Notice is hereby given that the Southwestern Ohio EPC and OMERESA will receive proposals for participating member school districts until September 4, 2012 4:00 PM EDST (Local Time) on the following items:

AUDIO VISUAL EQUIPMENT

Proposals shall be properly and completely executed as indicated in the instructions to bidder. Surety and other required forms/samples as described in the instructions to bidders should be mailed to Southwestern Ohio EPC, 303 Corporate Center Dr., Vandalia, OH 45377 and must be received prior to the bid closing date: September 4, 2012

The Audio Visual Committee reserves the right to waive any information, reject any or all quotations or accept the quotation as a whole or in part which, in their judgment, is for the best interest of the participating school districts of EPC. Quotations will be reviewed by the committee in September and successful vendors notified by shortly after the committee makes awards.

Vendor information forms must be signed by a duly authorized representative of the company submitting the quotation. Please complete the form, sign and return to the EPC by September 4, 2012 at 303 Corporate Center Dr., Suite 208, Vandalia, OH 45377

Each quotation submitted shall contain all information requested on the quotation form. Quotations must be completed on the Excel sheet provided. Quotations not submitted electronically on the provided sheet will not be considered in the award process. Complete information must be furnished to allow the EPC to make accurate analysis of submitted data. Submit completed quotations to dropbox

If there is a minimum order for delivery, this minimum must be so stated on the vendor information sheet.

All prices quoted are for a one-year period beginning September 1, 2012 through August 31, 2013. The EPC and OMERESA have the option to extend this agreement, with vendor agreement, for a period not to exceed (1) year beyond the above expiration date.

ALL VENDORS MUST GUARANTEE THE MANUFACTURER’S WARRANTY OR BETTER.

The price quoted for each item shall be based upon the free delivery to the destination stated on the purchase order after all trade and cash discounts, excise tax, or any other State or Federal taxes have been deducted.

PURCHASE ORDERS Purchase orders will be initiated by individual member districts of the EPC and OMERESA. If your company requires the use of product code numbers on purchase orders, then you MUST provide them on the quotation sheet.

GENERAL SPECIFICATIONS Vendors may list additional products they would like the committee to consider for inclusion on the awards.

For all additional items listed and awarded, vendors must provide a J-PEG image of all bid items. Vendors will have two weeks after award notification to send j-pegs to the EPC. Please send all clearly identified J-PEG images on a CD or as an email attachment.. Hyperlinks (manufacturer’s website) to the spec sheet for each item must be provided for all additional items. Vendors will have two week after the award to send links to the EPC. Links are provided to districts in the on-line catalog for additional information purposes.

Also, vendors will be asked to please provide a description (up to 200 words) of the additional bid items awarded. This will again be provided in the catalog to districts for informational purposes. A clear, concise informative description in the catalog will assist member districts in making informed decisions and drive purchases to awarded vendors.

All items including AC/DC equipment must be provided with a three (3) prong grounded plug – except video equipment. All must be UL approved.

PARTICIPATION FEE: Vendor pricing shall include a (1%) participation fee that will be remitted to the EPC on a semi-annual basis. The vendor will produce and provide to the EPC semi-annual reports ending January 31 and August 30. These reports shall be in Microsoft Excel format and be available in electronic form, shall identify the vendor and the quarter being reported, shall include a minimum of the fields listed below and shall allow for sorting on any of these fields: Date of order, School District, List or academic price sales total, EPC/OMERESA price sales total and Member savings totals.

Semi-annual reports and participation fee payments to the EPC are due the 15th of the succeeding month and all checks are to be made payable to the Southwestern Ohio EPC and sent to: The Southwestern Ohio EPC, 303 Corporate Center Dr., Vandalia, OH 45377

Vendors are encouraged to list any other service, such as equipment repair, that your company will provide which might be of assistance to the participating school districts of EPC and OMERESA.

Contract(s) will be awarded to responsive and responsible vendor(s) whose bid conforms to the solicitation and will be most advantageous to EPC and OMERESA Districts with consideration given to price as well as additional pertinent factors.

DELIVERY Delivery shall be made to one (1) delivery point in each school district as specified by the purchase orders or each member of the EPC. All price quotations are to include delivery. Deliveries are expected as soon as possible but not later than 30 days after receipt of the purchase orders.

INVOICING All invoices shall be submitted to the school district placing the order. No additional charges shall be assessed for invoicing, packing or delivery of goods. Partial payments shall be discouraged unless for some reason delivery on an item or items has been delayed. The vendor shall make every effort to invoice upon completion of delivery of all items listed on the purchase order. EPC and OMERESA member districts may choose to purchase using a purchase order or a credit card with NO penalty for either type of payment.

SUBSTITUTIONS Any substitutions of products without written consent of the receiving school district can be considered just cause for revocation of agreement between the vendor and all school districts, and exclusion from the submission of future quotations.
