How to book an appointment to collect HPB steps tracker
Step 1: Book an appointment Click ‘Book an appointment’ button.
Read, acknowledge and accept the website’s
Terms and Conditions
Please note that you are able to book a new appointment only if:
1. You have not booked an appointment before on this website
2. You are eligible for a HPB-issued steps tracker
[Please see ‘Eligibility Conditions’ under Terms and
Conditions]
Step 2: Enter your particulars Enter your NRIC number, email address and
full name (as per NRIC) to register for a new
appointment.
(Optional) Collecting for your family members/friends
You are able to include up to 2 family
members/friends in your appointment booking
i.e. you can also help your loved ones to collect
HPB steps trackers on their behalf.
Click ‘+’ button to add new participant.
Enter new participant’s NRIC and full name
(as per NRIC).
Please note that for every participant added, he or she has to
meet the eligibility conditions [Please see ‘Eligibility Conditions’
under Terms and Conditions]

Note: • The appointment confirmation letter will only be sent to
the registered email account.
• If there are 3 trackers to be collected within one appointment, the confirmation letter will reflect details of the 3 participants.
Step 3: Select collection venue &
appointment timeslot 1. Select your preferred collection region
2. Select Post Office under the chosen region Click here for list of participating Post Offices for issuance of
HPB steps tracker.
3. Select your preferred collection date from the
calendar
4. Select your preferred collection timeslot
Step 4: Verify your appointment Verify and confirm your appointment details:
1. Participant’s details
• NRIC
• Full Name
2. Collection details
• Chosen Post Office (address shown)
• Appointment Date
• Appointment Time
Click ‘Submit’ to confirm your appointment.
Step 5 : Your appointment is confirmed! Click on the link (as circled in red) to access the
appointment confirmation letter.
Alternatively, it will also be sent to the email
address that you have provided. If you did not
receive the email, please look through your junk
email folder.
Please print the appointment confirmation email
and bring along to the designated Post Office on
collection day.

• If you are collecting the steps tracker for someone, you are required to:
a) fill up the authorization section (found at the bottom section of the confirmation letter)
b) ensure that the actual participant signs on the authorization section
c) bring along the printed copy of the participant’s appointment confirmation letter, a copy of their NRIC and your own NRIC

Note: You will not be able to collect the steps trackers without the confirmation letter with the completed authorization form, a copy of the participant’s NRIC and a copy of own NRIC.
Changing details of your appointment If you have booked an appointment and would
like to change appointment details, hover over
the ‘Change details of appointment’ button.
These 3 options will be shown:
1. Change appointment location, date and time
2. Add/remove participants
3. Cancel appointment for all participants
How to change your appointment details to collect HPB steps tracker
Option 1: Change collection venue, date
& time To change collection venue and appointment
timeslot:
1. Enter the registered NRIC number and Email,
then click ‘Next’
2. Confirm all the participants’ particulars
within the same appointment, then click
‘Next’
3. Your existing appointment details will be
shown at the top section of the page.
4. Proceed to select your preferred venue, date
and/or time
5. Click ‘Submit’
6. You will receive a confirmation email with
your amended appointment details.

Note: By changing collection venue, date and/or time, it applies to all the trackers booked under the same appointment booking.
Option 2: Add/remove participants Enter the registered NRIC number and email,
then click ‘Next’
A) To add new participants to collect on behalf
Click ‘+’ button to add new participant
Enter new participant’s NRIC and Full Name (as
per NRIC)
Please note that for every participant added, he
or she has to meet the eligibility criteria to
collect a HPB-issued steps tracker.
B) To remove participants to collect on behalf
Click ‘-’ button to remove participant
Option 3: Cancel appointment for all
participants To cancel the appointment for all participants,
please enter your registered NRIC number and
Email.
Click “Proceed to Cancel” to cancel appointment.

Note: By canceling the appointment, it applies to all the trackers booked under the same appointment booking
Note: If you are collecting the steps tracker on behalf, you are required to bring along a printed copy of the appointment letter with the authorisation section completed by the actual participant.
