• Date: Tue 25 Jun 2013 • • On Mon 6 May 2013, the patient (male, 52 years) was bitten on his right hand by a small dog on a compound in Port-au-Prince, Haiti • He had cleaned the bleeding wound with water and alcohol. He had not been vaccinated against rabies before the accident and had not sought medical attention afterwards. •

• On 19 Jun 2013 (34 days after the bite), he developed fever and malaise, with headache and a painful right arm. Upon touch, he had dysaesthesia on his right arm and shoulder and on the right side of the trunk. On 20 Jun 2013, he developed hydrophobia and one day later, he developed swallowing problems, with reflex vomiting upon attempting to drink. He was admitted to a local hospital (Diakonessenhuis Utrecht, The Netherlands), where the attending neurologist suspected rabies. unspoilt Carpathian Mountains, with an estimated 4000-7000 animals, according to environmentalists.

• The outbreak in Syria has paralyzed at least 10 children in Del al-Zour province.

• They’re all younger than 2 and may have gone unvaccinated because of the ongoing conflict

• It’s unclear how polio got back into Syria, which had eradicated the virus in 1999, though officials say this bout may have originated in Pakistan.

• Polio immunization rates fell from 91 percent in Syria in 2010 to 68 percent in 2012, largely because of the impact of conflict, ( World Health Organization ).

• Vaccination efforts are focusing on more than 2 million children in Syria, as well as in seven neighboring countries: Iraq, Turkey, Lebanon, Jordan, Israel, the Palestinian territories and Egypt.

• Increased surveillance of sewage in settlements.

• European countries use inactivated polio vaccine, IPV, rather than oral polio vaccine, or OPV

• While the IPV reduces transmission and protects well against paralysis, it only partially protects against infection,.

• Only a handful of European member states allow use of OPV and none has a stockpile of the live vaccine.

• These samples were tested for quality

• 1) thin-layer chromatography to analyze

levels of active pharmaceutical ingredient

• 2) disintegration testing.

Of 713 samples tested, 9.1% failed basic quality testing for requisite levels of API or disintegration.

The failure rate was

16.6% in Africa,

10.1% in India,

3.9% in other middle-income countries. CONCLUSIONS:

Substandard and falsified drugs are readily available in the private marketplace

middle-income countries.

The Commission kindly invites the EWRS contact points in the Member States to provide through the EWRS the following information: 1: If cases of botulism similar to the ones described by Norway are currently observed in your countries. 2: If you estimate that at National level an adequate stocking of botulinum antitoxin is available for specific treatment of patients during an outbreak. • 29/10/2013- Posted by Norwegian EWRS Dear all, This is an update of the EWRS notification posted on 19 October 2013. On Friday 18 October the Norwegian Institute of Public Health was notified of two cases of clinically diagnosed wound botulism in people who inject drugs (PWID). These cases now been confirmed by mice inoculation. As of 28 October, four additional suspected cases have been reported. Date of symptom onset for the confirmed and suspected cases ranges from 1.10.2013 to 25.10.2013. The cases have been reported in four men and two women between the ages of 35 and 50. All cases reside in the Oslo area or in neighbouring municipalities. The cases have been reported in PWID and most have reported intramuscular injection of heroin. Test results are pending for the suspected cases. A botulinum antitoxin shortage in Norway was resolved by obtaining additional doses from the Finnish Institute of Health and Welfare while waiting for shipment from a supplier in Canada. This is the largest outbreak of wound botulism ever identified in Norway. These cases raise the possibility that a batch of contaminated heroin is in circulation. Wound botulism in PWID has been reported from several European countries the last few years. Considering the complex international distribution chain of heroin, the exposure of PWID in other EU Member States cannot be excluded. Further investigation into epidemiological links between cases is ongoing.

• Ten patients had liver biopsy data available at the time of this report; seven had histology consistent with hepatitis from drug/toxic injury, with findings including hepatocellular necrosis and cholestasis.

• One patient died, two patients received liver transplants, and two remain hospitalized; all other hospitalized patients have been discharged.

• Of the 29 identified patients, 24 (83%) reported using

OxyELITE Pro during the 60 days before illness onset. Twelve (41%) reported use of OxyELITE Pro and no other dietary supplement, and 12 (41%) reported use of both OxyELITE Pro and at least one additional dietary supplement.

• Three (10%) reported using other dietary supplements for weight loss or muscle building, but not OxyELITE Pro. Information about use of OxyELITE Pro is not yet known for two (7%) patients.

• For twelve patients with specified dates of use, the median duration from starting OxyELITE Pro to the onset of symptoms was 60 days (range: 7–130).

Drug- and herb-induced hepatotoxicity

• Drug- and herb-induced hepatotoxicity often resolves following discontinuation of the product

• Attributing liver injury to a specific ingredient can be challenging because of multiple ingredients, product variability, and lack of testing to confirm exposure to a product.

• Clinicians evaluating patients with acute hepatitis should ask about consumption of dietary supplements as part of a comprehensive evaluation.

• Clinicians should report patients meeting the case definition to the local or state health department, as well as the FDA’s MedWatch program.

• Clinicians can discuss patient management options with a medical or clinical toxicologist by calling their local poison center at 1-800-222-1222.

• Persons who use dietary supplements for weight loss or muscle gain should do so with caution and under a medical provider’s close supervision.
