Definition cholestasis – obstructive jaundice

Cholestatic jaundice should be excluded in all infants with prolonged jaundice

There is currently level 1B evidence that high-dose steroids do not significantly
increase rates of adverse events but that adverse events can occur earlier than patients who do not take steroids.

• Adverse reaction : not significant (esp cholangitis)
report case of CMV meningitis,
GI bleeding associated with Rota virus

Oral prednisolon 5 mg/k/d d5-9 then d10-14
4 mg/k/d tappering to d 30
MCT enrich formula providing 120-150kcal/kg/day
IV Prophylaxis Tazocin and gentamicin for 5 days
Oral cefalexin 25 mg/kg/day for 28 days
Cholestyramine ½ sachet bid given until clearance of
jaundice
Long-term medications :phenobarbitone 15mg/day
Nutritional support
Total caloric need ~ 150 % of RDA ,protein 2-3 g/kg/day
Breast milk with MCT oil ( 7.7 cal/ml) added ,
provide 24-27 cal/ oz
High-energy supplements formula , solid foods
Fat soluble vitamin supplements
