The Know-Do GAP

• Familiar and often quoted “Studies suggest that it takes an average of 17 years for research evidence to reach clinical practice” (Balas & Boren, 2000).

• “…about 30-40% of patients do not receive care according to scientific evidence, and about 20-25% of care provided is not needed or is potentially harmful” (Grol & Grimshaw, 2003).

• Commonwealth Reports confirm variation persists across jurisdictions and we all conclude that although progress has been made, optimizing the uptake of evidence is still needed.

• Providers and decision-makers expressed a need for a credible and comprehensive resource containing contextualized knowledge, referenced and aligned with Ontario’s key priority areas to guide their quality improvement efforts and maximize practice/institutional performance.

What is the Quality Compass?

• The Quality Compass is a user-friendly and intuitive tool that provides one stop access to a variety of resources aligned to HQO and Ontario’s key priority areas.

• The Quality Compass is designed for decision makers and providers who seek tools and resources to support their efforts to make change and implement best practices - leading to sustainable improvements.

What is the Quality Compass?

• The Quality Compass serves as a repository of success stories - highlights “bright spots” - what is working and how sustainable gains were achieved.

• The Quality Compass is an expansion and enhancement of IHI’s Improvement Map tool, built on a web platform with a range of functionality to support an interactive, collaborative exchange of knowledge.

Champion Program Objectives

1. To augment and compliment HQO’s QI initiatives to increase reach, catalyze spread and exchange evidence based practices and experience based knowledge.

2. Build an integrated network of champions to support cross-sectoral QI initiatives from a system perspective to accelerate uptake – tipping point.

3. Provide development opportunities for current and future champions to increase capacity within the system.– support and development of the current Network of Champions

– maintenance of the electronic group space

– ongoing recruitment, orientation and training of new champions.

4. Obtain feedback and evaluate to inform direction.

Champion Program

• An important mechanism to improve, sustain and spread quality improvement efforts across Ontario.

• Champions act as advocates for quality, storytellers and mentors who enable others to make change leading to sustainable improvements.

• Champions are those who have ‘been there, done that’ and can provide peer support, personal experience and knowledge, and share the context with which their improvement was achieved, often through storytelling.

• Champions are key enablers of adoption of change:o From Roger’s Adoption Curve (Rogers, 1995) we know that change, or adoption of

new or best practices often starts with innovators and early adopters, while the early and late majority are more open to ideas from peers they respect.

o Hence, a few key early adopters may champion an idea before it reaches the point where it is achieves widespread uptake.

Champion Network – Staying Connected

A Network or Community of Practice (CoP) is a group of providers, caregivers, policymakers, researchers, healthcare administrators, educators and community leaders who come together to exchange information on a topic of common interest. A community of practice is a network of individuals with common interests who get together to:

• Explore ways of working

• Identify common solutions

• Share good practice and ideas
