ผู้ป่วยชายอายุ 45 ปีCC : แขนขาอ่อนแรงมา 5 วัน PI :7 วัน เริ่มมีอาการชาปลายเท้าและปลายมือสองข้าง 5 วัน ชามากขึ้นถึงเข่า แขนขาเริ่มอ่อนแรง เดินลำบากมากขึ้น ลุกจากเก้าอี้ลำบาก รองเท้าหลุดเวลาเดิน ยกแขนหวีผมไม่ได้ หยิบจับช้อนแล้วหลุดมือ ขับถ่ายปกติ ทานอาหารได้ปกติ ไม่มีไข้ PH : 3 สัปดาห์ก่อน มีอาการท้องเสียถ่ายเหลว ตอนนี้หายเป็นปกติแล้ว

Cerebrovascular disease : Case 1 ผู้ชายอายุ 75 ปีCC : อ่อนแรงซีกขวามา 1 ชั่วโมง PI : 1 ชั่วโมง ขณะทานอาหารมีอาการอ่อนแรงซีกขวาขึ้นมาทันที เดินไม่ได้ ไม่พูด ฟังไม่รู้เรื่อง ปากเบี้ยว ไม่มีอาการชักเกร็งกระตุก PH : เป็น hypertension และ diabetes มา 10 ปี, smoking 10 pack/year เลิกมา 10 ปีPhysical ExaminationsGlobal aphasia, drowsiness, totally irregular heart rate. CN : eye deviate to left side, right facial palsy (UMN) Motor : normal muscle tone Rt Lt Upper Proximal / Distal I / I IV / IV Lower Proximal / Distal I / I IV / IV DTR : 1+ all, Clonus : negative, BBK : negative. ผู้หญิงอายุ 30 ปีCC : ขาอ่อนแรงสองข้างมา 4 วันPI : 4 วันก่อน มีอาการชาขาข้างซ้ายค่อยๆเป็นมากขึ้นเรื่อยๆ 3 วันก่อน เริ่มชาขาขวาด้วย และชาขึ้นมาถึงเอว ต่อมาอ่อนแรงที่ขาทั้งสองข้าง เดินไม่ได้ ปัสสาวะไม่ออก ไม่ถ่ายอุจจาระมา 3 วัน Physical ExaminationsCN : normal all Motor : flaccid tone Rt Lt Upper Proximal / Distal V / V V / V Lower Proximal / Distal II / II II / II DTR : biceps / triceps jerk were normal, knee and ankle jerk were absent both sides. Cerebrovascular disease : Case 2 ผู้ชายอายุ 68 ปีCC : วิงเวียนมา 1 ชั่วโมง PI : 3 ชั่วโมง ขณะดูทีวีมีอาการวิงเวียนขึ้นมาทันที เห็นสิ่งแวดล้อมหมุนไปรอบตัว เห็นภาพซ้อน เดินเซ พูดไม่ชัด กลืนน้ำแล้วมีอาการสำลัก ไม่มีไข้ ญาติจึงนำส่งโรงพยาบาล PH : เป็น hypertension มา 10 ปี Physical ExaminationsAlert, looked distress, dysarthria CN : normal EOM, mild ptosis right eye, pupil Rt 2 mm and Lt 4 mm, react to light both eyes, no facial palsy, decrease movement of right side of uvular Motor : Rt Lt Upper Proximal / Distal V / V V / V Lower Proximal / Distal V / V V / V DTR : 1+ all, Clonus : negative, BBK : negative. Cerebellar sign : intention tremor and dysmetria of right arm Sensory : decrease pinprick sensation on right side of face and left side of body.
ผู้ชายอายุ 28 ปีCC : อ่อนแรงมา 1 ชั่วโมงก่อน PI : 1 ชั่วโมง หลังตื่นนอนตอนเช้า มีอาการอ่อนแรง ลุกจากเตียงลำบาก ยกแขนใส่เสื้อลำบาก เดินเซไม่มีอาการชา ขณะทานอาหารกลืนได้ปกติ ปัสสาวะอุจจาระปกติ ไม่มีไข้ มองเห็นภาพปกติ ไม่มีอาการมองเห็นภาพซ้อน PH : เคยมีอาการแบบนี้ 3 เดือนก่อน เป็นหลังไปทำนา ไปรักษาอาการดีขึ้นจนหายเป็นปกติแล้ว Physical ExaminationsAlert, well cooperative, CLN -ve, thyroid was not enlarged, no thyroid bruit CN : normal all Motor : normal muscle tone Rt Lt Upper Proximal / Distal III / V III / V Lower Proximal / Distal III / V III / V DTR : 1+ all, Clonus : negative, Babinski sign : negative. 2 วันก่อน เคยมีอาการชาซีกขวาเป็นขึ้นมาทันที มีอาการอยู่ 5 นาที หายเป็นปกติ ไม่ได้ไปรักษาที่ใด

ผู้ชายอายุ 42 ปีCC : อ่อนแรงมา 6 เดือน PI : 6 เดือน มีอาการอ่อนแรงมือซ้าย หยิบจับไม่ถนัด ต่อมายกแขนลำบาก ไม่ชา 5 เดือน เริ่มอ่อนแรงแขนขวา ยกแขนลำบาก 4 เดือน เดินลำบากมากขึ้น เดินเซ ลุกลำบาก ไม่ชา ปัสสาวะอุจจาระปกติ กลืนอาหารแล้วไอบ่อย Physical ExaminationsCN : EOM intact, no facial palsy, tongue atrophy with fasciculation. Motor : generalised muscle atrophy with fasciculation Rt Lt Upper Proximal / Distal III / III III / II Lower Proximal / Distal III / III III / III DTR : 3+ all, Clonus positive both sides, Babinski sign positive both sides. Cerebellar sign : can not evaluate due to weakness Sensory : normal.

Cerebrovascular disease : Case 4 ผู้หญิงอายุ 62 ปีCC : อ่อนแรงซีกซ้ายมา 30 นาที PI : 2 ชั่วโมงก่อน มีอาการอ่อนแรงและชาซีกซ้ายขึ้นมาทันที ยกแขนลำบากและเดินเซ ญาติได้บีบนวดให้ 10 นาทีต่อมาอาการดีขึ้นหายเป็นปกติ ไม่มีอาการปวดหัว กลัวมีอาการอีกจึงมาโรงพยาบาล PH : 5 วันก่อนมีอาการอ่อนแรงซีกซ้ายเป็นอยู่นานประมาณ 5 นาที อาการดีขึ้นเอง ปฏิเสธโรคประจำตัว Physical ExaminationsAn elderly woman, alert, well cooperate. Carotid bruit positive on right side, regular heart rate. CN : intact EOM, no facial palsy, tongue was in midline. Motor : normal muscle tone, motor power grade V all, no pronator drift DTR : 1+ all, Clonus : negative, BBK : negative. Cerebellar sign : intact finger to nose, no dysdiadochokinesia, tandem gait normal. Sensory : no sensory loss.
ผู้หญิงอายุ 37 ปีCC : อ่อนแรงมา 3 เดือน PI : 3 เดือนก่อน มีอาการหนังตาตก ตอนบ่ายๆ เคี้ยวอาหารแข็งๆ ลำบาก กลืนไม่ค่อยลง เวลาทำงานหนักจะเหนื่อยง่าย อาการเป็นๆหายๆ มีอาการพูดไม่ชัดบางครั้ง ไม่มีอาการชา ปัสสาวะอุจจาระปกติ PH : ไม่เคยมีอาการแบบนี้มาก่อน, ปฏิเสธโรคประจำตัว Physical Examinations

Cerebrovascular disease : Case 5 ผู้ชายอายุ 59 ปีCC : อ่อนแรงซีกขวามา 1 ชั่วโมง PI : 1 ชั่วโมงก่อน ขณะกำลังร้องเพลง มีอาการปวดศีรษะขึ้นมาทันทีทั่วๆศีรษะ ไม่มีอาการคลื่นไส้อาเจียน ไม่เคยมีอาการปวดศีรษะแบบนี้มาก่อน มีอาการอ่อนแรงซีกขวา เดินลำบาก ยกแขนไม่ขึ้นข้างขวา ร่วมกับมีอาการชาซีกขวา PH : เคยตรวจพบความดันโลหิตสูง 2 ปีก่อนตอนตรวจสุขภาพประจำปี แต่ไม่ได้ไปรักษาต่อเนื่อง ไม่สูบบุหรี่, ปฏิเสธโรคประจำตัวอื่น Physical ExaminationsA middle age man, looked distress, BP 240/120 mmHg. • Right hemispheric lesion : neglect.
• Common Lacunar Syndrome : pure motor, pure sensory, sensorimotor, ataxic hemiparesis, dysarthria clumsy hand syndrome.
• Hypertensive hemorrhage : Putamen, Thalamus, Pons, Cerebellum.
