Payment Deadline:
Same date as your tuition and fees: June 30, 2013 for Summer 2013.
Late payment: $100 fine from a collections agency restrictive hold placed on college
record
How to Pay: same as tuition! Insurance cost is included on your
tuition bill, visible at your MyMadisonCollege Student Center
Pay online through Student Center, or
Submit personal check or money order to the Welcome Center. Make checks payable to “Madison College,” or
Credit cards and/or cash is also accepted at the Welcome Center.
Applying for an Insurance Waiver Submit Waiver Application form and
proof of current insurance plan to CIE.
Last day to submit a waiver is: June 24, 2013.
If waiver is not submitted before the deadline or is not approved, you will be held responsible for the full cost of the insurance.
Student Insurance Card
Your insurance card has your Student ID number, name, and other valuable information on it.
Keep this card with you at all times!
Show your card upon arrival at a doctor’s clinic or hospital.
Update your correct address and Madison College email at [email protected] or a delay in coverage payment may occur.
How do I pay for a doctor visit? Many claims (bills) will be paid directly
between the insurance company and the doctor if the student uses a network doctor
Sometimes the student may have to pay first and then get reimbursed
Not all services are provided by the doctor Lab work (blood tests, urine tests, etc) is
often sent to a lab and will be a separate charge
Prescriptions are a separate charge

Finding a Doctor/Clinic/Pharmacy:Go to www.lewermark.com 1. Click on “International” and then “Find a
Doctor or Pharmacy” to search for an approved doctor close to you.
2. Choose the GWH-CIGNA Provider network and search by “Specialty” or Medical Condition” to find a provider. “Select a Network” will default to PPO, which is best. Under “Provider” type we recommend Hospitals/Facilities. Under “Specialty” types, we recommend selecting “Medical Clinic, Urgent Care Center, or General Hospital” depending on need.
