Anatomia Laríngea Normal – Esqueleto Laríngeo
• A base da laringe é a car:lagem cricóide. • A maior car:lagem de suporte é a da :reóide. • A epiglote em forma de lágrima estende-‐se inferiormente e se liga ao parte interna da car:lagem :reóide.
• Somente uma pequena parte da epiglote estende-‐se acima do osso hióide, que consiste a porção supra-‐hióide ou a margem livre da epiglote.

• O ligamento vocal liga-‐se do processo vocal da aritenóide a superGcie interna da car:lagem :reóide, formando o suporte medial da cordas vocais.
• O ligamento ventricular parte superiormente da aritenóide até a superGcie interior da car:lagem :reóide, formando a margem medial da falsa corda vocal.
• A epiglote é man:da na sua posição pelo ligamento hioepigló:co, que atravessa o espaço pré-‐epigló:co.

Anatomia Laríngea Normal – Camada Mucosa e Espaço Laríngeos Profundos • O interior da laringe é dominado por 2 proeminentes
bandas paralelas, as falsas e verdadeiras cordas vocais, separadas por uma estreita abertura que se abre através do ventrículo laríngeo.
• As cordas vocais verdadeiras consistem de músculo, que percorrem paralelo e lateral ao ligamento vocal, entre a car:lagem aritenóide e a car:lagem :reóide, mais conhecido como músculo :reoaritenóideo.
• As falsas cordas vocais consistem principalmente de gordura e entre essas duas estruturas, o ventrículo laríngeo nasce.
• A relação de condições patológicas com essas duas estruturas é fundamental na avaliação do câncer laríngeo.

Anatomia Laríngea Normal – Camada Mucosa e Espaço Laríngeos Profundos • Acima das falsas cordas vocais, a par:r da car:lagem aritenóide, a mucosa se reflete para cima em direção à epiglote, formando as pregas ariepligló:cas.
• A parte da laringe ao nível da falsas cordas vocais é chamada de glote.
• A região abaixo da superGcie inferior da corda vocal verdadeira até a superGcie inferior da car:lagem cricóide é a subglote.
• Acima das cordas vocais verdadeiras está a região supragló:ca.
• No interior desses diferentes níveis, mais subdivisões são dis:ngüidas.

• Em crianças, a densidade tomográfica das car:lagens laríngeas é similar aos dos tecidos moles, de forma que a sua aparência tomográfica é bem variável.
• A epiglote e o processo vocal das aritenóides são compostos por car:lagem hialina, de forma que esta não ossifica.

Carcinoma de Células Escamosas
• É o tumor mais comum da laringe. • Cerca de 65-‐70% dos cânceres de laringe originam-‐se do nível gló:co, cerca de 30% do nível supragló:co e cânceres originários do nível infragló:cos são raros.
• Avaliação imaginológica do câncer de laringe é usado principalmente para detectar extensão para tecido profundos, como o espaço pré-‐epigló:co, a car:lagem laríngeas e base da língua.

Câncer Gló:co – Disseminação do Tumor Primário
• O local mais comum de envolvimento é a porção anterior da corda vocal verdadeira, usualmente na margem livre da superGcie superior.
• Lesões limitadas à comissura anterior são raramente vistas (< 2%).
• Lesões envolvendo a comissura anterior podem invadir diretamente a car:lagem :reóide, a região subgló:ca anterior, o espaço pré-‐epigló:co inferior, como também ocorrer disseminação extralaríngea através da membrana crico:reóidea.
• Espessamento de tecido mole adjacente a neoplasia gló:ca junto à superGcie interna da cricóide pode ser considerado como extensão à região subgló:ca.

• Disseminação lateral do câncer causa infiltração do ligamento vocal e músculo.
• Em um estágio mais avançado, o espaço paragló:co é infiltrado e o pericôndrio da car:lagem :reóide é alcançado.

• O tumor é desviado pela car:lagem :reóide para crescer ao longo do espaço paragló:co, estendendo-‐se cranialmente para região supragló:ca ou caudalmente para a região subgló:ca.
• Um câncer gló:co crescendo inferiormente pode ser desviado pelo cone elás:co lateralmente e para região extra-‐laríngea, indo em direção a abertura entre a car:lagem :reóide e cricóide.
• O envolvimento da car:lagem :reóide usualmente ocorre nas partes ossificadas, mais freqüentemente na margem inferior da car:lagem :reóide, a margem superior da cricóide ou ao nível da comissura anterior.

Achados Esperados Após Cirurgia na Laringe
• Ressecção à Laser: o Irá depender da extensão de tecido ressecado. o Os tecidos moles da laringe podem aparecer normal ou mostrar um defeito focal.
o Após uma ressecção extensa, os tecidos da laringe podem ser subs:tuídos por cicatriz, aparecendo, a superGcie interna, com densidade aumentada, mas homogênea, sendo necessário endoscopia para diferenciar de recorrência.

o Laringectomia horizontal supragló:ca é realizada quando um câncer supragló:co está localizado acima dos ventrículos, de forma que quase toda a laringe acima do nível dos ventrículos é removida.
o A car:lagem :reóide residual é puxada para cima e suturada ao osso hióide.
o Cânceres gló:cos iniciais podem ser tratados como hemilaringectomia, de forma que existem várias variantes desta cirurgia, sendo a mais limitada a cordectomia, onde a corda vocal verdadeira é totalmente removida desde a comissura anterior até o processo vocal da aritenóide.

At 3 months a`er radia:on treatment, clinical examina:on showed pronounced laryngeal edema, but no evidence of tumor. On CT, thickening of the supraglo\c so` :ssues is seen, more pronounced in the le` aryepiglo\c fold (arrowheads); the density within the le` aryepiglo\c fold is also somewhat higher than in the surrounding :ssues. This is a non-‐specifi c fi nding (score 2), warran:ng further imaging follow-‐up.

This was reported as suspicious for tumor recurrence. Direct laryngoscopy was performed but showed no mucosal abnormali:es; biopsies were nega:ve.

Apart from increasing generalized laryngeal edema, the enhancing mass in the aryepiglo\c fold is now extending more anteriorly into the le` paraglo\c space (arrowheads). Also note appearance of small necro:c lymph node in the le` neck (arrow). Direct laryngoscopy, performed aKer the CT study, showed caking of necroEc :ssue over the le` aryepiglo\c fold, suspect for tumor recurrence. Biopsy revealed squamous cell carcinoma.
