Familia Enterobacteriaceae Orden XIII Enterobacteriales. Está formada por 41 géneros y más de
100 especies. Muchos de los géneros y especies
recientemente reconocidos no tienen importancia clínica para el hombre y animales.

Hasta hace algunos años los géneros y especies eran definidos por análisis bioquímicos y antigénicos.
Actualmente se utilizan técnicas de hibridización y de secuenciación de ácidos nucleicos debido a que definen con mayor exactitud la relación de los organismos dentro de la familia.

Las enterobacterias son bacilos gram negativos y como tales poseen una pared celular típica de las bacterias gram negativas.
Está constituida:– una membrana externa, – de una capa delgada de peptidoglicano y – de un espacio periplásmico que recubre la
membrana citoplasmática.

La membrana externa protege a las enterobacterias de la acción de las sales biliares y de los fermentos digestivos.
Constituida de una doble capa lipídica dentro de la que se encuentran moléculas de lipopolisacárido (LPS) que comprende tres partes:– Lípido A que constituye la endotoxina– Core central formado por el polisacárido de base que
constituye el antígeno R– Poliosidas de las cadenas laterales que constituyen los
antígenos O

También encontramos proteínas y entre ellas las porinas (Omp) que forman canales en la membrana externa para el pasaje de moléculas hidrófilas.

El peptidoglicano constituye una capa rígida compuesta por cadenas lineales de poliosidas unidas entre sí por péptidos.
El ensamblaje y la remodelación de la pared celular depende de enzimas transpeptidasas y carboxipeptidasas que también intervienen en la unión de las antibióticos betalactámicos a la pared permitiéndoles su acción.

Antígeno común ECA (Enterobacterial Common Antigen). Sólo existe en las bacterias de la Familia Enterobacteriaceae (interés taxonómico)
Antígenos O o somáticos: poliosidas fijados sobre los LPS. – Son termoestables y resistentes al calor. – Permiten la clasificación de las especies en
serotipos o serovares, con gran significado clínico e epidemiológico.

Antígeno R polisacárido del core central. – La desaparición del antígeno O lo desenmascara y
revela las cepas rugosas, autoaglutinables más fácilmente fagocitadas y menos patógenas.
Antígeno H o flagelar sólo existen en las cepas móviles. – Son proteínas específicas denominadas flagelinas,
termolábiles e inactivados por el alcohol.

Comprenden: Antígenos K o capsulares de naturaleza
polisacárida. – Provocan reacciones de aglutinación suave
en presencia de los anticuerpos específicos. En Salmonella typhi se denomina antígeno Vi.
Antígenos de adherencia o adhesinas– de naturaleza proteica presentes en pilis y
fimbrias.

Ampliamente distribuidas en plantas, suelo, agua y en el intestino del hombre
y animales. Algunas especies ocupan nichos ecológicos
muy limitados como Salmonella typhi que causa la fiebre tifoidea y sólo se halla en humanos.
De la presencia de enterobacterias depende la calidad sanitaria del agua o de un producto alimenticio, la ausencia de estas bacterias garantizará la ausencia de patógenos.

Están asociadas a abscesos, neumonía, meningitis,
septicemia y en infecciones de heridas, tracto urinario e intestino como productores de diarrea.

Escherichia coli es conocida como habitante saprófito del intestino.
Sin embargo, otros serotipos se identifican como un frecuente agente causal de diarrea en neonatos y adultos.

Todas interaccionan con la mucosa intestinal, producen enterotoxinas o citotoxinas y pertenecen a ciertos serotipos O:H.

... la infección puede cursar con grandes pérdidas de agua y electrólitos, colitis hemorrágica, desaparición de las vellosidades de la mucosa intestinal y complicaciones clínicas diversas en vasos sanguíneos mesentéricos, absorción deficiente de alimentos, desnutrición, pérdida de peso y muerte en los casos sin tratamiento.

Escherichia coli puede hallarse en todo el planeta, aún en el sector antártico.
No obstante, la situación geográfica, temperatura y humedad ambiente son factores que determinan su incidencia.
El suelo y el agua suelen ser las fuentes de infección, contaminados por las deposiciones de humanos o animales diarreicos.

Enterotixinas LT y STEnterotixinas LT y ST
Están codificadas en plásmidos, conjuntamente con los genes que otorgan resistencia a antibióticos.
La enterotoxina LT (termolábil) es una proteína constituida por una molécula de la subunidad A y cinco moléculas de la subunidad B. La subunidad B se une a un gangliósido receptor situado sobre la superficie de las células epiteliales del intestino, mientras que la subunidad A actúa como activador de la adenilato ciclasa.
