Reactions 1455 - 8 Jun 2013 S Caspofungin Kounis syndrome: case report A 58-year-old woman with acute myeloid leukaemia experienced Kounis syndrome after receiving a single loading dose of caspofungin 70mg by infusion, to treat pulmonary aspergillosis complicating severe chemotherapy-induced neutropenia. She developed hypotension and bradycardia, resulting in a fatal complete heart block [time to onset not stated]. Author comment: "[A]n immediate hypersensitivity effect inducing transient myocardial ischaemia and Kounis syndrome through histamine-mediated vasospasm or an adenosine-mediated process seems possible."
