เครือ่งมือคือตวัช่วยให้เราท างานได้ง่ายขึน้ สะดวกขึน้ ได้ผลดีขึน้การใช้เครือ่งมือให้เกิดประโยชน์ต้องเข้าใจเป้าหมาย และไม่ติดรปูแบบ

Know WHAT เครือ่งมือน้ีคืออะไร ค าเรียกมีท่ีมาอย่างไร เช่ือมโยงกบัอะไรได้Know WHY เครือ่งมือน้ีมีเป้าหมายอะไร พฒันาขึน้มาเพื่อแก้ปัญหาอะไรKnow HOW การใช้เครือ่งมืออย่างถกูต้องมีวิธีการอย่างไร สามารถพลิกแพลง

ตวัอย่าง เป้าหมายของ Control Chart

Control Chart เป็นเครื่องมือเพื่อท าความเข้าใจ variation หรอืพฤติกรรมของกระบวนการท างาน/ระบบงาน

ต้องเข้าใจต่อว่า variation คืออะไร? มีก่ีประเภท? เข้าใจไปเพื่ออะไร?เมื่อเข้าใจแล้วจะหมดค าถามต่อไปน้ี? KPI ทุกตวัต้องใช้ control chart หรอืไม่ -> ไม่มี variation กไ็ม่ต้องใช้? ต้องท าเป็นข้อมลูรายเดือนเสมอหรอืไม่ -> ซอยช่วงเวลาจนเหน็ variation? ถ้าข้อมูลเป็น 0 จะท าอย่างไร -> ขยายช่วงเวลา หรอื ใช้ตวัวดัอ่ืน

• ต้องการ new state ท่ีแตกต่างไปจากสภาวะท่ีเป็นอยู่• ในช่วงเปล่ียนผา่นจะต้องยกรือ้ถอน old state ออก• ขัน้ตอนการเปล่ียนผา่น

• Unfreezing กวนภาวะสมดลุขององคก์รท่ีมีอยู่• Moving เคล่ือนเข้าไปสู่ new position • Refreezing ท าให้มีภาวะสมดลุใน new position

• Don’t discharge patients presenting emergently with acute cholecystitis without first offering laparoscopic

cholecystectomy.

• Avoid routine cholecystectomy for patients with asymptomatic cholelithiasis.

• Avoid other imaging tests apart from ultrasound for the initial evaluation of patients with suspected gallstone

disease.

• Avoid the routine use of ultrasound in evaluating clinically apparent inguinal hernia.

• Avoid opioid-only modalities for post-operative pain control.

• Don’t initiate empiric antibiotic therapy in the patient with suspected invasive bacterial infection without first

confirming that blood, urine or other appropriate cultures have been obtained, excluding exceptional cases.

• Don’t use a broad spectrum antimicrobial agent for perioperative prophylaxis or continue prophylaxis after the

incision is closed for uncomplicated clean and clean-contaminated procedures.

• Don’t treat uncomplicated community-acquired pneumonia in otherwise healthy, immunized, hospitalized

patients with antibiotic therapy broader than ampicillin.

• Don’t use vancomycin or carbapenems empirically for neonatal intensive care patients unless an infant is known

to have a specific risk for pathogens resistant to narrower-spectrum agents.

• Don’t place peripherally inserted central catheters and/or use prolonged IV antibiotics in otherwise healthy

children with infections that can be transitioned to an appropriate oral agent.

Choosing Wisely: Evidence for Communication

1. Monitoring of daily incident e.g. fall, pressure sore, infection, med error, ADR

2. Concurrent review alerted by triggers Lab (pos blood culture, PTT>100, INR>6, glucose<50, 2x rising BUN) Pharmacy (vit K, Benadryl, Naloxone, Flumazenil, anti-emetic admin) OR (change in proc., intra-op X-ray, intra or post-op death, organ

inj/removal) RR (intubation/reintubation/BiPAP use, X-ray in RR) ICU (post-op ICU admission, use of post-op ventilator >24 hrs) LR (instrumented delivery) Blood bank
