Vision
§ Use Person Centered Thinking skills to support Transition § Increase self-advocacy § Improve parent involvement § Insure important “to” and “for” is included
in transition planning § Coordinate planning with districts and adult
agencies
Challenges
Ø Schools belief Ø that they were already doing person-
centered planning by using checklists, web based programs and published workbooks (Been there done that)
Ø The time commitment to use a person centered approach

Required for Facilitators Model
– Observe process with 2 students with varying eligibilities.
– After each assessment the trainees will discuss and evaluate the process

• Upon endorsement, the trained facilitator will receive a certification from The Institute for Person Centered Practices
• All trained facilitators are expected to attend an annual Gathering to prevent drift in content.

Certification Process
• PC-TAP is based on skills from Person Centered Thinking (as developed by The Learning Community for Person Centered Practices) and requires training and modeling to ensure the integrity of the process. A trained facilitator will receive a certificate from The Institute for Person Centered Practices after becoming certified.
Rubric for Evaluation
• No jargon • There is a balance between Important “to”
and “for” • Student’s personality shows • Postsecondary goals and Plan of Action
are based on PC-TAP • There is a clear description of how to “support” the person and not “fix”
