Deux grands cadres nosologiques:
A un stade précoce de l’infection VIH, patients asymptomatiques (T CD4 > 200/mm3) Burkitt avec une présentation clinique classique(atteinte ganglionnaire au premier plan). L’atteinte viscérale, témoigne d’une dissémination du lymphome.
A un stade tardif de l’infection VIH, CD4 < 100/mm3, Immunoblastique, souvent polymorphe, pratiquement toujours lié à l’EBV. L’atteinte viscérale localisée peut initialement orienter vers une autre pathologie et retarder le diagnostic.

Une ou plusieurs adénopathies peuvent révéler un lymphome. A fortiori s’il s’agit d’adénopathies:- asymétriques- volumineuses- dures- non inflammatoires
L’étude cytologique en ponction peut parfois suffire au diagnostic…La biopsie reste l’examen de référence.

EpiduriteCompression médullaire.LCR : hyperprotéinorachie et méningite lymphocytaire. Mise en évidence de cellules lymphomateuses inconstante. La PCR EBV aurait une bonne valeur prédictive positive.
Méningite blastiqueParticulièrement fréquente au cours des lymphomes de Burkitt avec atteinte médullaire.

• Atteinte viscérale par extension rapide (doublement en moins de 48 h) hépatique, splénique, médullaire et neurologique (PC et/ou méningite) • Syndrome de lyse:- insuffisance rénale- hypocalcémie, hyperphosphorémie- hyperuricémie- LDH > 10N

Le myélogramme permet le diagnostic des formes disséminées (LAL 3)
Le traitement débute par une corticothérapie qui peut à elle seule entraîner une disparition de la masse en quelques jours.
Une chimiothérapie intensive reste nécessaire pour assurer une rémission de bonne qualité.
Les rechutes après 18 mois sont exceptionnelles.

HHV8Caractéristiques épidémiologiques
L’incidence de la maladie de Kaposi dépend de la prévalence de l’infection KSHV / HHV8. Cette infection est rare dans les populations du nord de l’Europe (< 2%) avec un gradient vers la Méditerranée.
Dans les populations infectées par le VIH en Europe et aux Etats-Unis, la séroprévalence atteint 40%.
En Afrique Noire, la séroprévalence est variable mais se rapproche de 50% dans la population adulte des pays d’Afrique Centrale et de l’Est.

Parallèlement au choix de l’examen diagnostique, il est nécessaire d’évaluer :
- l’extension du lymphome, - le retentissement clinique et immunologique de l’infection VIH - l’état général du patient.
Ce bilan doit être réalisé rapidement, sans retarder le début du traitement et doit se limiter aux examens susceptibles d’influencer les choix thérapeutiques.
