1shy Co ntractor will supply to th e Company re moval of ex isting insu lation and installat ion of new insulation as per the Company requirements an d to co mple te the job

2shy Company will be res ponsible for 10 Cards and gate passes for th e Co ntractors personnel that are going to be mobilized to the site Contractor will guaranty to provid e all th e requ ired documents for all employees under his sponsorship for the purpose and in rela tion to thi sse rvice ag ree ment to th e Co mpany on a tim ely manner

3shy Co mpany will pay to th e Co ntrac tor for the services re nd ered at ra tes and under the co nditionsag ree d in the Purchase Order

The ag reement is valid from06 Decem ber 2015 until 3 1 July 20 16
