SECTION  TWO:  BENEFICIARY  STATUS  To  be  a  beneficiary  of  Tui  Pā  Marae  you  must  fulfill  at  least  one  of  the  following  criteria.    Tick  all  that  apply  to  you.  

    Whakapapa  to  Ngāti  Tumutumu  from  the  tupuna  whare,  Tama-­‐a-­‐Ariki       Whakapapa  to  Ngāti  Rahiri  landowners  as  per  1878  Native  Land  Court  decision       Inhabitant  of  the  district  generally  (i.e.  current  resident/ratepayer  of  Te  Aroha  community  &  District)  

   

SECTION  THREE:  DECLARATION  AND  STATEMENT  TO  COMPLY  WITH  THE  PRIVACY  ACT  1993  I  hereby  declare  that:  1.   I  am  over  18  years  old.    2.   I  make  an  application  to  enrol  myself  as  a  beneficiary  of  the  Tui  Pā  Marae.      3.   I  understand  the  information  I  provide  will  be  held  by  the  Tui  Pā  Marae  Trustees  for  the  purpose  of  maintaining  a  register  of  beneficiaries  and  any  other  purpose  it  shall  determine  from  time  to  time  to  further  the  objectives  of  the  Marae  for  the  benefit  of  Marae  Beneficiaries.    4.  I  am  a  blood  descendant  from  one  or  more  of  the  iwi  with  whom  I  have  declared  an  affiliation  in  section  two;   and/or   I   am   a   current   inhabitant   of   the   district   generally   (i.e.   current   resident/ratepayer   of   Te   Aroha   community   and  district).     5.   The   Tui   Pā  Marae   Trustees,   or   agent   appointed   by   them,  may   use   this   information   to   contact  me   about  Marae  elections   and   other  matters   of   importance   that  may   be   of   interest   to  me   and  my  whānau.    6.   I   authorise   the   Tui   Pā  Marae  Trustees,  where  appropriate,  to  disclose  information  about  me  that  may  be  relevant  to  this  enrolment,  the  maintenance  of  the  register  to  which  this  application  relates  and  any  other  purpose  as  it  shall  determine  from  time  to  time  to  further  the  objectives  of  the  Marae  for  the  benefit  of  Marae  Beneficiaries,  provided  that  request  for  access  is  for  a  lawful  purpose  of  Tui  Pā  Marae.    7.  I  have  the  right  at  any  time  to  request  access  to  and/or  correct  the  information  held  by  the  Tui  Pā  Marae  Trustees  for  the  Register  of  Marae  Beneficiaries.
