Fetma och missbildning-
mekanismer?
• Metabola förändringar: hyperglykemi, hyperinsulinemi, latent diabetes typ II
• Brist på näringsämnen: bantning, näringsfattig kost
• Större behov av näringsämnen ex folsyra
• Kronisk hypoxi, hyperkapné

• They should be counseled on the lower detection
rate and increased residual risk due to poor
visualization.
• The optimal timing of ultrasound is between 18 and
24 weeks
• Experienced sonographers
• Repeat examination and more advanced equipment
may improve visualization
• Using an ultrasound window with the shortest depth
of subcutaneous adipose tissue can improve
visualization

vid högt BMI?
Välkända risker för modern vid sectio:
• Anestesiologiska risker:
intubationssvårigheter, hypotoni,
otillräcklig effekt av LA
• Operationskomplikationer: tekniskt
komplicerat, blödning
• Postoperativa: serom, sårruptur, infektion

Amning
Mothers who are obese before pregnancy
breastfeed their infants for a shorter duration than
normal weight mothers independent of maternal
socioeconomic and demographic characteristics
• Mekaniska orsaker
• Lägre prolaktin nivåer

Maternal and neonatal outcomes after bariatric surgery; a systematic review and
meta-analysis: do the benefits outweigh the risks?
• Primary analysis (comparison of pregnancy outcomes
between women in the surgery group and women in a
nonsurgery group who had similar body mass indexes with
the surgery group before surgery)
• Subanalysis (comparison of pregnancy outcomes between
women in the nonsurgery group who had similar body
mass indexes with the women in the surgery group before
pregnancy but after their surgery).

Pregnancy and neonatal outcome after bariatric
surgery
• Expert opinion recommends post-operative parturients should be
monitored for nutritional deficiencies and supplementation given
as required.
• Expert opinion recommends close observation and regular
assessment of fetal growth throughout pregnancy.
• Large, retrospective studies have found no significant differences
between pregnancies during and after the first year post-operatively.
Nevertheless, expert opinion still recommends avoiding pregnancy
for at least the first post-operative year.
• Experts strongly recommend the early involvement of a bariatric
surgeon when post-operative parturients present with a potential
surgical complication.
Willis K,et al Best Pract Res Clin Obstet Gynaecol. 2015
