Multi-Vessel Disease and AMI:Current Guidelines
Cardiogenic shock in patients less than 75 years old with ST elevation or LBBB or posterior MI who develop shock within 36 hours of STEMI, have severe multivessel or left main disease, and are suitable for revascularization that can be performed within 18 hours of shock
In patients with multi-vessel disease, primary PCI should be directed only at the infarct-related coronary artery (culprit vessel), with decisions about PCI of non-culprit lesions guided by objective evidence of residual ischaemia at later follow-up

Guidelines on myocardial revascularization
Multi-Vessel Disease and ACS NSTE:Current Guidelines
If PCI is desirable it should be recommended to identify the culprit lesion with the help of angiographic determinants and with ECG guidance, and to intervene on this lesion first.
In case of multiple angiographically significant non-culprit stenoses or lesions whose severity is difficult to assess, liberal use of FFR measurement is recommended in order to decide on the treatment strategy

Interventi nei pazienti sopravvissuti ad IMA
identificare precocemente i pazienti ad alto rischio di eventi (reinfarto, scompenso cardiaco, morte improvvisa) implementare interventi per prevenire tali eventi
- STEMI e malattia MV: staged PCI
- NSTEMI e malattia MV: staged o MV PCI
- disfunzione VS, miocardio vitale e coronaria occlusa/stenotica: rivascolarizzazione
- disfunzione VS post-fase acuta: eco seriati
- FEVS < 35%, QRS > 120-150 msec, RS : CRT-D
- FEVS < 30-35% : ICD
