Dynamic Stabilization with Active Locking Plates Delivers Faster, Strong-er, and More Symmetric Fracture- Healing

Abstract
Background: Axial dynamization of fractures can promote heal-
ing, and overly stiff fixation can suppress healing. A novel tech-
nology, termed active plating, provides controlled axial dynami-
zation by the elastic suspension of locking holes within the plate.
This prospective, controlled animal study evaluated the effect of
active plates on fracture-healing in an established ovine osteoto-
my model. We hypothesized that symmetric axial dynamization
with active plates stimulates circumferential callus and delivers
faster and stronger healing relative to standard locking plates.
Methods: Twelve sheep were randomly assigned to receive a
standard locking plate or an active locking plate for stabilization
of a 3-mm tibial osteotomy gap. The only difference between
plates was that locking holes of active plates were elastically sus-
pended, allowing up to 1.5 mm of axial motion at the fracture.
Fracture-healing was analyzed weekly on radiographs. After sac-
rifice at nine weeks postoperatively, callus volume and distribu-
tion were assessed by computed tomography. Finally, to deter-
mine their strength, healed tibiae and contralateral tibiae were
tested in torsion until failure. Results: At each follow-up, the
active locking plate group had more callus (p < 0.001) than the
standard locking plate group. At postoperative week 6, all active
locking plate group specimens had bridging callus at the three
visible cortices. In standard locking plate group specimens, only
50 % of these cortices had bridged. Computed tomography dem-
onstrated that all active locking plate group specimens and one
of the six standard locking plate group specimens had developed
circumferential callus. Torsion tests after plate removal demon-
strated that active locking plate group specimens recovered 81 %
of their native strength and were 399 % stronger than stand-
ard locking plate group specimens (p < 0.001), which had recov-
ered only 17 % of their native strength. All active locking plate
group specimens failed by spiral fracture outside the callus zone,
but standard locking plate group specimens fractured through
the osteotomy gap. Conclusions: Symmetric axial dynamization
with active locking plates stimulates circumferential callus and
yields faster and stronger healing than standard locking plates.
Kommentar
In dieser prospektiven tierexperimentellen Multicenter-Stu-
die wurde die Heilungstendenz von Extremitätenbrüchen bei
Schafen mit unterschiedlichen Verplattungen untersucht. Die
Standard-Osteosyntheseplatte unterschied sich von der Acti-
ve-Osteosyntheseplatte nur im Design der Schraubenlöcher.
Die Schraubenlöcher des „Active-locking“-Systems sind mit
Silikonplatzhaltern unterfüttert und erlauben eine axiale Ro-
tation von bis zu 1,5 mm. Ziel dieser Studie war, die Annah-
me zu prüfen, ob diese mögliche axiale Rotation einen positi-
ven Reiz auf den entstehenden Kallus ausübt, welcher damit
kräft iger und schneller entsteht und die Frakturheilung be-
schleunigt wird. Wöchentlich wurde radiologisch das Kallus-
wachstum beobachtet und nach 9 Wochen erfolgte eine Ab-
schlusskontrolle mittels CT. Anschließend wurde die Belast-
barkeit der verheilten Brüche nach Plattenentfernung getestet.
Es zeigte sich in der „Active-locking“-Gruppe mehr Kallus-
bildung und in allen Ebenen Brückenbildungen im Gegensatz
zur Standardgruppe. Im Anschluss hielt die Frakturstelle in
der „Active-locking“-Gruppe viermal soviel Bruchlast aus als
die Standardplattengruppe.
No Diff erence on Quantitative Magnet-ic Resonance Imaging in Patellofemo-ral Cartilage Composition Between Patients With Patellofemoral Pain and Healthy Controls

Abstract
Background: Retropatellar cartilage damage has been suggest-
ed as an etiological factor for patellofemoral pain (PFP), a com-
mon knee condition among young and physically active indi-
viduals. To date, there is no conclusive evidence for an associa-
tion between cartilage defects and PFP. Nowadays, advanced
quantitative magnetic resonance imaging (MRI) techniques en-
able estimation of cartilage composition. Purpose: To investigate
differences in patellofemoral cartilage composition between pa-
tients with PFP and healthy control subjects using quantitative
MRI. Study Design: Cross-sectional study; Level of evidence, 3.
Methods: Patients with PFP and healthy control subjects under-
went 3.0-T MRI including delayed gadolinium-enhanced MRI
of cartilage and T1ρ and T2 mapping. Differences in relaxation
times of patellofemoral cartilage were compared between groups
by linear regression analyses, adjusted for age, body mass in-
dex, sex, sports participation, and time of image acquisition. Re-
sults: This case-control study included 64 patients and 70 con-
trols. The mean (± SD) age was 23.2 ± 6.4 years and the mean
body mass index was 22.9 ± 3.4 kg/m2; 56.7 % were female. For
delayed gadolinium-enhanced MRI of cartilage, the mean T1GD
relaxation times of patellar (657.8 vs 669.4 ms) and femoral car-
tilage (661.6 vs 659.8 ms) did not significantly differ between pa-
tients and controls. In addition, no significant difference was
found in mean T1ρ relaxation times of patellar (46.9 vs 46.0 ms)
Relevanz für die Praxis
Das „Active-locking“-System kann eine interessante Ent-wicklung in der Osteosynthese von Extremitätenbrüchen werden. Weitere Studien sind wünschenswert. Erwähnt werden muss, dass diese Studie von Zimmer gesponsert wurde.

and femoral cartilage (50.8 vs 50.2 ms) and mean T2 relaxation
times of patellar (33.2 vs 32.9 ms) and femoral cartilage (36.7 vs
36.6 ms) between patients and controls. Analysis of prespecified
medial and lateral subregions within the patellofemoral cartilage
also revealed no significant differences. Conclusion: There was
no difference in composition of the patellofemoral cartilage, es-
timated with multiple quantitative MRI techniques, between pa-
tients with PFP and healthy control subjects. However, clinical-
ly relevant differences could not be ruled out for T1ρ in the ado-
lescent population. Retropatellar cartilage damage has long been
hypothesized as an important factor in the pathogenesis of PFP,
but study findings suggest that diminished patellofemoral carti-
lage composition is not associated with PFP.
Kommentar
In dieser kontrollierten Querschnittstudie aus den Niederlan-
den wurde der Zusammenhang von retropatellaren Knorpel-
veränderungen und vorderem Knieschmerz untersucht. Die-
se besagten Knorpelveränderungen werden immer wieder als
Ursache für den vorderen Knieschmerz angegeben. Es wurden
64 Probanden mit vorderem Knieschmerz und 70 Kontroll-
probanden in die Studie eingeschlossen. Das Durchschnitts-
alter betrug 23 Jahre und der BMI lag um die 23. Etwas über
die Hälft e der Probanden war weiblich. Andere messbare
mögliche Ursachen eines vorderen Knieschmerzes wurden
ausgeschlossen. Es zeigten sich keine fassbaren Unterschiede
im Aufb au und der Struktur der retropatellaren Knorpelstruk-
turen in beiden Gruppen bei Anwendung von speziell auf den
Knorpel gewichteten MRT-Bildschnitttechniken. Es wird pos-
tuliert, dass der vordere Knieschmerz nicht mit retropatellaren
Knorpelveränderungen zusammenhängt.
Outcomes and Related Factors of C5 Palsy Following Cervical Laminectomy With Instrumented Fusion Compared With Laminoplasty

Abstract
Study Design: A retrospective study. Objective: The aim of this
study was to analyze the clinical outcomes and related factors of
C5 palsy (C5P) following posterior cervical laminectomy with
fusion (LF) compared with laminoplasty (LP). Summary of
Background Data: C5P is more common after LF than after LP.
There have not been any studies on C5P-LF compared with C5P-
LP. Methods: We retrospectively analyzed consecutive cases that
underwent cervical LF for nontraumatic cervical myelopathy
or myeloradiculopathy (CMR). To analyze the related factors,
C5P-LF and non-C5P-LF groups were compared. To assess the
clinical parameters, preoperative diagnosis, clinical symptoms,
surgical procedures, and outcome instruments were analyzed.
Radiographically, we analyzed preoperative maximal spinal cord
compression ratio, presence of C4–5 foraminal stenosis (FS),
and correction angles on the sagittal plane. To compare with
C5P-LP, we analyzed the incidence, time of onset, grade of mus-
cle weakness, other accompanying cervical nerve root palsies,
recovery time, and degree of final recovery in the C5P-LF and
the C5P-LP groups of 100 consecutive LPs. Results: A total of 90
LF patients were enrolled (M:F = 54:36, mean age 61.1 yr, mean
follow-up 35 months). C5P occurred in 26 patients (28.9 %), and
14 cases (53.8 %) demonstrated other cervical nerve root pal-
sies. Clinically significant differences were observed between
the C5P-LF and non-C5P-LF groups with regard to preoperative
clinical diagnosis (CMR 88.5:42.2 %, P < 0.001) and presence of
preoperative upper extremity weakness (57.7:32.3 %, P = 0.02).
Comparison between the C5P-LF and C5P-LP groups showed
significant differences between incidence (28.9:4 %), mean
grade of weakness (2.1:3.5), accompanying nerve root symp-
toms (53.8:0 %), recovery time (20.8:10.5 weeks), and incidence
of incomplete recovery (15.4:0 %). Conclusion: C5P-LF pa-
tients showed higher incidence, more severe weakness, frequent
involve ment of multiple cervical nerve roots, and longer recov-
ery time than C5P-LP patients. Preoperative clinical manifesta-
tion of CMR and preexisting upper extremity weakness were the
related factors of C5 palsy.
Kommentar
In dieser koreanischen retrospektiven Studie wurde ein restli-
ches neurologisches Defi zit C5 nach C5-Dekompressions-Fu-
sionsoperation und nach Laminektomie verglichen. 90 Pro-
banden nach Fusionsoperation und 100 Probanden nach La-
minektomie wurden eingeschlossen. Nach der Fusionsopera-
tion traten bei knapp 30 % C5-Läsionen und bei 53,8 % andere
Nervenläsionen auf. Dies war häufi ger als in der Laminekto-
mie-Gruppe. Auch die motorischen Schwächen anderer Ner-
venwurzeln traten in der Fusionsgruppe häufi ger auf und die
Regenerationszeit war signifi kant länger. Diese Studie spricht
bei C5-Läsionen eher für eine Laminektomie-Operation und
nicht für eine Fusionsoperation aufgrund häufi gerer Nerven-
läsionen postoperativ und längerer Rehabilitationszeit.
Relevanz für die Praxis
Laut dieser Studie gibt es keinen Zusammenhang zwi-schen vorderem Knieschmerz und retropatellaren Knor-pelschäden. Es zeigt sich wieder einmal, dass auch die teuersten medizinischen Untersuchungstechniken die kli-nische Untersuchung und Erfahrung nicht ersetzen kön-nen. Eine genaue Inspektion, Palpation sowie die Ge-lenkstestung, die Berücksichtigung von Gangbild, Stabili-tät, Bänderschwäche, Kapsellaxizität, Muskelbeschaffen-heit, Achsenfehlstellung, Fußanatomie, Verkürzungen und Gelenksblockaden bleiben weiterhin die wichtigsten Werkzeuge, um auf die Schmerzursache zu kommen und die richtige Therapie einzuleiten. Es sei dahingestellt, ob es in der Ausbildung zum neuen Facharzt für Orthopädie und Traumatologie neben dem Erlernen zahlreicher Ope-rationstechniken beider Fachgebiete genug Zeit und kom-petente Zentren geben wird, um eine fundierte Untersu-chungstechnik aufbauen zu können.

Relevanz für die Praxis
Patienten mit einer radikulären Symptomatik C5 und nach einer zervikalen Fusionsoperation C5/6 zeigen signifikant häufiger Restläsionen C5 und motorische Schwächen an-derer Nervenwurzeln als Patienten mit einer radikulären Symptomatik C5 nach einer C5-Laminektomie-Operation.
