¡ 1. ผู้ป่วยที่มีภาวะไตผิดปกตินานติดต่อกันเกิน 3 เดือน ทั้งนี้ผู้ป่วยอาจจะมีอัตรากรอง
ของไต (glomerular filtration rate, GFR) ผิดปกติหรือไม่ก็ได้ ภาวะไตผิดปกติหมายถึงมี
ลักษณะตามข้อใดข้อหนึ่งดังต่อไปนี้0
! ตรวจพบความผิดปกติจากการตรวจปัสสาวะอย่างน้อย 2 ครั้งในระยะเวลา 3 เดือน ดังต่อไปนี้0
▪ ผู้ป่วยเบาหวานที่ตรวจพบ microalbuminuria 0
▪ ผู้ป่วยที่ไม่ได้เป็นเบาหวานที่ตรวจพบ proteinuria มากกว่า 500 มก./วัน0
▪ ตรวจพบเม็ดเลือดแดงในปัสสาวะ (hematuria)0
! ตรวจพบความผิดปกติทางรังสีวิทยา0! ตรวจพบความผิดปกติทางโครงสร้างหรือพยาธิสภาพ0
¡ 2. ผู้ป่วยที่มี GFR น้อยกว่า 60 มล./นาที/1.73 ตารางเมตร ติดต่อกันเกิน 3 เดือน โดยที่อาจจะ
ตรวจพบหรือไม่พบว่ามีร่องรอยของไตผิดปกติก็ได้0

กรณีผู้ป่วยเบาหวาน ¡ 1 ถ้าตรวจพบมีโปรตีนรั่วทางปัสสาวะตั้งแต่ระดับ 1+ ขึ้นไป และไม่มีสาเหตุที่
สามารถทำให้เกิดผลบวกปลอม ถือได้ว่ามีภาวะไตผิดปกติ ! (น้ำหนักคำแนะนำ ++ คุณภาพหลักฐานระดับ 3)
¡ 2 ถ้าตรวจไม่พบโปรตีนรั่วทางปัสสาวะด้วยแถบสีจุ่ม ควรส่งตรวจ urinary albumin/creatinine ratio (UACR) จากการเก็บปัสสาวะตอนเช้า (spot morning urine) ถ้ามีค่า 30-300 mg/g แสดงว่า มีภาวะ microalbuminuria ! (น้ำหนักคำแนะนำ ++ คุณภาพหลักฐานระดับ 2)
¡ 3 ในกรณีที่ไม่สามารถส่งตรวจ UACR ได้ อาจส่งตรวจปัสสาวะด้วยmicroalbuminuria dipstick แทน ถ้าได้ผลบวกให้ถือว่า ผู้ป่วยอาจมีภาวะ microalbuminuria
! (น้ำหนักคำแนะนำ + คุณภาพหลักฐานระดับ 4)
ในกรณีที่ตรวจพบความผิดปกติตามข้อ 1 -3 ควรได้รับการตรวจซ้ำอีกครั้งในระยะเวลา 3 เดือน
หากยืนยันความผิดปกติสามารถให้การวินิจฉัยผู้ป่วยเป็นโรคไตเรื้อรัง
(น้ำหนักคำแนะนำ ++ คุณภาพหลักฐานระดับ 4)หากผลการตรวจซ้ำไม่ยืนยันความผิดปกติ ให้ทำการคัดกรองผู้ป่วยในปีถัดไป
¡ ผู้ป่วยได้รับการทำ TLC เป็นเวลา 3 เดือน แล้วผลเลือดอื่นไม่เปลี่ยนแปลง แต่ตรวจ HbA1C = 9.5 mg/dL
¡ หลังการรักษา เบาหวาน และไขมันในเลือดสูง ตรวจพบปัสสาวะ

¡ ตรวจพบค่า Creatinine เพิ่มขึ้นจาก 1.0 mg/dL เป็น 1.8 mg/dL
¡ ปัจจุบันได้รับการรักษาด้วย ¡ Metformin(500mg) 1 tab OD ¡ Simvastatin (20 mg) 0.5 tab OD ¡ ต่อมา พบว่ามีความดันโลหิต 150/90 mmHg

¡ Patients with CKD are at increased risk of cardiovascular disease.
