1. Arritmias causadas por QT
prolongado • LQTS es un trastorno genético causado por
mutaciones en los genes que codifican los canales de Na y K en el músculo cardiaco (“channelopathies”) y se caracteriza por la prolongación del intervalo QT del ECG y por arritmias durante:
• La activación simpática
• El reposo
• El sueño
De acuerdo al tipo de gen afectado
Tipo de mutación
• En canales del Sodio: 4 mutaciones y 5 variantes identificadas en 13 casos en localización citoplasmática (78%)
• En canales de potasio: en 11 casos de SIDS, 9 mutaciones diferentes o variantes raras se identificaron; 7 de las 9 mutaciones eran nuevas mutaciones (78%)
• En total, 10% de los SIDS tuvieron algún tipo de mutación FUNCIONAL en los genes del LQTS.
• En un estudio estudio colaborativo con Peter Schwartz utilizando DHPLC (denaturing high-performance liquid chromatography) estamos corroborando la incidencia de los genes de LQTS mas frecuentes KCNQ1, KCNH2 y SCN5A en 300 casos de SIDS y encontramos un 15% de incidencia
Resultados
• Algunas variantes identificadas son capaces de causar arritmias fatales y otras que pueden ser parte del “triángulo fatal”
• Estadio del desarrollo Vulnerable, predisposición y factor desencadenante.
• Los niños están constantemente expuestos a situaciones que aumentan la inestabilidad eléctrica del corazón : sueño REM con activación vagal y simpática, infección de vías aéreas que puede inducir hipoxemia y desencadenar reflejos quimioreceptores
• Coexistencia de estos eventos puede incrementar el riesgo de arritmias fatales

• Es posible que la presencia de variantes comunes (2nd hit) pueda amplificar el deterioro funcional electrofisiológico de variantes genéticas localizadas en el mismo gen

• Ya ha sido demostrado en muestras de tejido no identificadas (estudio Noruego) que el 10% de los casos de MSL tienen una variante genéticamente significativa en alguno de los genes del QT prolongado.
Objetivos
M&M • Investigación molecular de 300 casos de MSL
del RU, con investigación de las familias de los casos positivos
• Aplicación de DHPLC (denaturing high-performance liquid chromatography) y anáalisis de los principales genes del QT prolongado: KCNQ1, KCNH2 and SCN5A.
• Variantes genéticas verificadas en un panel de controles internos y en 3 bases de datos de exomas públicas
Resultados
• 7 variantes missense identificadas en 7 (15%) casos de MSL
• Se identificaron 4 variantes genéticas, no existentes en la base de datos de referencia de exomas, en los genes KCNH2 (3 variantes) y SCN5A : podrían considerarse mutaciones de susceptibilidad para el QT prolongado
• 3 casos de MSL presentaron 3 variantes genéticas del SCN5A descriptas anteriormente.
Conclusión
• Confirmamos que el QT prolongado contribuye en un porcentaje de casos de MSL
• A través del estudio de la familia intentamos identificar si son mutaciones de novo o si son heredadas y las familias tienen riesgo de MS
• El estudio permitirá entender el rol de screening geneticos para el QT prolongado en las muertes de SMSL e implicaciones epidemiológicas.

Samples taken at post mortem offer the last opportunity for obtaining a diagnosis of IMD. Between 1989 and 2007 we have investigated over 800 cases of unexplained / unexpected death in neonates, infants and children by fibroblast studies on skin biopsies obtained post mortem. Where possible we obtained urine for organic acid analysis and more recently we added the investigation of acylcarnitine analysis by MS/MS on DBS, bile and CSF samples. We have analysed 197 DBS, 93 bile and 21 CSF samples and established post mortem reference ranges for the full range of acylcarnitine species C0-C18(OH) see Table 2. Using post mortem acylcarnitine analysis we detected and/or confirmed a case of each of the following:- MCADD, MADD, 3-ketothiolase, propionic acidaemia and GAI, the latter two cases were unexpected deaths in known patients. DBS C3 & C3/C2 ratio in PA was 164µmol/L & 2.8 (reference C3/C2 ratio ~0.1) respectively. DBS C8 & C8/C10 ratio in MCADD was 8.4 µmol/L & 11.3 (reference C8/C10 ratio ~1.2) respectively.
The use of acylcarnitine ratios in post mortem samples is of paramount importance as it helps to exclude false positives e.g. C8/C10 in MCAD, C3/C2 in PA/MMA, C14:1/C12:1 in VLCAD. Profiling in DBS, bile & CSF provides valuable complementary data.
Overall in the period between 1989 and 2007 we have diagnosed, or confirmed a suspected diagnosis, in cultured fibroblasts in 70 cases, mostly fatty acid oxidation disorders and respiratory chain defects (see Table 1). Acylcarnitine analysis on DBS, bile and CSF and urine OA analysis with confirmation by fibroblast assay is an effective method of detecting and confirming IMD through autopsy and offers the opportunity for future prenatal diagnosis for the family.

Reflejo laríngeo
(Laryngeal chemoreflex (LCR) • Activado por la estimulación directa de liquido
en contacto con la mucosa laríngea
• Desencadena respuestas complejas: apnea, bradicardia, tragar, hipotensión, redistribución regional del flujo sanguíneo
• Existe solo en los lactantes (no se sabe a que edad desaparece)
LCR: experimentos en animales
• La apnea inducida por el LCR solo es letal cuando el animal esta anestesiado y el estimulo se aplica directamente en la laringe (a través de traqueotomía) o ha estimulado la laringe desde la faringe
• Esto sugiere que luego de que el liquido estimula la faringe, la apnea que induce el LCR puede ser letal si el mecanismo que protege a la vía aérea de la entrada de líquido, tal como el tragar, despertar y reflejos espiratorios están “deprimidos” en el lactante (por ejemplo: AR).

- Se identifico una asociacion genomica significativa entre un SNP en Chr. 15 y SMSL. OR=8.9 (95%CI (6.3-13.1, p=4.0x10-28)
- El Chr.15 incluye mas de 20 marcadores geneticos signficativos , tiene al menos 3 picos estadisticamente independientes y se esta localizado dentro de la region PWS/AS de imprinting (parentally imprinted)

Vit D levels
The report received from the chemical pathology laboratory refers to 25-OHD level of
< 25nmol/L : severely deficient
25 - 50 nmol/L moderately deficient
50 - 75 nmol/L sub-optimal
75 nmol/L adequate
(to convert from nmol/L to ng/mL, which is used in some other studies values should be divided by 2.5).
Histological review of ribs
Addressed presence or absence of:
-widening of the metaphysis,
-thickening of the growth plate,
- presence of blood vessels penetrating the growth plate
- identification of abnormal trabeculae in the rib shaft. The latter was defined as the presence of rather thickened trabeculae showing a core of woven bone with persistent mineralised cartilage
(no case had excess osteoid, fractures or changes or fully developed rickets)

• 62.5% abnormal trabeculae
• These changes could be secondary to inadequate levels of vitamin D.

Radiological findings in the skeleton • 19% cases all with
inadequate levels (but 2 cases) had abnormalities
• Reduced bone density was identified in cases 26, 21, 29, 37 and 38.

Conclusions 1 • A significant proportion of infants and children
who died suddenly and unexpectedly had inadequate levels of vitamin D.
• Our cohort included 25 cases where the cause of death remained unexplained after a thorough post mortem examination. We were unable to confirm an association between VDD and increased risk of infection, and this could potentially be due to the small size of the sample.
• The histology of the ribs demonstrated abnormal features in a high proportion of infants and children with inadequate levels of vitamin D.
Conclusions 2 • Our finding of a higher incidence of bone abnormalities
on histology than on radiology (62.5% versus 19%) is not unexpected as bone mineralization needs to drop approximately 30% before reduced density is appreciated on the radiography.
• Some specific histologic features of “rickets” such as thickening of the cartilaginous growth plate and presence of penetrating vessels cannot be detected on X rays
• It is likely that the abnormal trabeculae seen under the microscope reflect as reduced bone density on radiographs.
• We confirm that Vit D measurement can be done at post mortem
• The results of this study are limited in that it is an observational study, and therefore causality should not be inferred.
