Ley Foral 1/2013, de 30 de enero, por la que se crea y se regula el Banco de Sangre y Tejidos de Navarra.
Artículo 4. Objetivos.
El Banco de Sangre y Tejidos de Navarra tiene como objetivos generales:
a) Velar por el cumplimiento del Real Decreto 1088/2005,…
d) Conseguir la autosuficiencia de sangre y hemoderivados en Navarra.
f) Promover la disminución de la exposición a sangre homóloga y hemoderivados, actualizando y divulgando recomendaciones de uso e indicaciones y potenciando otras alternativas a la transfusión, incluyendo todas las modalidades de autotransfusión.
j) Promover programas de formación del personal del Banco de Sangre y Tejidos de Navarra, de carácter continuo, que incluirán programas de buenas prácticas.

Ley Foral 1/2013, de 30 de enero, por la que se crea y se regula el Banco de Sangre y Tejidos de Navarra.
Artículo 5. Funciones.
1. Como Centro Comunitario de Transfusión desempeñará las funciones siguientes:
a) Coordinar las actuaciones de los Centros y Servicios en la Red de transfusión y de células y tejidos humanos de Navarra.
n) Fomentar programas de autotransfusión, ahorro de sangre y esquemas mínimos de transfusión en todos los centros hospitalarios de la Comunidad.
p) Implantar programas de formación del personal vinculado a la hemoterapia

To promote improvements in the quality of the clinicaltransfusion process, which is defined as:
Transfusion of the right unit of blood to the rightpatient at the right time, and in the right conditionand according to appropriate guidelines.
A chain of integrated events that begins with a correct decision that the patient needs blood and ends with an assessment of the clinical outcome of the transfusion. Its goal is to achieve optimal use of blood.

12. Patients should be informed of the known risks and benefits of blood transfusion and/or alternative therapies and have the right to accept or refuse the procedure. Any valid advance directive should be respected.

Los estudios indican que los productos sanguíneos se usan a menudo de formainapropiada, tanto en los países desarrollados como en los países en desarrollo.
Las transfusiones innecesarias y las prácticas peligrosas y los errores de transfusióncomprometerán gravemente la seguridad exponiendo a los enfermas al riesgo desufrir reacciones transfusionales graves e infecciones transmitidas.
El uso innecesario reduce gravemente la disponibilidad de productos sanguíneospara los pacientes que los necesitan.

It is necessary to reduce the unnecessary transfusions. This can be achievedthrough the appropriate clinical use of blood, avoiding the needs fortransfusion and use of alternatives to transfusion.
The commitment of the health authorities, health care providers andclinicians are important in prevention, early diagnosis and treatment ofdiseases/conditions that could lead to the need for blood transfusion.
Blood transfusion is an essential part of modern health care. Used correctly, it can save lifeand improve health. However, as with any therapeutic intervention, it may result in acute ordelayed complications and carries the risk of transmission of infectious agents.

Anemia “inexplicable” debería ser investigada más específicamente y el
procedimiento quirúrgico retrasado, si es posible.
Los déficits hematínicos, aún sin anemia, deberían ser
suplementados/tratados para permitir:• La mejor optimización de la Hb Preoperatoria
• Hastening the recovery from postoperative anaemia.

"Whenever clinically feasible, patients undergoing elective surgery with a high
risk of severe postoperative anaemia should have their haemoglobin level and
iron status tested, preferably at least 28 days before the surgical procedure.
For patients >60 yr old, vitamin B12 and folic acid should also be measured".

Recommendations• Healthcare pathways should be structured to ensure anaemia screening and correction beforesurgery (Grade 2B).• Patients should be counselled about the relationship between anaemia, morbidity and mortality, andshould be given the opportunity to defer non-urgent surgery until anaemia is investigated and treated(Grade 1C).• To avoid causing unnecessary delay to patients, anaemia screening should take place when referralfor surgery is first made, in order to allow investigation and correction if appropriate (Grade 1C).• Where surgery is urgent, whatever time is available before operation should still be used foranaemia investigation and treatment initiation (Grade 1C).

7. Se recomienda la detección de la anemia preoperatoria, puesto que esta asociada a un aumento de mortalidad perioperatoria.
Recomendación fuerte +. Nivel de evidencia alto.
8. Se recomienda la realización de una determinación de Hb en pacientes a los que se va a realizar cirugia electiva, al menos 28 dias antes de la cirugia, lo que permite un tiempo suficiente para la estimulación de la eritropoyesis, si fuera necesario.
Recomendación fuerte +. Nivel de evidencia moderado.
9. Se sugiere que el nivel de Hb preoperatorio antes de la intervencion quirurgicase encuentre dentro de los margenes de normalidad definidos por la OMS (hombres Hb ≥13g/dl; mujeres ≥12g/dl).
Recomendacion debil +. Nivel de evidencia moderado.
10. Se sugiere el tratamiento con FE oral, en pacientes anemicos, durante 14 diasprevios a la intervencion con 200mg/dia de sulfato ferroso; para aumentar la Hbpreoperatoria y disminuir la TSA en pacientes con cancer colorrectal.
Recomendacion fuerte +. Nivel de evidencia moderado. .
11. Se sugiere el tratamiento con FE EV en pacientes anemicos que van a precisar cirugia ginecologica y colorrectal para aumentar la Hb preoperatoria y disminuir la TSA.
Recomendación fuerte +. Nivel de evidencia moderado.
12. Se sugiere el uso de FE EV, en lugar de Fe oral, en aquellos casos en que estese contraindique o el tiempo sea insuficiente.

Respect to control, very short-term perioperative IV iron administration, with or without rHuEPO, significantlyreduced (*p<0.01):No clinically relevant AEs were observed.The scheduled IV iron dose (200-600 mg) may not cover total iron loss, especially in patients with preoperative iron deficiency.Preoperative rHuEPO was only administered in 351 out of 1059 patients presenting with Hb level <13 g/dL and no contraindication.

First Recommendation
The AABB recommends a restrictive RBC transfusion thresholdin which the transfusion is not indicated until the Hb level is7g/dL for hospitalized adult patients who are hemodynamicallystable, including critically ill patients, rather than a liberal threshold when the Hb level is 10g/dL (strong recommendation,
moderate quality evidence).
For patients undergoing orthopedic surgery or cardiac surgeryand those with preexisting cardiovascular disease, the AABB recommends a restrictive RBC transfusion threshold (Hb levelof 8g/dL) (strong recommendation,moderate quality evidence).
The restrictive Hb transfusion threshold of 7 g/dL is likely comparable with 8 g/dL, butRCT evidence is not available for all patient categories.
