RESUMEN
Introducción: La cicatrización de una herida es un proceso complejo. La
oxigenoterapia hiperbárica se ha propuesto como tratamiento complementario para
favorecer la cicatrización de las heridas y con ello mejorar la cura.
Objetivo: Describir las características que conforman la muestra y evaluar el tipo de
herida sometida a la oxigenoterapia hiperbárica.
Material y Métodos: La producción de una revisión bibliográfica requiere la búsqueda
en diferentes bases de datos bibliográficas. Una vez obtenidos los artículos, se
descartaron artículos duplicados, se realizó la lectura de título y resumen, y se efectuo la
búsqueda del texto completo, por último se descartaron aquellos que no cumplían con
los crterios de selección medidos por la escala metodológica, conocida como escala
JADAD.
Resutados: El resultado predominante determina la procedencia, el sexo y la edad de
los individuos de la muestra, además del tipo de herida que padecen.
Conclusiones: Los participantes de la muestra proceden de diferentes países y
continentes y son principalmente hombres de edad adulta y longeva. El tipo de herida
predominante en la muestra, son úlceras en miembros inferiores, destacando las úlceras
de pie diabético.

ABSTRACT
Introduction: Healing a wound is a complex process. Hyperbaric oxygen therapy has
been proposed as a complementary treatment to promote healing of a wound and thus
improve the cure.
Objective: To describe the features that make up the sample and to evaluate the type of
wound submitted to hyperbaric oxygen therapy.
Material and Methods: The production of a bibliographic review requires searching
different bibliographic databases. Once the articles were obtained, duplicate articles
were discarded, title and summary reading was performed, and full-text
search was carried out, Finally, those that did not comply with the selection criteria
measured by the methodological scale, known as the JADAD scale, were discardeds.
Results: The predominant result determines the provenance, sexand age of the
individuals in the sample, in addition to the type of wound they suffer.
Conclusions: The sample participants come from different countries and continents
and are mainly men in adulthood and long life. The predominant type of wound in the
articles of this study are ulcers in the lower limbs highlighting diabetic foot ulcers.

1. INTRODUCCIÓN
1.1. HERIDAS
Se define herida como la “pérdida de continuidad de la piel o mucosa producida por
algún agente físico o químico, que cursa con una serie de signos y síntomas tales como
separación de bordes de la piel, dolor, inflamación, hemorragia…1”
1.1.1. Tipo de heridas
Las heridas se pueden clasificar desde distintos puntos de vista, a continuación se
muestran los más comunes2.

1.2. LA CICATRIZACIÓN.

La cicatrización es un proceso especial que implica la movilización de fibroblastos,
células endoteliales, células inflamatorias y queratinocitos. Para el desarrollo del
proceso de cicatrización necesitamos distintos señalizadores a nivel molecular y celular
que promueven la proliferación celular, la deposición de colágeno y la renovación de
irrigación sanguínea hacia la herida4.

1.2.1. Tipos de cicatrización.
La cicatrización de las heridas se puede dar de diferentes formas según la unión de
bordes:

1.2.2. Fases de la cicatrización.
Las principales fases de la cicatrización son: la fase hemostática e inflamación, fase de
proliferación y, por último, la fase de maduración aunque hay diversos autores que
describen fases intermedias, las más importantes son las tres que se han mencionado1.

1. Fase hemostática e inflamación1: es la primera respuesta a la lesión. Su función
es mantener la hemostasia por la llegada de plaquetas y la formación del trombo
de fibrina en el sitio de la herida, con la intención de producir la formación del
coágulo y una vasoconstricción refleja. Tras la hemostasia se produce una
vasodilatación en la herida y su entorno, produciendo un aumento del flujo
sanguíneo para que los neutrófilos (función: limpieza de herida), monocitos
(evolucionan a macrófagos, línea celular más importante por la cantidad de
funciones en la cicatrización) y linfocitos lleguen al sitio de la herida.
Ocurre desde el momento en que hay lesión y aparece la herida hasta el tercer o
cuarto día.
2. Fase proliferativa1: se produce la reepitelización de la herida. Se trata de una
fase que dura hasta los 14 días y que consta de tres procesos diferentes:
- La granulación: aparición de nuevos vasos sanguíneos produciendo
angiogénesis, estimulada por los mediadores del macrófago y acción de los
fibroblastos. Al mismo tiempo, se induce la producción de colágeno.
- La epitelización: proceso llevado a cabo principalmente por los queranocitos
encargados de actuar como protector de infecciones y de la pérdida
hidroelectrolítica.
- La contracción: este proceso es realizado por la diferenciación de los
fribroblastos a miofibroblastos. Los miofibroblastos tienen la capacidad de
hacer que se unan los bordes de la herida por su función contráctil.
3. Fase de maduración o remodelación1: consiste en el depósito de colágeno en la
herida. La fuerza tensil final de la herida dependerá de su calidad, cantidad y
buena organización. Esta fase puede llegar a durar hasta los 2 años.

1.2.3. Posibles complicaciones
El proceso de cicatrización puede verse afectado por factores locales y generales que
puede producir posibles complicaciones. Las más habituales son5:

Existen diferentes tratamientos o terapias para favorecer la cicatrización de las heridas.
La oxigenoterapia hiperbárica (OHB) se ha propuesto como tratamiento
complementario para favorecer la cicatrización7.
1.3. OXIGENOTERAPIA HIPERBÁRICA
La oxigenoterapia hiperbárica es un método no invasivo y sin dolor 8 que se basa en
respirar oxígeno puro al 100% en una cámara hiperbárica. En el interior de la cámara, la
presión de aire se eleva hasta tres veces más que la presión de aire normal9. Es decir el
aire que respiramos contiene un 21% de oxígeno a nivel del mar, suficiente para las
necesidades de los tejidos del cuerpo humano y con una presión Atmosférica de
1atmosferas (ATA) (760 mm hg) según European Committe for Hyperbaric Medicine
(ECHM) establece que la oxigenoterapia hiperbárica (OHB) debe administrarse a una
presión mínima de 2 ATA durante un tiempo no inferior a 60 minutos10
. Dependiendo
del tipo de patología y del paciente. Generalmente se realizan de 10 a 40 sesiones,
extendiéndose o reduciéndose el tratamiento en ciertos casos11
.

1.3.1. Tipos de cámara hiperbárica
Una cámara hiperbárica se define como un receptáculo hermético capaz de superar una
presión superior a la normal12
. Las presiones alcanzadas dentro de la cámara durante
una sesión hiperbárica es de 2.2 ATA, y pudiendo alcanzar una presión máxima de
3 ATA13
.
La oxigenoterapia hiperbárica, en adelante OHB, se administra en dos tipos de cámaras
hiperbáricas, las cámaras monoplazas y las multiplazas13, 14, 15
.
Cámaras monoplazas: son cámaras para un solo paciente, siutado en una camilla. Se
consigue una presurización utilizando oxígeno puro al 100%. Ante la gran
concentración de oxígeno existe riesgo de deflagración, es decir, que puede arder sin
eplosión. Este riesgo es producido por alcanzar concentraciones tan elevadas de oxígeno
con un volumen de expansión relativamente alto.
Este tipo de cámaras suele estar construido por material transparente muy resistente
como el metacrilato. Las cámaras más modernas incorporan una televisión en el exterior
y el paciente puede disfrutar de una sesión con entretenimiento15
. El equipo de salud
esta en contacto durante toda la sesión con el paciente mediante un sistema de
comunicación integrado14, 16
.

Cámaras multiplazas: En las cámaras multiplazas pueden recibir el tratamiento varios
pacientes (2 o más) al mismo tiempo. La presurización de la cámara se realiza mediante
compresores (eliminan partículas y olores no deseados) que utilizan el aire del exterior.
Esto provoca que la concentración de oxígeno dentro de la cámara mantenga unos
valores similares a los ambientales, permitiendo que un profesional del equipo de salud,
instruido y entrenado, permanezca dentro de la cámara. Por lo general se trata de
profesionales de enfermería. Los pacientes respiran oxígeno puro con una
concentración próxima a 100% a través de mascarillas nasofaciales herméticas o
cascos integrales 13, 14, 15
.
Hay diferentes tipos de cámara multiplaza, las que están compuestas por un único
compartimento principal o las que poseen dos compartimentos, la antecámara y el
compartimento principal16
.
En el compartimento principal se alojan los pacientes en una posición sentada, y el
número de pacientes depende de la dimensión de la cámara16
.
La antecámara es un espacio con presurización y despresurización independiente al
compartimento principal. Presenta dos compuertas, una comunica con el
compartimento principal y la otra con el exterior, por lo que los profesionales pueden
entrar o salir sin interrumpir la sesión de tratamiento16
.
Si el profesional se encuentra dentro del compartimento principal y necesita salir al
exterior la antecámara se presurizará para alcanzar la misma presión que el
compartimento principal, se abrirá la compuerta y pasará a la antecámara. Una vez en la
antecámara, deberá de despresurizarse para poder salir al exterior. En el caso contrario,
en el que un miembro del equipo tuviera que entrar al compartimento principal, deberá
de realizar el proceso explicado a la inversa.
Algunas cámaras multiplazas también poseen una exclusa, es un apartado ubicado en el
compartimento principal que permite el intercambio de objetos pequeños, utensilios o
fármacos sin que la cámara sufra modificaciones de presión.

Dentro de las cámaras multiplazas es esencial una zona equipada de material de
urgencia y de un sistema contra incendios. Esto debe ser revisado antes de iniciar cada
sesión16
:

El tiempo de despresurización y presurización de las cámaras, tanto la monoplaza y de
la multiplaza, es de vital importancia. Reducir el tiempo de cada proceso puede
provocar posibles complicaciones. El tiempo dependerá de la presión que se quiere
alcanzar. A mayor presión mayor tiempo de presurización y despresurización.
Normalmente la presurización suele alcanzarse en 5 minutos, una vez presurizada
empieza el tiempo isopresión que debe ser entre 60-90 minutos. Y la despresurización
finalizará cuando la presión de la cámara hiperbárica se reduzca a la presión
ambiental14, 15
.

1.3.2. Equipo de salud
El personal encargado de realizar la OHB esta formado por los siguientes profesioanles,
aunque su número variará dependiendo del tamaño de la unidad16
:

1.3.3. Efectos de la oxigenoterapia hiperbárica
La OHB presenta un conjunto de efectos terapéuticos y fisiológicos basados en el
aumento del transporte de oxígeno plasmático fundamentalmente. La OHB nos permite
aplicaciones muy variadas no solo en la actualidad, la utilización de la OHB es
empleada desde hace más de 350 años como medicina hiperbárica. Sin embargo, no ha
sido aplicada bajo criterios científicos hasta mediados del siglo XX. En 1662, el médico
Británico Henshaw mantenía que el aumento de la presión del aire podía aliviar las
lesiones agudas, mientras que las presiones bajas podrían ser beneficiosas en algunas
heridas, por lo que llevó a cabo la elaboración de una Cámara llamada DOMICILIUM,
sin base científica e impulsada por fuelles de órganos y válvulas para controlar el flujo
de aire17
.”

La OHB presenta unos efectos en los cuales distinguimos entre efectos fisiológicos y
terapéuticos.
A) Efectos fisiológicos: dependerán del aumento de la presión ambiental per se y de
la elevación de la presión parcial del oxígeno.
Con el aumento de la presión ambiental per se obtenemos efectos volumétricos.
Con base en la ley de Boyle- Mariotte, la elevación de la presión ambiental
disminuye el volumen de todas las cavidades orgánicas que no están en contacto
con las vías respiratorias (vejiga, tracto digestivo….), en función
proporcionalmente inversa es decir a mayor presión ambiental menor volumen
de las cavidades18
.
De la elevación de la presión parcial del oxígeno obtenemos los efectos
solumétricos, ley de Henry: al respirar oxígeno puro se produce un aumento de
la presión arterial, que puede llegar a superar los 2.000mmHg con una presión
ambiental de 3 atmósferas (ATA). El volumen de oxígeno disuelto y
transportado por el plasma aumenta más de 22 veces14
.
B) Efectos terapéuticos: distinguimos entre dos tipos de efectos, los directos y los
indirectos19
.
Efectos directos19
: en enfermedades que existe hipoxia tisular, general o local,
como factor etiopatogénico, el aumento del transporte y disponibilidad de
oxígeno facilita un posible efecto terapéutico. Al tratarse de oxígeno en forma
física, disuelto en plasma libre de limitaciones reológicas (que tratan
la deformación y el fluir de la materia) o condicionamientos metabólicos que
limitan la transferencia o aprovechamiento del oxígeno eritrocitario, accede por
capilaridad, transferido a favor de gradiente por difusión simple.

Efectos Indirectos19
: Acciones específicas de algunas enfermedades en función
de determinados estados fisiopatológicos. Algunos efectos son: Disminución del
volumen de las burbujas en caso de embolismo gaseoso, efecto Robin – Hood,
angiogénesis, reactivación de la capacidad fagocítica oxígeno- dependiente de
los granulocitos polimorfonucleares, acción bactericida, bloqueo de la formación
de toxinas clostridiales, eliminación rápida de carboxihemoglobina.
1.3.4. Indicaciones
A) Según Undersea & Hyperbaric Medical society (UHMS) la aplicación de la
OHB está indicada en trece condiciones médicas20, 21
:

indicaciones de tipo complementaria o experimental como por ejemplo:
intoxicación por cianuro, anemias graves refractarias a transfusiones, gran
quemado, intoxicación por tetracloruro de carbono, la mucormicosis...

1.3.5. Contraindicaciones
Se dividen en dos grupos: absolutas o relativas13

1.3.6. Complicaciones
Si la OHB es aplicada por un equipo de salud capacitado en la materia, se respetan las
pautas y medidas del tratamiento médico como si fuera un tratamiento farmacológico,
los efectos secundarios pueden presentarse pero son poco frecuentes. Nombrados a
continuación13, 14
:

• Dolor agudo (barondoltagia).
• Barotrauma en el oído interno, medio y externo, en los senos paranasales y en los pulmones.
Por el cambio de presión:
• ED.
• Neumotórax
• Parada Cardiorespiratoria (PCR).
Por mala praxis en la despresurización:
• Ocasiona molestias.
La entrada de aire en el aparato gastrointestinal:
• Intoxicación por O2.
• Irritación cortical desencadenando una crisis epiléptica focal o Jacksoniana, en la que los síntomas pueden ser convulsión muscular, alucinaciones, sudoración, náuseas o taquicardia.
• Tos seca,dolor torácico retro-esternal o edema pulmonar.
Por la presión parcial de oxígeno (PpO2):
• Narcosis producida por acumulación de nitrógeno en la sangre.
Por la frecuencia y duración:
• Miopia progresiva.
Por la prolongación del tratamiento:

1.3.7. Procedimiento.
Tras el reconocimiento médico y ser apto para someterse a la OHB, se informa del
proceso y se pauta un protocolo de actuación. El protocolo servirá de guía al operador
de la cámara hiperbárica.
Antes de dar comienzo a la sesión, el profesional de enfermería tiene gran importancia,
ya que debe revisar que estén todos los elementos necesarios y, antes de que los
pacientes entren a la cámara hiperbárica, debe instruir y educar sobre las medidas
preventivas y de seguridad descritas a continuación16
:
Normas de obligado cumplimiento22
:
- Indispensable utilizar ropa de algodón e higienizada. La fibra sintética
genera electricidad estática, aumentando el riesgo de explosión.
- Evitar la aplicación de cremas, colonias…, con bases alcohólicas.
- Todos los pacientes que ingresen en la cámara deben de haber ingerido
comida antes de la sesión, especialmente los pacientes con Diabetes
Mellitus, ya que la OHB sensibiliza a los receptores de insulina por lo que
aumenta el riesgo de hipoglucemias.
- Prohibido entrar con materiales metálicos. Si algún paciente que por su
gravedad o patología requiera aparataje eléctrico, este aparataje será retirado
y sustituido por uno manual, siempre que sea posible. En pacientes con
prótesis metálicas o marcapasos se precisa especial atención y seguimiento.
Técnicas de relajación.
Técnicas de ecualización de los oídos: una buena ecualización de los oídos como
la maniobra de Vasalva, la de Toynbee o la técnica simple es de vital
importancia para evitar complicaciones23
.
Enseñar la forma de colocar correctamente las mascarillas o campanas según sea
el material utilizado.
Técnica de respiración abdominal o diafragmática durante la administración de
oxígeno al 100% para garantizar al máximo su aporte.

Durante la descompresión la posición corporal del paciente debe de ser erguida
para evitar que se acumulen burbujas de nitrógeno en el interior de las
articulaciones.
Una vez el paciente ha recibido toda la información por parte del profesional para la
compresión y descompresión de la cámara, entra dentro y se cierra la puerta del
compartimento desde el exterior, iniciando así la sesión.
Tanto en la cámara multiplaza como en la monoplaza el paciente siempre estará
controlado por el equipo médico15, 16
.
1.3.8. Adaptaciones y cambios de técnicas en un medio hiperbárico
A continuación se describen los diferentes cambios y adaptaciones de técnicas dentro
de un medio hiperbárico 22, 23, 24, 25
:
El globo de fijación de las sondas debe rellenarse con agua destilada o suero
salino fisiológico, para evitar las variaciones de volumen por el aumento de la
presión.
Las sondas nasogástricas deben sellarse durante toda la sesión para evitar la
entrada de aire al aparato digestivo.
El aparato eléctrico de respiración asistida será sustituido por uno adaptado a
condiciones hiperbáricas.
Para realizar una aspiración de secreciones dentro de la cámara hiperbárica se
utilizara un aspirador manual o mecánico con pedal.
En caso de necesitar un desfibrilador, se procedería a la descompresión de la
cámara para poder sacar al paciente y utilizar un desfibrilador común, ya que un
desfibrilador adaptado al medio hiperbárico es difícil de conseguir por su
elevado precio.
El equipo de sueroterapia debe de ser de plástico, si es un plástico rígido ha de
ser agujerado en la parte de arriba para equilibrar presiones. Además, la ventana
de la cámara de goteo debe de permanecer abierta.

Administración y preparación de fármacos: la vía de administración será la vía
intravenosa, ya que el aumento de presión produce una vasoconstricción
periférica. Los fármacos han de ser precargados fuera de la cámara hiperbárica.
Si se realiza una extracción de muestra dentro de la cámara, es importante
eliminar burbujas y mantener la muestra en posición vertical.
En definitiva, el profesional de enfermería debe de estar preparado y entrenado para
actuar en un medio hiperbárico. La actuación de una emergencia dentro de la cámara
es exclusiva del enfermero/ enfermera hasta que se haya realizado la descompresión
correctamente y se pueda sacar al paciente.

2. OBJETIVOS
2.1. Objetivo general
Describir las características que conforman la muestra y evaluar el tipo de herida
sometida a la oxigenoterapia hiperbárica.
2.2. Objetivos específicos
Valorar la frecuencia en la que se administra la oxigenoterapia hiperbárica.
Estimar el tiempo de isopresión por cada sesión.
Determinar el tipo de cámara hiperbárica utilizada.
Exponer la evolución de la herida tras la utilización de la oxigenoterapia
hiperbárica en la cura de heridas.

3. MATERIAL Y MÉTODOS
3.1. Pregunta de investigación
La pregunta de investigación es formulada mediante el formato PIO26
, constituido por
tres componentes (paciente, intervención y los outcomes o resultados). Esta
configuración permite transformar una pregunta clínica en una pregunta estructurada.

Como se muestra en la tabla se obtiene como resultado la siguiente pregunta de
investigación: ¿Qué efecto tiene la oxigenoterapia hiperbárica en las heridas?
3.2. Planificación de la búsqueda
Para la ejecución de esta revisión bibliográfica, se utilizó un único revisor basado en la
pregunta PIO y en los respectivos objetivos ya presentados.
La combinación de descriptores y términos libres en las diferentes bases de datos
bibliográficas permitió conocer la producción científica existente.
3.3. Diseño de la búsqueda
3.3.1. Producción de una revisión sistemática
Una producción de una revisión sistemática precisa la búsqueda en diferentes bases de
datos bibliográficas. El manual de Cochrane menciona tres base de datos como las más
importantes: Medline, Embase y Central.
Según el autor Bramer WM.27
, las búsquedas deben de realizarse como mínimo en
EMBASE y MEDLINE. Para las búsquedas de Revisiones de Ensayos Clínicos
Aleatorizados (ECA) deben realizarse en Pubmed.

Además Pubmed posee una sensibilidad mayor que Ovid/Medline y una minuciosidad
comparable “Number Needed to Read” (NNR) 28
.
Hacer una búsqueda adicional de Embase y Pubmed para una revisión bibliográfica,
además de la búsqueda principal en Ovid/Medline para una revisión bibliográfica es útil
ya que se realiza rápidamente e incluye estudios únicos actuales que pueden recuperarse
fácilmente 29
.
Pubmed abarca aproximadamente 21 millones referencias procedentes de la base de
datos de Medline, de revistas de ciencias de la salud y libros electrónicos. Por eso
Pubmed no es solo Medline, pero si su mayor componente.
3.3.2. Descriptores y términos libres empleados
Para los descriptores se utilizó la página web DeCS (Descriptor en ciencias de la
salud) 30
. A partir de la búsqueda en la página web conocimos los términos adecuados
para nuestra revisión bibliográfica.
Para obtener los descriptores en inglés se utilizó “Medical Subjects Headings”
(MeSH) 31
en el portal bibliográfico de Pubmed.
A continuación se presentan en la tabla 13 las palabras clave o descriptores utilizados.

3.4. Criterios de inclusión y exclusión
Criterios de inclusión:
Ensayos clínicos aleatorizados, controlados y pragmáticos.
Estudios publicados en español o inglés.
Estudios con 10 años o menos.
Criterios de exclusión:
Animales.
Se excluye el transplante de piel.
3.5. Escalas
La evaluación de la calidad de los estudios científicos es considera como un proceso
esencial en la producción y selección de la literatura científica en la salud 36
.
Se ha utilizado la escala Jadad (Anexo 5) para la evaluación de la calidad metodológica
de Ensayos clínicos aleatorizados.
Está formada por 5 ítems: Asignación aleatoria, doble enmascaramiento, inclusiones y
exclusiones del estudio, método de asignación aleatoria y método de enmascaramiento.
El máximo de puntos son 5, porque cada ítem puntúa 1 punto. Se considera que tienen
buena calidad metodológica, pues, los estudios en los que la puntuación total sea mayor
o igual a 3. Por el contrario, aquellos estudios que presenten menos de 3 en la
puntuación total son considerados de baja calidad metodológica.

3.6. Proceso de selección de la muestra
Tras realizar la búsqueda en las 4 bases de datos (Medline, Pubmed, Embase y Joanna)
se registraron todos los artículos en tablas de Excel clasificados los artículos según base
de datos. (Anexo 6)
El número total de artículos es de 181 repartidos de la siguiente manera: Embase 76
artículos, Medline 42 artículos, Pubmed 62 artículos y Joanna 1 artículo.
De la base de datos de Joanna se eliminó el artículo encontrando por no ser un ensayo
clínico.
De los artículos de Embase se procedió a eliminar aquellos artículos que fueran
conferencias, resúmenes, revisiones y duplicados en la misma base de datos.
Eliminamos 17 conferencias y resúmenes, 5 revisiones y 4 duplicados (Anexo 7). Total
artículos de Embase 50.
Se compararon los artículos de Embase con los de Medline y se eliminaron 21 artículos
de Embase por duplicados en Medline (Anexo 8). Total artículos Embase 29 y total
artículos Medline 42.
A continuación se compararon los artículos de Embase y Medline con Pubmed
eliminando duplicados con Pubmed. Se eliminaron 8 artículos de Embase y 39 artículos
de Medline (Anexo 9).
Finalmente, contábamos con 21 artículos de Embase, 3 artículos de Medline y 62
artículos de Pubmed obteniendo un total de 86 artículos. Se prosiguió con la lectura del
título y resumen de estos para descartar aquellos que no eran útiles para la revisión.
Tras descartar aquellos artículos que no se consideran útiles, eliminando 69 artículos
(Anexo 10), se obtiene un total de 17 artículos distribuidos de la siguientes manera: 4
artículos de Embase, 13 de Pubmed y ninguno de Medline (Anexo 11).
Por lo tanto, de un total de 17 artículos, se realiza la búsqueda del texto completo, tanto
en la Universidad Católica de Valencia como en la biblioteca del Hospital de la Ribera,

para prodecer a su lectura. De los artículos totales, se obtuvieron 14 a texto completo,
mientras que 3 de ellos, 2 procedentes de Pubmed y 1 de Embase, fue imposible de
conseguirlos (Anexo 12).
Los 14 artículos se evaluaron mediante la escala Jadad y se descartaron 9, 3 de Embase
y 6 de Pubmed, por no superar o no presentar los 3 puntos mínimos en los ítems
(Anexo 13).
Aquellos artículos con una puntuación mayor o igual a 3 formaron la muestra (n),
siendo un total de 5 artículos, extraídos, todos ellos, de Pubmed. (Anexo 14).

3.7. Factor de Impacto y cuartil de las revistas
“Journal Citation Report” (JCR) es el indicador de calidad más conocido y valorado por
los organismos de evaluación de la actividad investigadora. Mide el impacto de una
revista en función de las citas recibidas por los artículos publicados y recogidos en la
“Web of Science” (WOS) 37
.
Limitaciones del Factor de impacto en JCR 37
:
- No todos los artículos de una revisión reciben el mismo número de citas. No es
correcto asignar a todos el mismo factor de impacto.
- No distingue entre las citas; artículos de investigación, cartas, editoriales…
- La comparación de factores de impacto entre diferentes categorías temáticas no
es válida.
- Algunos editores animan a los autores a citar artículos de la misma revista en la
cual publican, o bien los autores se autocitan.
- Hay citas negativas y estas computan igual que los positivas.
- El factor de impacto no es un indicados infalibre de calidad. Algunos artículos
pueden recibir citas años después de haber sido publicados, hecho frecuente en
algunas disciplinas.
Las revistas que forman la n se han analizado a través de JCR.
Nivel de evidencia
Los niveles de evidencia vienen determinados por la fortaleza del diseño de la
investigación, y así los estudios experimentales son lo que tienen un mayor nivel de
evidencia, y los descriptivos el menor. Los niveles de evidencia se suelen medir en
números romanos (I, II, III).
El grado de recomendación sin embargo tiene que ver con su relevancia y con el
balance riesgo/beneficio que de esta recomendación se derivara. Los grados de
recomendación se suelen identificar con letras mayúsculas (A, B, C) 38
.
Las escalas propuestas en 2014 por JBI por su estrecha vinculación con la enfermería,
son las que hemos utilizado (Anexo 15).

A continuación se procede a una revisión de la muestra obteniendo los siguientes datos
demográficos.
4.2.1. País

De los 5 artículos pertenecientes a la muestra dos de ellos realizan el ensayo
clínico aletorizado y doble ciego en el mismo País; Suecia. Estos artículos son
los artículos II y IV.
Por otra parte los artículos I, III y V el estudio es realizado en: Canadá, Autralia
e Italia, respectivamente.

Respecto al tipo de heridas, todos los artículos que consituyen la muestra
coinciden en aplicar la oxigenoterapia hiperbárica en heridas crónicas.

oxigenoterapia, el I y III. De los 3 artículos restantes que son el II, IV y V se
aplican 40 sesiones de oxigenoterapia.

Las revistas de la n se han analizado a traves de JCR, obteniendo los siguientes
resultados. Al extraer los datos de Factor de Impacto, cuartil y categoría de los
artículos de la muestra, no podemos analizar muchos datos, ya que, al ser años
diferentes y categorías diferentes no se pueden analizar conjuntamente.
Solo podemos concluir, que en la categoría de las revistas, destaca la de "Endocrinology
& Metabolism" en un 60 % (Es decir 3 de las 5 revistas pertenecen a la categoria de
"Endocrinology & Metabolism". Y que las revistas de Diabetes tambien ocupan un 60%
del total de la muestra siendo esta la revista de publicación de los artículos I, II y IV.
Todas las revistas de publicación de todos los artículos son revistas médicas.

5. DISCUSIÓN
La oxigenoterapia hiperbárica, como se ha comentado, es un tratamiento utilizado desde
la antigüedad en diferentes aplicaciones.
En la bibliografía consultada se puede observar que son muchas las heridas susceptibles
de aplicación de OHB para llevar a cabo los cuidados y tratamientos de las mismas.
Aunque la revisión bibliográfica realizada según los criterios de inclusión y las
puntuaciones óptimas tras evaluarlos por medio de la escala Jadad, nos muestran
resultados que coinciden con el uso de la oxigenoterapia hiperbárica en heridas de tipo
úlceras en miembros inferiores. Fedorko L, et al. I y Löndahl M, et al.
II, IV, aplican la
OHB en úlceras de pie diabético; Thistlethwaite KR, et al. III
en úlceras venosas y por
último Nasole E, et al. V introduce pacientes con diferentes tipos de úlceras.
Fedorko L et al. I y Löndahl M, et al.
II, IV, recogen una muestra en sus ensayos de
pacientes diabéticos, con lesión en los pies. Con una puntuación en la escala Wagner de
2-4 puntos y con una duración de la no curación de la herida de, al menos 3 meses
previos a la inserción del estudio. Excepto Fedorko L, et al. I que incorporó a su estudio
pacientes con heridas de al menos 4 semanas previas a la inclusión, al igual que
Thistlethwaite KR, et al. III
.
Thistlethwaite KR, et al. III
, abarca pacientes con signos de hipoxia en la herida. La
hipoxia tisular local con lesión repetitiva por isquemia- reperfusión conduce a una
inflamación prolongada, y la interacción entre esto y la infección son las causas
dominantes de una herida crónica39
.
Por último, Nasole E, et al. V, como los pacientes de su estudio presentan diferentes
tipos de úlcera, utiliza la oximetría transcutanea peri-lesional como método de
introducción para la OHB.
Otros estudios revisados describen el uso de la OHB en distintas heridas como:
Yildiz H, et al.40
, presenta la OHB como terapia de tratamiento de la hidrandenitis
supurativa (HS); Ersoz F, et al.41
, es un ensayo clínico de los efectos de la OHB como

tratamiento de la intervención quirúrgica de un quiste pilonidal; y un estudio piloto
realizado por Chong J, et al.42
, que aborda el efecto de la OHB como tratamiento de
quemaduras. Por último, un protocolo de estudio realizado por Millar IL, et al. 43
,
emplea la OHB ante una fractura abierta.
La OHB se suministra entre 10 y 40 sesiones, aunque depende del paciente, del tipo de
patología a tratar y de si presenta alguna contraindicación para seguir con el tratamiento.
Todos los autores de la muestra no sobrepasan las 40 sesiones, coinciden con la
duración que se contempla en: Löndahl M, et al. II, IV
y Emanuele Nasole, et al. V,
siendo la frecuencia de tratamiento de 5 días semanales durante 8 semanas para el
tratamiento de úlceras de pie diabético II, IV
y diferentes tipos de úlceras V.
Por otra parte para el mismo tipo de úlcera de pie diabético, Fedorko L, et al. I, utiliza
una frecuencia de tratamiento de 5 sesiones semanales durante 6 semanas, resultando un
total de 30 sesiones, como señala Thistlethwaite KR, et al. III
Para el tratamiento de otras patologías revisadas en la búsqueda bibliográfica los
diferentes autores adjudican una cantidad de sesiones diferentes en cada uno de los
artículos.
Yildiz H, et al.40
, atribuye una frecuencia de tratamiento de 5 sesiones semanales
durante 4 semanas sumando un total de 20 sesiones. Ersoz F, et al.41
, realiza 18
sesiones repartidas en 6 sesiones semanales durante 3 semanas.
Y, según el protocolo del ensayo clínico de Millar IL, et al. 43
, fija un total de 12
sesiones, iniciando el tratamiento 24 horas tras la cirugía, con una frecuencia de 2 veces
diarias durante 6 días consecutivos.
De la bibliografía revisada Chong J, et al. 42
, solo emplea 2 sesiones durante las 22
horas posteriores al ingreso, por lo que no llega a las 10 sesiones mínimas de
tratamiento.
Según European Committe for Hyperbaric Medicine (ECHM) establece que la OHB
debe administrarse a una presión mínima de 2 ATA durante un tiempo no inferior a 60
minutos.

El tiempo de tratamiento por sesión también varía dependiendo del tipo de patología a
tratar.
El periodo de tratamiento esta formado por tres tiempos: compresión, isopresión
(tiempo que el paciente recibe oxígeno al 100% con la presión correcta e indicada por el
equipo especialista) y descompresión.
La totalidad de los artículos que han constituído la muestra del presente trabajo
cumplen con la administración de la OHB como indica el ECHM.
El tiempo en los períodos de compresión y descompresión de la cámara hiperbárica
varían en todos los estudios, los que forman la n y los revisados. En referencia al tiempo
de isopresión, los artículos que se contemplan para la realización de este trabajo
coindicen en lo siguiente: Löndahl M, et al. II, IV
, utiliza en sus dos estudios un tiempo
de 85 min a 2.5 ATA. Un tiempo inferior a 85 min es aplicado en el artículo de
Emanuele Nasole et al. V a una misma presión y Thistlethwaite KR, et al.
III y por otro
lado Yildiz H, et al. 40
a una presión de 2.4 ATA.
Siguiendo con los tiempos de isopresión Fedorko L, et al. I, suministra la OHB con
exposición de 90 min a 244 kPa equivalente a 2.4 ATA.
Se corresponden con Fedorko L, et al. I, en cuanto al tiempo de isopresión y la presión
de la cámara hiperbárica: Ersoz F, et al. 41
, Chong J, et al. 42
y Millar IL, et al. 43
Como se ha visto anteriormente, el tratamiento de la OHB se administra dentro de una
cámara hiperbárica, que puede ser de dos tipos: las cámaras hiperbáricas monoplaza o
cámaras hiperbáricas multiplaza.
La cámara monoplaza solo tiene capacidad para un paciente, el equipo de salud controla
al paciente en todo momento desde el exterior de la cámara.
La cámara multiplaza tiene capacidad para varias personas y el equipo de salud,
normalmente, suele ser el personal de enfermería, que está presente dentro de la
cámara. El oxígeno al 100% puede ser administrado por diferentes formas: mascarillas
nosofaciales, cascos hérmeticos…, por lo que el tratamiento puede realizarse de manera
individualizada según las necesidades del paciente.

Según la muestra especifican explícitamente qué tipo de cámara es utilizada los
siguientes autores: Fedorko L, et al. I utiliza una cámara monoplaza y Löndahl M, et
al. II
utiliza una cámara multiplaza.
Por otra parte, autores como Thistlethwaite KR, et al. III
y Löndahl M, et al. IV
describen el tipo de cámara utilizada de la siguiente manera. El artículo III nombra un
tipo de cámara donde hay espacio para el paciente y la enfermera, mientras que en el
artículo IV se describe que todos los pacientes pueden ser tratados al mismo tiempo, ya
que se administra el oxígeno a través de mascarillas conectadas a tubos diferentes. Esto
nos lleva a deducir que se tratan de cámaras hiperbáricas multiplaza.
La OHB presenta diferentes beneficios utilizados en la cura de heridas. Estos son los
siguientes: reduce significativamente el edema, modera los procesos inflamatorios y
aumenta los sistemas antioxidantes endógenos. La OHB activa la actividad
antibacteriana y farmacológica a través de los neutrófilos, acelara la angiogénesis y la
actividad de los fibroblastos para la recuperación de la herida 44
.
Estos beneficios están relacionados con la respuesta positiva de la OHB ante las heridas
que forman la revisión bibliográfica. Cuatro de los cinco artículos de la muestra
coindicen en que hay buena evolución de la herida tras la aplicación, provocando una
mejora en la cicatrización.
La OHB usada en úlceras de pie diabético presenta un avance de curación en los
artículos II y IV. Löndahl M, et al. II,
IV
, u otros artículos como el de Abidia A, et al.
45, solo muestran efectos beneficiosos en la curación, pero no en la reducción de la tasa
de amputación. La amputación es uno de los mayores riesgo de pacientes diabéticos con
úlcera de pie diabético, por lo que Fedorko L, et al. I, llega a la conclusión que la OHB
no ofrece una ventaja adicional para el cuidado integral de heridas al reducir la
indicación de amputación. La conclusión a la que llega Fedorko L et al. I, coincide con
la de otra de los autores no pertenecientes a la muestra, Santema katrien T.B, et al. 46
.
Opuestamente, el artículo de Chen-Yu Chen, et al. 47
, señala que la terapia de OHB si
que influyó en la reducción de el riesgo de amputación de la extremidad afectada.
Fedorko L, et al. I hipotetiza en su estudio una reducción de la amputación del 28 % de
diferencia entre el grupo OHB y el grupo simulado. Sin embargo los resultados

indicaron 3 puntos porcentuales, por lo que no se puede establecer una diferencia
significativa entres ambos grupos.
Por consiguiente, en el estudio de Löndahl M, et al. IV
, se respalda la idea de que el
tratamiento con OHB mejora la cicatrización de las úlceras de pie diabético con
individuos diabéticos. Las heridas tratadas con OHB al año de seguimiento duplicaron
el número de curación comparadas con úlceras tratadas únicamente con el tratamiento
convencional. Además de la mejoría de las úlceras de pie diabético en el artículo II
Löndahl M, et al. II, concluye que una de las ventajas experimentadas en la OHB fue el
desarrollo positivo de las relaciones sociales con otros pacientes durante las sesiones de
tratamiento. Esta interacción proporciona, a largo plazo, una mayor autoconfianza y una
depresión reducida atribuible, posiblemente, a una mayor cicatrización de la úlcera.
Thistlethwaite KR, et al. III
, respalda el mismo concepto que Löndahl M, et al. IV
,
afirmando que pacientes con heridas de difícil curación son reconducidas a una
trayectoria de curación. La OHB aumenta el oxígeno en sangre y la entrega de este al
lecho de la herida, iniciando eventos celulares y bioquímicos que apoyan la curación.
Estos sucesos conducen a una elevación de los factores de crecimiento y fibronectina
para fomentar la prolifieración, migración celular y la producción de moléculas de
matriz extracelular 48, 49
.
La exposición al oxígeno a una presión superior a la atmosférica conduce a una mayor
formación de especies reactivas de oxígeno. Esto daña las estructuras celulares como los
lípidos, proteínas y los ácidos nucleicos. Emanuele Nasole, et al. V, mediante el uso de
ácido lipoico (LA) o el ácido enantiomerómer R-(b)- lipoico (RLA) que es la forma
biosintetizada en humanos, pretende reducir el daño oxidativo del oxígeno porque este
estrés oxidativo puede interaccionar con las citocinas en el proceso de curación.
Llegando a la conclusión de que la OHB suplementado con LA o RLA tuvo un mejor
efecto curativo que solo OHB. Aumentó la producción de beta EGF (factor de
crecimiento, estimulador del crecimiento celular) y VEGF (factor de crecimiento,
estimula la formación de nuevos vasos sanguíneos) en el tejido de la herida y una
disminución de los niveles de TNF-a e IL-6 en el plasma. En estudios anteriores

coincidiendo con Emanuele Nasole, et al. V, Alleva R, et al
.50, 51, también demostró un
efecto sinérgico beneficioso de OHB con suplementación de LA.
Estudios como el de Yildiz H, et al.40
, de acuerdo con los resultados obtenidos, muestra
que la OHB es eficaz frente a la HS. Presenta el primer ensayo clínico controlado
formal de OHB como tratamiento complementario de la rifampicina y clindamicina,
antibióticos convencionales para el tratamiento de HS de moderada a severa. Con unos
resultados favorables. La OHB provoca que el oxígeno a presiones aumentadas
aumente la presión parcial, facilitando el aumento bactericida al proporcionar sustrato
para la formación de radicales libres de oxígeno y aumentar el estallido respiratorio,
además, la hiperoxia provoca una angiogénesis permitiendo que el antibiótico penetre
con mayor eficacia.
El edema en heridas traumáticas, como fracturas abiertas o tras una intervención
quirúrgica son consecuencias no deseadas por lo que la OHB tiene efecto antiedema,
además de todos los efectos mencionados en los diferentes estudios.
En definitiva todos los estudios están de acuerdo en cuanto a las funciones de la OHB
y, por lo tanto, coinciden en una respuesta positiva de la herida. Se exceptúa, no
obstante, el uso de la OHB ante una reducción de la amputación mayor en pacientes
diabéticos, descrtio en el artículo de Fedorko L, et al.I, ya que no encuentra diferencias
significativas.

6. CONCLUSIÓN
1. Los participantes de los estudios que conforman la muestra, son
mayoritariamente hombres de edad adulta y vejez, procedentes de países de
diferentes continentes que presentan heridas crónicas sobre todo de pie
diabético, tratadas con OHB.
2. La aplicación de la terapia se lleva a cabo alrededor de un par de meses de
tratatamiento hasta completar los ciclos de sesiones panificadas en diferentes
días de la semana.
3. Se emplea alrededor de un par de horas, para llevar a cabo la sesión, desde la
preparación del paciente y de la cámara hasta su finalización.
4. Las cámaras multiplaza son las que predominan en la terapia aplicada.
5. La mayoría de los estudios demostraron una buena respuesta en la cicatrización
de las heridas en mayor o menor medida.
