Les limites de la morphine
• Effets indésirables
Dépression respiratoire post-opératoire induite par les morphiniques

Les limites de la morphine
• Effets indésirables
• Hyperalgésie induiteD. Fletcher et al, BJA 2014
• Immuno-modulation?Sacerdote et al, Curr. Pharmac. Des 2012
Anesthésie sans morphiniques!
• ALR
• Anti-inflammatoires (dexaméthasone, AINS)
• Agonistes alpha-2 (clonidine, dexmetomidine)
• Kétamine
• Lidocaine IV
PROPRIETES
LOCALE (haute C°) = blocage canaux Na
SYSTEMIQUE(basse C°)
- ANTALGIQUE- ANTI-HYPERALGESIQUE- ANTI-INFLAMMATOIRE
Antalgique et anti-hyperalgésique
Anti-inflammatoire

Quelles recommandations actuelles?
RFE SFAR 2008: Douleur post-opératoire
• En l’absence d’ALR, probablement
recommandée en chirurgie abdominale
• Pas assez d’arguments pour autres chirurgies
Diminution de la douleur post-opératoire et de la consommation de morphine
Koppert et al, Anesthesia Analgesia 2004
24h 48h 72h
Réduction de la durée de l’iléus post-opératoire
Herroeder et al, Annals of Surgery 2007
Diminution de la durée d’hospitalisation
Herroeder et al, Annals of Surgery 2007
Depuis 2008
• Intérêt confirmé en coelioscopieKaba et al, Anesthesiology 2007
Kim et al, 2013
• Extension à d’autres chirurgies
– gynécologieGrady et al, AANA Journal 2012
– urologie– Prostatectomie
Lauwick et al, BJA 2009
– Néphrectomie
Tauzin-Fin et al, J Anesth Clin Phar 2014
Lidocaine IV:
->En laparotomie, si contre-indication à la péridurale -> Intérêt en cœlioscopie
