Padre operato per aneurisma aortico. Fratello diabetico. Dieci anni fa rilievo di elevati valori pressori
per cui da allora è in trattamento con atenololo. Esame del fondo dell’occhio fatto anni fa negativo.
Dislipidemia (ipercolesterolemie ed ipertriglicerdiemia) e modesto rialzo delle transaminasi.
Dimostrazione ecografica di steatosi epatica. Iperglicemia, per cui ha iniziato trattamento con
metformina. Ad ecotomografia carotidi ispessimento intimale senza alterazioni emodinamiche.
EO 160 cm. 79 Kg, BMI = 30.8 toni cardiaci ritmici validi. Non soffi. Carotidi normopulsanti, non soffi.
Polsi periferici tutti palpabili. PA 120/80 mmHg.

La NAFLD rappresenta ormai la causa più frequente di epatopatia cronica nella praticaclinica ambulatoriale
La NAFLD è definita da un eccessivo accumulo di trigliceridi nel fegato (>5%),generalmente evidenziato da un quadro di iper-riflettenza all’ecografia, in presenza diun consumo di alcol (accuratamente determinato) < 30/20 g al giorno rispettivamenteper M/F
Oltre all’esclusione di altre cause di danno epatico, la diagnosi di NAFLD è rafforzatadalla contemporanea presenza di alterazioni tipiche della sindrome metabolicariconducibili ad uno stato di insulino resistenza: in particolare iperglicemia,dislipidemia pro-aterogena, ipertensione arteriosa, obesità addominale

In base ai dati di studi di popolazione generale, circa 1/4 degli Italiani tra 18 e 65 annidi età sarebbe affetto da NAFLD. Complessivamente, la prevalenza stimata dellaNAFLD nella popolazione generale si attesta intorno al 20-40%, ma è maggiore negliobesi e nei diabetici
La NASH rappresenta circa il 20% delle NAFLD (prevalenza stimata pari al 2-3% nellapopolazione generale) ed è indicata come la causa eziologica di circa l’80% delle cirrosicriptogenetiche
Nell’area mediterranea, la NAFLD è più frequente negli uomini nella terza e quartadecade di vita (70% dei casi) rispetto alle donne, ma la protezione del sesso femminileviene persa dopo l’insorgenza della menopausa
L’importanza clinica della NAFLD è inoltre legata all’effetto indipendente dipromozione del danno cardiovascolare

L’INSULINO RESISTENZA INDUCE IMPORTANTI
ALTERAZIONI NEL METABOLISMO LIPIDICO
L’ aumentata lipolisi del tessuto adiposo ad opera delle lipasi ormono-
sensibile ed un decremento della lipogenesi. L’ aumento degli acidi
grassi liberi (FFA) a livello ematico determina un aumentato uptake
epatico di FFA che stimola la sintesi dei trigliceridi e la gluconeogenesi.
Una ridotta lipolisi dal fegato, in presenza di iperinsulinemia, infatti, la
produzione delle VLDL viene inibita per un’aumentata distruzione
dell’apoproteina B-100
Un’attivazione della lipogenesi de novo (LDN) degli FFA a livello epatico,
conseguente all’iperinsulinemia

È ormai noto che la NAFLD rappresenta un fattore di rischio
cardiovascolare indipendente.
Da studi prospettici condotti su larga popolazione è stato evidenziato
che l’incidenza di morte o eventi cardiovascolari è quasi doppio nei
pazienti con livelli più elevati di transaminasi rispetto ai soggetti con
livelli normali. L’associazione tra NAFLD e vasculopatia (carotidea
precoce ed avanzata) è risultata indipendente dai classici fattori di
rischio, dal controllo glicemico, dall’uso dei farmaci e dalla presenza di
sindrome metabolica
Pertanto i soggetti affetti da NAFLD dovrebbero periodicamente
sottoporsi ad indagini clinico-strumentali volte a valutare la presenza di
cofattori di rischio cardiovascolare (ipertensione arteriosa, dislipidemia,
diabete mellito) il grado di interessamento cardiovascolare
(ecocolordoppler dei tronchi sovra-aortici, ecocardiografia, test da
sforzo).
In presenza di vasculopatia e diabete, è da tenere inoltre presente
l’aumentato rischio di insufficienza renale cronica

CHIRURGIA BARIATRICA
La chirurgia bariatrica è indicata nei soggetti con obesità
morbigena. Si è dimostrata efficace anche nel ridurre il
danno epatico a lungo termine in alcuni casi selezionati
(BMI>40 kg/m2, BMI>35 kg/m2 con comorbidità quali
diabete, dislipidemia ed ipertensione arteriosa non
controllata), mentre l’indicazione specifica per NASH è
dibattuta ed ancora non dimostrata

Nonalcoholic Fatty Liver Disease (NAFLD)
Encompasses the entire spectrum of fatty liver disease in individuals
without significant alcohol consumption, ranging from fatty liver to
steatohepatitis and cirrhosis.
Nonalcoholic Fatty Liver (NAFL)
Presence of hepatic steatosis with no evidence of hepatocellular injury
in the form of ballooning of the hepatocytes or no evidence of fibrosis.
The risk of progression to cirrhosis and liver failure is minimal.
Nonalcoholic steatohepatitis (NASH)
Presence of hepatic steatosis and inflammation with hepatocyte injury
(ballooning) with or without fibrosis. This can progress to cirrhosis,
liver failure and rarely liver cancer.
NASH CirrhosisPresence of cirrhosis with current or previous histological evidence of
steatosis or steatohepatitis
Cryptogenic Cirrhosis
Presence of cirrhosis with no obvious etiology. Patients with
cryptogenic cirrhosis are heavily enriched with metabolic risk factors
such as obesity and metabolic syndrome.
NAFLD Activity Score (NAS)
An unweighted composite of steatosis, inflammation, and ballooning
scores. It is a useful tool to measure changes in liver histology in
patients with NAFLD in clinical trials.
