペス脳炎は，先進国では散発性に起こるウイルス性脳炎のなかで最も頻度が高くかつ急速に重症化することも多い脳炎である．Whitleyらの総説ではどの年齢でもみられるが小児や高齢者にやや多くみられ，年間 250,000〜500,000 人に 1 人の割合で発生すると記載されている 3〜5）．発生に性差や季節性はない 6, 7）．

日本における単純ヘルペス脳炎の発生頻度を検討した既報として，200 床以上の病床を有する病院を対象とし 1989 年から 1991 年に行われた全国疫学調査があり，この検討では単純ヘルペス脳炎の発生が 3.5 人/100 万人/年と推計されている 8）．このほか，鳥取で 2001 年から 2005年に行われた 16 歳以上を対象とした疫学調査では 3.9 人/100 万人/年の単純ヘルペス脳炎の発生が推計されている 9）．日本での新生児・小児のみを対象とした報告はない．

諸外国での検討では，スウェーデンでの 1990 年から 2001 年に行われた全国疫学調査から，単純ヘルペスウイルス 1 型による脳炎の発生が 2.2 人/100 万人/年 10），オーストラリアのニューサウスウェールズ州での 1990 年から 2007 年に行われた疫学調査から，単純ヘルペス脳炎の発

生が 6.7 人/100 万人/年 11）と推計されている．一方，ニュージーランドのオークランドでの 2005年から 2009 年に行われた 15 歳以上を対象とした疫学調査では，単純ヘルペス脳炎の発生が 0.54人/100 万人/年 12）と推計され，英国での 1998 年から 2001 年に行われた小児のみを対象とした検討では，2〜11 ヵ月の児での単純ヘルペス脳炎の発生が 15.6 人/100 万人/年，12〜35 ヵ月の児での発生が 4.3 人/100 万人/年 13）と推計されている．
