elective hepatic resection. Br J Surg 2007;94:297-303.BACKGROUND: The value of routine nasogastric tube (NGT) decompression after elective hepatic resection has not been
investigated. METHODS: Of 200 patients who had elective hepatic resection, including 68 who had previously had colorectalsurgery, 100 were randomized to NGT decompression, where the NGT was left in place after surgery until the passage of flatus
or stool, and 100 to no decompression, where the NGT was removed at the end of the operation. RESULTS: There was nodifference between patients who had NGT decompression and those who did not in terms of overall surgical complications (15.0versus 19.0 per cent respectively; P = 0.451) medical morbidity (61.0 versus 55.0 per cent; P = 0.391), in-hospital mortality (3.0versus 2.0 per cent; P = 0.640), duration of ileus (mean(s.d.) 4.3(1.5) versus 4.5(1.7) days; P = 0.400) or length of hospital stay
(14.2(8.5) versus 15.8(10.8) days; P = 0.220). Twelve (12%) patients randomized to no NGTdecompression required reinsertion of the tube 3.9 (1.9) days after surgery. Previous abdominalsurgery had no influence on the need for NGT reinsertion. Severe discomfort was recorded in 21 patients in the NGT group and
premature removal of the tube was required in 19. Pneumonia (13.0 versus 5.0 per cent; P = 0.047) andatelectasis (81 versus 67 per cent; P = 0.043) were significantly more common in the NGT
group.CONCLUSION: Routine NGT decompression after elective hepatectomy had no advantages. Its use was associated with an
increased risk of pulmonary complications.

Introduction. La résection hépatique représente le traitement de référence des métastases hépatiques des cancerscolorectaux (MHCR). L’hépatectomie droite (HD) dans cette indication est l’intervention la plus fréquemment réalisée.
Patients et méthodes. De 1997 à 2000, 57 patients d’âge moyen 63 ± 10 ans ont eu une HD (sans extension au segment 4ni geste controlatéral) pour MHCR sur foie sain. Dix-neuf patients (16%) avaient reçu une chimiothérapie néoadjuvante, 6 (11%)avaient eu une embolisation portale (EP). Le diamètre de la tumeur la plus grosse était de 6±3 cm, le nombre de lésions était de
2,5±1,3. Cinquante-cinq (96%) patients avaient plus de une lésion, 29 (51%) plus de 3 lésions.
Résultats.- La durée opératoire était de 313±96 min,- Clampage 90% (41 minutes en moyenne),
- 70% des patients n’ont pas été transfusés [nbre moyen 0,5±1 (0-4) ; 6 patients (6/40, 15%) > ont reçu 3 CG]- La mortalité était nulle,
- 58% des patients ont eu au moins une complication post-opératoire,- 3,5% réopérations pour hémorragie sur la tranche de section,
- La durée d’hospitalisation était de 12±6 jours.- L’age, le score ASA, l’EP, l’index de masse corporelle, la chimiothérapie n’influençaient pas la morbidité.
Conclusion. L’hépatectomie droite est une intervention fiable.

OBJECTIVE: To standardize the definition of postoperative liver failure (PLF) for prediction of early mortality after hepatectomy. SUMMARYBACKGROUND DATA: The definition of PLF is not standardized, making the comparison of innovations in surgical techniques and the timely use ofspecific therapeutic interventions complex.METHODS: Between 1998 and 2002, 775 elective liver resections, including 69% for malignancies and60% major resections, were included in a prospective database. The nontumorous liver was abnormal in43% with steatosis >30% in 14%, noncirrhotic fibrosis in 43%, and cirrhosis in 12%. The impact ofprothrombin time (PT) <50% and serum bilirubin (SB) >50 micromol/L at postoperative days 1, 3,5, and 7 on postoperative MORTALITY was analyzed.RESULTS: The lowest PT level was observed on postoperative day (POD) 1, while the peak of SB was observed on POD 3. These 2 variablestended to return to preoperative values by POD 5. The median interval between hepatectomy and postoperative death was 15 days (range, 5-39days). Postoperative mortality significantly increased in patients with PT <50% and SB >50 microml/L. The conjunction of PT <50% and SB >50micromol/L on POD 5 was a strong predictive factor of mortality. In patients with significant morbidity, this "50-50 criteria" was met 3 to 8 daysbefore clinical evidence of complications.CONCLUSIONS: The association of PT <50% and SB >50 microml/L on POD 5 (the 50-50 criteria) was a simple, early, and accurate predictor ofmore than 50% mortality rate after hepatectomy. This criteria could be identified early enough, before clinical evidence of complications, for specificinterventions to be applied in due time.
