DEFINITION
Définition de l’ischémie myocardique : déséquilibre entre besoins en oxygène et en énergie du myocarde et apports assurés par la circulation coronaire. L’angine de poitrine est par définition la douleur intermittente consécutive à l’apparition de cette ischémie myocardique . Stable : sans évolutivité dans le temps (fréquence et durée des crises similaires)

symptomatologie
• Survient à l'effort, lors d'une contrariété, au froid.
• Cède à l'arrêt de l'effort/stress • Calmé à la TNT en sublingual (moins de 1’)
• Durée de 10 à 15‘ • Siège :rétro sternal irradiant (oppression,
constrictions rétrosternales) vers mandibule, bras gauche, dos et épigastre.
• Type : constrictive, brûlure, pesanteur, blockpnée.

EXAMEN CLINIQUE • Examen clinique le plus souvent normal. • Rechercher :
– les atteintes vasculaires périphériques (pouls, et recherche de souffle sur les principaux trajets vasculaires, anévrysme aorte abdominale.
– Souffle (sténose A, CMO) – Les FDR: HTA, Hypercholestérolémie

Examens complémentaires (I) • ECG normal en dehors de la crise.
– Rarement T nég, St sous décalé.
– Séquelle Infarctus, HVG. • Biologie
– Troponine négative (si Pos = SCA = Urgence, sauf IRC) – Anémie (angor fonctionnel), glycémie, lipides
• RX Thorax – Si clinique évoque origine pulmonaire
• CONCLUSION
