predicción/resistencia a trastuzumab
• Obstacles preventing trastuzumab binding to
extracellular HER2 domain: MUC4 (epitope
masking), p95 HER2 (epitope loss)
• Upregulation of HER2 down-stream signaling
pathways: PI3K/AKT, PTEN loss
• Signaling through alternate pathways: other
TK receptors: IGFR1, EGFR, HER3, MET, ER
signaling
• Cell cycle related:
• Downregulation of p27
• Cyclin overexpression
• Loss of inmunogenicity?

Frequency of HER2 mutations in breast cancer
• Frequency of HER2 mutations: 1.7% in primary tumor(1), 2.5% in recurrent or MBC(2) and 11.6% in MBC(3)
1TCGA Nature 2012; 2Ross JS Cancer 2016; 3Fang Y Tumor Biol 2014; 4Bose R Cancer Discovery 2012; 5Park YE Oncotarget 2015
• Frequency of HER2 mut in HER2 amplified BC seems to be higher
- 5.5% in recurrent or metastatic BC(2), and from 16.7% (5) to 34,8%(3) in MBC refractory or relapsed after anti-HER2 therapies
HER2 mutations are “driver and actionable”
• Using in vitro kinase assays, the majority of HER2 somatic mutations in breast cancer patients are activating mutations:– “Driver aberrations” (confer growth advantage to the
cells and induce aggressive phenotypes)– “Oncogene addiction” behavior – “Actionable”
Bose R Cancer Discovery 2012
HER2 mutations are sensitive to neratinib
• HER2 mut cells are usually “resistant” to trastuzumab • Some mutations are resistant to lapatinib, but are sensitive to
neratinib
Bose R Cancer Discovery 2012
