“Cuando los productos de degeneración pulpar alcanzan el
periodonto producen una periodontitis localizada que puede
extenderse a la cavidad oral” (Gutmann y cols. 2007):
- Respuesta inflamatoria periodontal (periodontitis).
- Pérdida ósea periapical, perirradicular o en la furcación.
- Movilidad dentaria.
- Fístula a través del surco gingival.

* Patología dentaria: surco de desarrollo radicular, fracturas...
* Periodontitis de origen endodóncico (caries, fracturas):
- Los productos de degradación de las proteínas y las toxinas bacterianas intrapulpares originan inflamación periodontal.
- Reabsorción ósea perirradicular / periapical.
- Tractos fistulosos que desembocan a nivel del surco gingival.
* Yatrogenia durante el tratamiento endodóncico:
- Sobreinstrumentación.
- Sobreobturación.
- Perforaciones.

tienen pocos efectos a largo plazo en la pulpa

- Placa bacteriana del epitelio de la bolsa.
- Yatrogenia: raspado y alisado radicular.
- Traumatismos sin fractura: luxaciones.
Exposición de túbulos dentinarios tras el raspado y alisado radicular.

deposición de dentina (esclerosis tubular).

- Concepto -
Síndrome clínico caracterizado por:
1) Dolor dental localizado y agudo.
2) Absceso periodontal (dento-alveolar).
3) Imagen radiolúcida de pérdida ósea alveolar,
cuya etiología (pulpar, periodontal o combinada)
es inicialmente incierta, exigiendo un cuidadoso
diagnóstico diferencial que determine el plan de
tratamiento a seguir.

(a y b). Primary endodontic lesion. Pathway of communication: apex or a lateral canal.

area. (d) Primary periodontal lesion. The progression of periodontitis is toward the

- Hay: caries, traumatismo, restauración coronal, tto. endodóncico previo,
edema periapical, dolor a la percusión, dolor a la palpación, …
- Supuración por el surco gingival.
- No hay: enfermedad periodontal, movilidad , placa, cálculo…
4) RADIOGRAFÍA: pérdida ósea localizada (periapical, perirradicular, furca).
Radiografía de trazado con gutapercha: señala al ápice, furca…
5) TRATAMIENTO: curan tras el tratamiento de conductos.

Anamnesis
Diente 46 con varias obturaciones en el último año. Hace 1 mes presentó un
episodio de dolor e infección aguda tratado en urgencias con antibióticos y
AINEs. En estos momentos el paciente se encuentra asintomático.
Exploración clínica
- Percusión vertical y horizontal no especialmente molesta.
- Respuesta a test térmicos fríos negativa.
- Palpación molesta en encía insertada vestibular del 46
con tumefacción fluctuante.
- Sondaje V (3-12-3) L (3-3-3).
- Sondaje de furca vestibular completo.
- Buen estado periodontal general en el resto de la boca.
Exploración radiológica
Obturación filtrada y lesión radiolúcida a nivel de la furca del 46.

Diagnóstico
Pulpar: Necrosis.
Periapical: Periodontitis apical sintomática - crónica.
Tratamiento
Se plantea el caso en dos sesiones con medicación intermedia de Ca(OH)2 por
cuatro semanas.
- Primera sesión:
Apertura y localización de conductos, instrumentación hasta #35, irrigación
con PUI, medicación con Ca(OH)2.

continua de calor.

- SONDAJE: bolsa profunda y amplia, infraósea, en U; afectación furcal.
- RADIOGRAFÍA: pérdida ósea periapical, perirradicular o en la furca;
defectos verticales-angulares; extensión ápico-coronal.
3) CLÍNICA: periodontitis apical crónica / asintomática / persistente
- Hay: movilidad, caries, traumatismo, restauración coronal, tto. endodóncico
previo, perno que perfora, perforación o falsa vía…
- “No hay”: edema periapical, dolor a la percusión, dolor a la palpación,
enfermedad periodontal, movilidad , placa, cálculo…
4) TRATAMIENTO: curan tras el tratamiento de conductos y el tto.periodontal.

lesion, however, a bony defect at the furcal area remained. Note that endodontic
treatment alone did not yield complete healing of the defect. Periodontal treatment is

Dolor moderado, al frío, localizado.
Exploración clínica
Restauración superficial.
Pruebas de vitalidad normales.
Movilidad grado III. Trauma oclusal.
Sondaje periodontal: 7, 10, 5.
Encía marginal eritematosa y edematosa.
Fístula periodontal.
Exploración radiológica
Obturación de RC en mesial.
Pérdida ósea en mesial.
Ensanchamiento del espacio periodontal.
Borramiento de la cortical.

Diagnóstico:
Enfermedad periodontal (P. Crónica del Adulto).
Estado Pulpar: pulpitis irreversible.
Estado Periapical: periodontitis apical aguda.
Tratamiento:
Tratamiento de conductos (Biopulpectomía).
Tratamiento periodontal.

Exposed lateral canal is one of the
possible pathways of infection of the root
canal.

1) PRUEBAS DE VITALIDAD PULPAR: diente no vital, relativas.
2) PATRÓN DE PÉRDIDA ÓSEA:
- SONDAJE: bolsa profunda y amplia, en U, infraósea.
- RADIOGRAFÍA: osteolisis coronal y periapical.
3) CLÍNICA: enf. periodontal + periodont. apical
- Hay: enfermedad periodontal generalizada, movilidad, cálculo…
- También hay: caries, traumatismo, tto. endodóncico previo…
4) TRATAMIENTO: tratamiento de conductos y tto. periodontal.

Lesión endodóncica y lesión periodontal en el mismo diente pero
separadas y distintas, con evolución independiente.

LESION ENDO-PERIODONTAL TIPO III:
COMBINADA O VERDADERA
Mujer de 41 años. Dolor en 36 desde hace varios meses.
Test de vitalidad negativos.
Sondaje: bolsa de 6 mm por V; furca grado I por V y L; gingivitis; sangrado al sondaje.
Exudado a la presión digital.
Movilidad tipo 2.
Radiografía: radiolucidez de periápice al 1/3 medio radicular MV (8 x 3 mm); pérdida
ósea horizontal y vertical generalizada.
LESION ENDO-PERIODONTAL TIPO III:
COMBINADA O VERDADERA
Control al finalizar la obturación Control a los 3 meses
Se realizó tratamiento de conductos seguido de tratamiento
periodontal.
Está asintomática, no hay movilidad, no hay sangrado al sondaje
y la profundidad de bolsa es casi fisiológica.

La posibilidad de infecciones por contigüidad entre el
implante y el diente adyacente ha dado origen al concepto de
“patología implanto-endodóncica.

1) Diente a implante (Endodontic implantitis).
INTERRELACIÓN DIENTE ENDODONCIADO – IMPLANTE
- Patología implanto-endodóncica -
Cuando tras la colocación del implante se produce la exacerbación
de un proceso infeccioso periapical previo que por contigüidad
produce la peri-implantitis retrograda del implante.
Pueden darse tres casos:
1) Diente adyacente no endodonciado con periodontitis apical.
2) Diente adyacente endodonciado con periodontitis apical crónica
persistente.
3) Alveolo post-extracción de diente con PA.

INTERRELACIÓN DIENTE ENDODONCIADO – IMPLANTE
- Patología implanto-endodóncica -
Se evidencia una lesión radiolúcida en la región periapical del
implante y del diente adyacente, desarrollada poco después de la
inserción, acompañada de dolor, sensibilidad, hinchazón y presencia
de un tracto fistuloso.
2) Implante a diente (Implant endodontitis).
INTERRELACIÓN DIENTE ENDODONCIADO – IMPLANTE
- Patología implanto-endodóncica -
“Implante a Diente”:
Cuando la inserción del implante provoca la necrosis pulpar y la
periodontitis apical del diente adyacente previamente sano.
Estaría provocada por el trauma directo durante la preparación del
lecho implantario (por invasión del espacio periodontal,
sobrecalentamiento del hueso o interrupción del aporte sanguíneo a la
pulpa) o por infección intraoperatoria.
La lesión endodóncica que se produce se manifiesta por una lesión
osteolítica periapical, radiolúcida, que puede afectar por contigüidad
al implante y alterar su osteointegración.

- Patología implanto-endodóncica -
Grupo A: implantes sin dientes adyacentes.
Grupo B: implantes con dientes adyacentes no endodonciados.
Grupo C: implantes con dientes adyacentes endodonciados.
- Respetar distancia adecuada implante - diente .
- Correcta dirección colocación implante.
- Evitar sobrecalentamiento en la osteotomía: activa focos
infecciosos.

DIENTE ADYACENTE NO ENDODONCIADO-IMPLANTE:
- Pruebas vitalidad pulpar a posibles dientes adyacentes problema
(necrosis , traumatismo).
- Endodoncia intencionada dientes adyacentes (caries, fractura)..
- Exodoncia previa de dientes con periodontitis apical en los que esté
indicada.
PREVENCIÓN PATOLOGIA DIENTE - IMPLANTE
DIENTE ADYACENTE ENDODONCIADO-IMPLANTE:
- Re-endodoncia dientes ady. sospechosos de p. apical persistente.
- Apicectomia de dientes (pm inf sobre todo) con radiolucidez apical
en el momento de la colocación de implantes.
- Control de la distancia implante y diente endodonciado y /o el
tiempo transcurrido entre endodoncia y la colocacion del implante.
- Evitar sobrecalentamiento: activación lesiones latentes.
PREVENCIÓN PATOLOGIA DIENTE - IMPLANTE
IMPLANTE EN EL ALVÉOLO
DE UN DIENTE EXTRAÍDO
CON PERIODONTITIS APICAL:
- Antibióticos bactericidas: amoxicilina; contraindicada la inserción
si la infección continúa activa.
- Tratamiento endodóncico previo a la exodoncia para regenerar las
lesiones radiolúcidas en el futuro lecho del implante.
- Cirugía periapical en dientes endodonciados que se vayan a extraer.
Así se regenera la zona ósea dañada antes de la colocación de los
implantes.
