Board of Physical Medicine and Rehabilitation
• Physical medicine and rehabilitation (PM&R), also referred to as physiatry, is a medical specialty concerned with diagnosis, evaluation, and management of persons of all ages with physical and/or cognitive impairment and disability. This specialty involves diagnosis and treatment of patients with painful or functionally limiting conditions, the management of comorbidities and co-impairments, diagnostic and therapeutic injection procedures, electrodiagnostic medicine, and emphasis on prevention of complications of disability from secondary conditions.
• Physiatrists are trained in the rehabilitation of neurologic disorders, and in the diagnosis and management of impairments of the musculoskeletal (including sports and occupational aspects) and other organ systems, and the long-term management of patients with disabling conditions. Physiatrists provide leadership to multidisciplinary teams concerned with maximal restoration or development of physical, psychological, social, occupational and vocational functions in persons whose abilities have been limited by disease, trauma, congenital disorders or pain to enable people to achieve their maximum functional abilities.
Definitie Kinderrevalidatie
• “Kinderrevalidatie is een specialisatie in de geneeskunde die zich richt op lichamelijk gehandicapte kinderen die in hun ontwikkeling worden bedreigd door de gevolgen van een ziekte of een aangeboren of verworven aandoening.

Definitie Kinderrevalidatie 2
• Kinderrevalidatie beoogt het herstel van de (potentieel) verstoorde interactie met de omgeving en het bereiken van een optimale graad van autonomie en maatschappelijke participatie.
• Het onderzoek en de behandeling vinden plaats in een pedagogisch kader en richten zich zowel op de ontwikkeling van het kind als op de gevolgen voor het gezin.

Wanneer overgang naar revalidatiecentrum ?
• De medische toestand is stabiel
• Er is enige vorm van communicatie mogelijk : – logopedie
– comastimuleringsprogramma (H. Eilander)
Lichamelijke aandachtspunten : in ziekenhuisfase
• Conditiedefect : dagprogramma
• Mondfunktiestoornissen : eten en drinken
• Fijne Motoriek : ADL
• Grove motoriek : houding en beweging

Onderzoek motoriek
• Staat van alertheid beschrijven
• Op stoornis niveau : kracht, mobiliteit van gewrichten, spierlengte en TONUS
• Op aktiviteitenniveau : liggen, zitten, staan, lopen, voortbewegen
• Aandacht voor abnormale houding of beweging: links versus rechts , proximaal versus distaal, verergeren bij willekeurige motoriek ( tremor,dysmetrie )?
• Secundaire letsels : fracturen , perifere zenuwen
TONUS REGULARISATIE
• Weerstand die men voelt bij het passief bewegen van een lidmaat
– Hypertoon : teveel spanning
– Normotoon : normale spanning
– Hypotoon : te weinig spanning
– Dystoon : wisselende spanning
• Spasticiteit

Definitie Spasticiteit
• Spasticity is a motor disorder characterized by a velocity-dependent increase in tonic stretch reflexes (‘muscle tone’) with exaggerated tendon jerks, resulting from hyperexcitability of the stretch reflex, as one component of the upper motor neuron syndrome." (Lance 1980)
• Pyramidaal syndroom :klinische kenmerken – Snelheidsafhankelijke weerstand – Hyperreflexie – Bewegen in patroon – Pathologische reflexen
Spasticiteit - Rigiditeit
• Catch and release (Tardieu)
• “Between the bone and the nerve there is the muscle” (Tardieu)
• Clonus : snelheid van uitdoven bepaalt ernst
• Rigiditeit : loden pijp ; snelheidsonafhankelijk; tijdens gehele beweegtraject; zowel agonist als antagonist
• In ICF = 1 verzamelbegrip : F72 : spastische parese

Doorbewegen
• Doorbewegen is het langzaam, voorzichtig en voornamelijk passief (maar bij voorkeur ook actief) bewegen van de gewrichten Het wordt meestal gedaan door de kinesitherapeut ;
• Nadelen : – effect van doorbewegen snel
weg als de patiënt na de behandeling niet blijft bewegen;
– vrij arbeidsintensief
Op lengte houden van spieren
• Spieren kunnen niet op lengte gehouden worden door enkele malen per dag door te bewegen, echter wel door positionering.
• Tardieu bewees dat spieren zich door langdurige rek (meer dan 6 uur per dag) aanpassen door toename van het aantal sarcomeren, zodat ze niet verkorten
• Spieren verkorten zelden door tonusverhoging, wel soms door secundaire letsels op spierniveau of door continue spierdysbalans

Spierverkortingen voorkomen
• Positionering – Lighouding ; evt.zit/stahouding
– Orthesen
– Invloed zwaartekracht
• Kinesitherapie – Niet doorbewegen om spierverkorting tegen te gaan, wel
om ontspanning te betrachten
– De kinesist is de bewegingsprofessional in revalidatie team
– Trachten actieve beweging in de antagonisten uit te lokken
Oefentherapie
• Taakspecifiek oefenen is aangewezen ( activiteitenniveau)
• Geen plaats meer voor traditionele neurologische oefenmethoden (bvb.NDT)

• Orthesen of orthetische hulpmiddelen zijn hulpmiddelen die extern worden aangebracht om functies van het bewegingsapparaat en anatomische eigenschappen van structuren verwant aan beweging te wijzigen of te ondersteunen

Zich zelfstandig voorbewegen
• Lopen is ten opzichte van rolstoelrijden geen hogere vorm van voortbewegen, alleen een andere manier.
• Waar het om gaat is het betrchten van de zelfstandigheid van voortbewegen
• Snel starten in een rolstoel leidt vaak sneller tot weer lopen

CONCLUSIE
• Revalidatie is er voor personen met fysieke en/of cognitieve stoornissen en beperkingen
• De revalidatie arts stuurt een multidisciplinair team aan
• Doel van kinderrevalidatie is het bereiken van een optimale graad van autonomie en maatschappelijke participatie
• In een pedagogisch klimaat en met oog voor het gezin/kindsysteem
