Cuatro genotipos diferentes que determinan un solo serotipo.
Transmisión fecal- oral
Factores de Riesgo
Zonas de pobreza (contaminación de agua y alimentos)
Niños en centros de cuidados (contacto cercano con infectados)
Hombres homosexuales.

Hepatitis A: Generalidades
40 – 60 % de los casos de hepatitis aguda en USA.
Daño hepático es secundario a la repuesta inmune del huésped
El virus causa infección citopática al replicarse en el citoplasma del hepatocito.
La destrucción celular es mediada por linfocitos CD8+ y células Natural Killer.
Una respuesta inmune excesiva (observable clinicamente por marcada disminución de RNA VHA durante infección aguda) se asocia a hepatitis más severa.

Hepatitis A: Presentación Clínica
Periodo de incubación: 2 a 6 semanas.
Pródromo de inicio insidioso: Fatiga, dolor abdominal inespecífico o en hipocondrio derecho, anorexia, baja de peso, náuseas intermitentes y vómitos.
Los pródromos clásicamente desaparecen al instalarse la ictericia, 1 a 2 semanas después del inicio de síntomas.
Fase ictérica: Coluria, acolia, prurito, ictericia (70%), hepatomegalia (80%).
Signos poco comunes: esplenomegalia, adenopatías cervicales, rush evanescente, artritis, vasculitis leucocitoclástica.

Hepatitis A: Pronóstico y tratamiento
Autolimitada en la mayoría de las veces. 20% de los casos pueden requerir hospitalización. El tratamiento es sólo de soporte.
El mayor determinante de severidad es la edad
Puede cursar con fase colestásica prolongada hasta por 6 meses, o de forma bifásica.
Puede tener curso fulminante en menos del 1% de los casos, incluso requiriendo transplante, especiamente en comorbilidad hepática (VHC)

Hepatitis A: Diagnóstico Serológico
Marcadores Virales
IgM Anti – VHA
Rápidamente positivo, está presente durante la enfermedad .
Persiste positivo por 2 a 6 meses.
Persiste en promedio 3 meses positivo.
IgG Anti – VHA
Persiste positivo décadas después.
Marcador de Inmunidad.

Hepatitis A: Profilaxis post-exposición
Considerar que los pacientes son contagiosos desde el periodo de incubación hasta 1 semana después de iniciada la ictericia.
Debe ser administrada hasta 2 semanas post – exposición a:
Contactos domiciliarios.
Cuidadores diurnos.
Parejas.
Dosis única de Vacuna Hepatitis A.
Inmunoglobulina (0.002 mL/Kg)

Hepatitis E: Generalidades
• Similar al VHA: transmitido por vía entérica, causa hepatitis aguda y no causa hepatitis crónica.
• Alta incidencia en Asia, Africa, y America Central.
• Incubación entre 2 semanas y 2 meses, con colestasia prolongada.
• La mortalidad general es muy baja, y la incidencia de falla hepática fulminante es rara; sin embargo, si una embarazada se infecta en el tercer trimestre de gestación, su mortalidad puede ser hasta 25%.

Hepatitis E: Tratamiento y Prevención
• El diagnóstico se hace mediante la detección de IgM anti VHE. Éstos exámenes no se encuentran disponibles de forma comercial, y deben ser enviados a laboratorios de referencia nacional.
• No existe tratamiento específico, ni vacuna o profilaxis post-exposición.
• La prevención consiste en medidas de higiene generales, y evitar el consumo de sustancias potencialmente contaminadas (agua, comidas poco cocinadas) en áreas endémicas.

Virus Hepatitis B• Cerca de 350 milliones personas en el mundo cursan con la
infección por el virus Hepatitis B (VHB)
• Puede causar tanto hepatitis aguda como hepatitis crónica.
• Altamente transmitible por vía sanguínea (jeringas), mucocutánea (relaciones sexuales) o perinatal.
• Cerca del 95% de los adultos se recuperan y desarrollan inmunidad duradera de por vida.
• 5% cursa con infección crónica, con persistencia del antígeno de superficie (HBsAg) por más de 6 meses.

Hepatitis B: Cuadro Clínico
• Los síntomas, cuando están presentes, son más severos que los de la Hepatitis A. La ictericia rara vez dura más de 4 semanas.
• Algunos pacientes pueden tener síntomas pre-ictéricos relacionados a la presencia de complejos inmunes: artralgias, urticaria, poliarteritis, glomerulonefritis.
• Síntomas extra-digestivos: poliarteritis nodosa y glomerulopatías (principalmente glomerulonefritis membranosa), ambas mediadas por complejos inmunes circulantes.

Hepatitis B: Tratamiento
• Infección aguda: tratamiento de soporte.
• Hepatitis crónica: sólo tratar si se tiene signos de actividad (transaminasas anormales, biopsia con signos de inflamación). Pacientes inmuno-tolerantes ó portadores inactivos NO se benefician del tratamiento.
• Objetivos del tratamiento: negativizar el HBsAg (rara vez se logra), negatividad el HBeAg y suprimir al máximo la replicación viral.

Virus RNA defectuoso.
Afecta al 5% de los pacientes portadores de Hepatitis B, y requiere el HBsAg para replicarse.
Endémica de región Mediterránea, sur de Italia.
Vías de contagio parenteral.
Existen 3 genotipos, con diferencias en la evolución de la enfermedad.

Genotipo I Países Europeos.
Mayor riesgo de hepatitis fulminante. Avance acelerado a cirrosis y Hepatocarcinoma
Genotipo II Sudeste asiático y lejano oriente.
Aparentemente de curso más benigno, no alteraría el curso natural de la hepatitis B.
Genotipo III América del Sur (cuenca amazónica de Venezuela,
Colombia, Brasil y Perú) e India. Brotes epidémicos con alta tasa de hepatitis fulminante.

Hepatitis D: Formas de Infección
Coinfección
Indistinguible de una hepatitis B clásica, aunque frecuentemente es más grave y puede tener un curso bifásico.
La tasa de cronicidad de la hepatitis B no es afectada por la presencia de coinfección con virus D.
Superinfección
Se manifiesta frecuentemente como una hepatitis grave y en casi todos los pacientes se hace crónica.

Hepatitis D
Inhibe la replicación del virus Hepatitis B, por lo cual se imposibilita su tratameinto con análogos de nucleósidos.
Requiere de tratamiento con Interferón-2a por largos periodos de tiempo.

Genero Hepacivirus.
• 50nm, forma esferica y posee envoltura.
• Existen al menos 6 genotipos principales y varios subtipos.
• Cuasiespecies: generalmente, del plasma e higado de una persona infectada se pueden obtener multiples variantes del VHC, debido a la alta velocidad de recambio de viriones y a la ausencia de correccion de errores de la ARN polimerasa.

Habitualmente se presentan con fatiga, discomfort en hemiabdomen superior y artralgia.
Presentan manifestaciones extra-hepáticas hasta en un 38% de los casos: crioglobulinemia mixta, glomerulonefritis proliferativa, porfiria cutánea tarda, y linfoma no-Hodgkin.

Hepatitis C: Diagnóstico Serológico
Pese a que la detección de anticuerpos anti VHC es un método confiable y de bajo costo, el gold standard es la determinación de RNA VHC en suero.
La biopsia hepática no es necesaria para el diagnóstico.

Hepatitis C: Tratamiento
Debido a que la mortalidad se asocia principalmente a cirrosis, debiese tratarse a pacientes de alto riesgo de desarrollarla:
Infección de larga duración
Alcoholismo
Enfermedad activa en la biopsia.
Manifestaciones extrahepáticas.
En pacientes en los que se espera respuesta favorable, como los Genotipos II ó III, se puede considerar tratamiento sin biopsia.

Hepatitis C: Tratamiento
Pacientes que no son candidatos a tratamiento.
Control clínico y de pruebas hepáticas anual.
Biopsia hepática cada 3 – 5 años en caso de etapa inicial.
En pacientes con cirrosis se recomienda vigilancia ultrasonográfica cada 6 a 12 meses.
Transplante hepático en caso de infección descompensada o hepatocarcinoma, pese a riesgo de viremia recurrente. En un 15 - 30% recidiva la cirrosis.

Hepatopatía por Alcohol: Introducción
Tanto la dosis consumida, duración, patrón de ingesta y tipo de alcohol influyen en el riesgo de desarrollar daño hepático.
Otros factores de riesgo incluyen sexo femenino, latinos, obesidad, estados de sobrecarga de hierro, hepatitis viral crónica y factores genéticos.
COMPROMISO HEPÁTICO AGUDO:
Esteatosis: habitualmente es asintomática y se resuelve después de 4 a 6 semanas con abstinencia alcohólica.
Hepatitis alcohólica: se produce en algunos pacientes con daño hepático crónico y tiene un mal pronóstico a corto plazo.
COMPROMISO HEPÁTICO CRÓNICO
Consumos de alcohol mayores a 40 g/día tienen un riesgo de progresar a cirrosis en un 30%.

Hepatitis Alcohólica: Diagnóstico
Habitualmente se diagnostica ante los hallazgos de compromiso hepático asociado a la historia de consumo de alcohol.
La transaminasa GOT/AST habitualmente está elevada 2 a 6 veces por sobre el valor máximo normal, con una proporción AST/ALT mayor a 2-3.
Niveles muy elevados de GOT/AST (sobre 500 U/L) o GPT/ALT (sobre 200 U/L) son poco comunes y debieran hacer buscar un diagnóstico alternativo.
Una biopsia hepática puede confirmar la hepatitis alcohólica y descartar otras etiologías.

Hepatitis Alcohólica: Pronóstico
Score de función discriminante de Maddrey (MDF)
4.6 x (Protrombina [s] – Protrombina Control [s]) + Bilirrubina (mg/dL)
Pacientes con un MDF de 32 puntos o más indica una hepatitis severa, con riesgo de mortalidad cercano a 50%.
En éstos pacientes se plantea el uso de corticoides (prednisona 40-60mg al día) ó pentoxifilina (400mg VO cada 8 horas).

Hepatitis por Tóxicos
• Incidencia global de entre 13.9 y 24 casos por 100,000 habitantes.
• La toxicidad inducida por drogas es la causa más común de falla hepática fulminante que requiera transplante en USA, 70% de los casos son por fármacos de prescripción médica, y un 9% se atribuyen a suplementos herbales o dietarios.
• La falla hepática puede ser dosis-dependiente (por ejemplo, paracetamol) o idiosincráticas (antibióticos, fenitoina, ácido valproico, inmunomoduladores, hipolipemiantes).

Intoxicación por Paracetamol
• La dosis habitualmente descrita como tóxica en adultos es 7 g/día, pero en pacientes con factores de riesgo, dosis terapéuticas pueden desencadenar falla hepática fulminante.
• El tratamiento se basa en N-acetil-cisteína en dosis de 140 mg/kg VO, y luego 70mg/k/dosis por 17 dosis.
• Si el paciente presenta falla hepática fulminante, debe derivarse precozmente a un centro de transplante. Si bien su sobrevida a 1 año no es tan buena como los pacientes con daño hepático crónico (73%), es la única alternativa que mejora la sobrevida.
• Complicaciones potencialmente letales: hipoglycemia, infección, edema cerebral y coagulopatía.
