Case: ชายไทยอาย22ป• Chief complaint: ปวดหวไหลขวาขยบไมได 2

ชวโมงกอนมารพ.

Present illness• 2 ชวโมงกอนมารพ. ขณะเลนวอลเลบอล ลมใช

มอขวายนพนในทาเหยยดแขนไปดานหลง เจบหวไหลขวา ขยบแลวเจบ ไมมศรษะกระแทกพน ไมบาดเจบบรเวณอน

• ทหองฉกเฉน ผปวยเดนมาในทาพยงแขนงอขอซอกไวขางลำาตว ปวดมาก pain score 8/10

Past history• ปฏเสธโรคประจำาตว• ปฏเสธประวตแพยา แพอาหาร• ดมเหลา social drinking• สบบหรวนละ 5 มวน นาน 5 ป• ปฏเสธโรคตดตอในครอบครว

Physical examination• Vital sign: BT 36.7°C PR 108bpm RR

18/min BP 128/78mmHg• GA: Young Thai male, alert, not pale,

no jaundice• HEENT: not pale conjuctivae,

anicteric sclerae• Heart: pulse full and regular, normal

s1s2 sound, no murmur• Chest&lungs: equal breath sound,

clear• Abdomen: soft, normoactive bowel

sound, not tender

Physical examination• Extremities: Flatten right deltoid,

right arm in slightly abduction and external rotation, Duga’s test positive, Hamilton ruler test positive

• Neuro examination: Axillary nerve – intact sensation and motor

Investigation• Film right shoulder AP, scapular Y

view

Diagnosis• Right Anterior shoulder dislocation

(subcoracoid type)

Management• Sedate ดวย Morphine 4mg v stat, Valium 10mg v

stat• Close reduction ดวย Zero position failed• Traction - countertraction success• Pain control Paracetamol(500) 1 tab PO prn for

pain q 6hr Naproxen(250) 1x2 PO pc

• Film right shoulder(AP, lateral, transcapular) after reduction

• On arm sling and swath• F/U OPD 1 wk

Physical examination (after reduction)

• Joint stability• Neurovascular

Anterior shoulder dislocation

• Epidemiology: shoulder เปน mojor joint ท dislocation บอยสดประมาณ 45% โดยเปน anterior dislocation 84%, recurrence 75%

• Etiology: indirect force เมอแขนอยในทา abduction external rotation หรอ hyperextension

• Types: 90%

Anterior shoulder dislocation

• Sign & symptoms:1. Flatten right deltoid2. Axillary nerve injury3. Anterior fullness at deltopectorial

groove4. Right arm in slightly abduction and

external rotation5. Duga’s test6. Hamilton ruler test

Anterior shoulder dislocation

• Associated injury: 1 .กระดกยบทางดาน posterolateral ของ

humeral head เรยกวา Hill-Sachs lesion พบได 25%

2. กระดกหกบรเวณ anteroinferior glenoid labrum เรยกวา Bony Bankart lesion

3. กระดกหกบรเวณ proximal humerus4. กระดกหกของ greater tuberosity

Treatment• Close reduction1. Zero position ดงในทา abduction 165° flexion 45°

Treatment2. Traction - countertraction

Treatment3. Stimson’s technique: นอนควำา ถวงนำาหนก 10 ปอนด รอจน auto-reduction

Treatment• After reduction– Immobilizationดวย interlocking sling นาน

1-3 สปดาห (ในผสงอายไมเกน 1 สปดาห)– หามทำา abduction and external rotation ใน

ระยะแรก– Pain control– Rehabilitation (Acute, Recovery,

Maintenance)

Rehabilitation• Acute phase: <1 wk

Rehabilitation• Recovery phase: 1-6 wks

Rehabilitation• Maintenance phase: >6 wks
